O U R V I S I O NEveryone with palliative needs in our community receives responsive, proactive and equitable care.

O U R P U R P O S E• To provide expert and compassionate palliative support,

information and advice to patients and whānau living in the Hutt Valley

• To work collaboratively with all health and social care providers to ensure as many people as possible benefit from our expertise through education

• To improve communities’ understanding and acceptance of death and dying

• To improve and ensure equity and inclusion in our services

• To be true to our values as a good employer

• To be good stewards of resources

W H AT I S PA L L I AT I V E C A R E ?Palliative care supports you and those you care about, to live with a life-limiting illness, where death is inevitable. The Palliative Care Team recognises that from birth to death we all have physical, social, emotional, and spiritual needs that must be valued. Palliative support can happen anywhere - at home, in hospital, in hospice, or residential care.

The Palliative Care Team focus their care on living well at the end of life and regard dying as a natural process. Palliative care values life and does not shorten it. It helps you understand what is happening and what to expect, enabling you to make decisions that are right for you and your family towards the end of life.

T E A W I T H E L I Z A B E T HAn evening spent acknowledging one of the world’s famous pioneers of the Hospice movement: Elizabeth Kubler-Ross.

Stories of Elizabeth’s wisdom shared by those privileged to work closely with her as she encouraged people to “live before they die” and resolve all “unfinished business”.

Let her remarkable work to de-stigmatize dying be remembered and carried on by those that work or have an interest in supporting others at the end of life.

Dying, like birth, is a human event everyone has to go through. Therefore, care of the dying is everyone’s business. Health professionals can ease and support this process knowing it belongs with families in communities.

• What is a meaningful death

• How do we encourage others to live without a cure

• What to expect at the end of life

• What is needed and what helps people at this time

• Preparing for our own death. What legacy do we want

to leave behind?
