Parent Code of Ethics • I hereby pledge to provide positive support, care and encouragement for my child participating in youth

sports by following this Parents’ Code of Ethics: • I will encourage good sportsmanship by demonstrating positive support for all players, coaches, and

officials at all LWYFL and GCYC practices, games, functions and youth sports events. • I will place the emotional and physical well-being of my child ahead of my personal desire to win. • I will insist my child play in a safe and healthy environment. • I will require that my child’s coach be trained in the responsibilities of being a youth sports coach and

that the coach uphold the Coaches’ Code of Ethics. • I will support coaches and officials working with my child, in order to encourage a positive and

enjoyable experience for all. I will demand a sports environment for my child that is free from drugs, alcohol, tobacco, and profanity and I will refrain from their use at all LWYFL and GCYC practices, games, functions and youth sports events. • I will remember that the game is for youth and not adults. • I will do my very best to make youth sports fun for my child and all of the children of the LWYFL and

GCYC. • I will ask my child to treat other players, coaches, fans, and officials with respect regardless of race,

sex, creed or ability. • I will help my child enjoy the youth sports experience by conducting myself in a positive manner that

sets a good example for the children at all LWYFL and GCYC practices, games, functions and youth sporting events, by doing whatever I can such as being a respectful fan, and providing assistance where needed. • I agree to follow all of the rules and guidelines as set down by the LWYFL and by the GCYC, to which

my organization belongs, and abide by the interpretation and/or decisions handed down by those organizations. I/we hold harmless the LWYFL Board of Directors for any decisions or interpretations they may make regarding the status of my continued participation/membership or the participation/membership of the registered participant.

Background Checks Back ground checks are to be completed by all team parents. The GCYC screening policy is as follows:

GCYC Background Screening Policy

1. In order to protect the youth of the Gem City Youth Conference (GCYC) and to raise the standards of

GCYC, a criminal background check is mandatory for all members of the Sideline Roster that are 18 years of age or older, and Board Members. Any person designated by the head coach as an assistant or any volunteer who shall be alone with players, or responsible for a team during a game or practice. It is the policy of GCYC that at any GCYC sanctioned game, practice or event, there must be at least 1(one) adult present who has successfully completed a criminal background check. If there is not 1 (one) adult present who has completed a GCYC Background Check, the game, practice or event must be halted or postponed until such an adult is available.

2. An authorization, either written or online, allowing the GCYC to order an individual criminal background

check is required by the GCYC. It is the responsibility of the candidate for the volunteer or coaching position (the “Applicant”) to fill out and submit the GCYC background check consent form and return it to their Conference Representative, or as otherwise as directed. Failure to provide a background check consent form will disqualify the applicant from consideration for their volunteer or coaching position.

3. The Background Screening “Committee” shall administer all background screening procedures. The Committee shall be made up of the current GCYC Executive Board. The Conference Representatives shall receive the completed background screening consent forms (electronically or hard copy) and forward them to the Background Screening Committee or notify the Background Screening Committee that the screening process has been completed online. The Committee shall be responsible for transmitting the forms (if necessary) to the reporting agency selected by the Board. The Committee shall receive and review the results of the background screening reports from the agency. The Board will address reports if the individual does NOT meet the GCYC minimum standards for coaches using the following criteria: Individual applicants (“Applicants”) will be reviewed by the Background Check Committee and may be disqualified from positions as a Sideline Roster member if they have ever been found guilty or entered a plea of guilty or nolo contendere, regardless of the adjudication, for any of the following:

A. Any crimes against children B. Any conviction involving violence C. Any sexual offense Should any pending charges described in “A” through “C” be uncovered, or should any charges be brought against an Applicant during the season, the Applicant will be reviewed by the Background Check Committee and may be suspended from serving as a volunteer until such time as the charges have been cleared or dropped and he/she is reinstated by a majority vote of the Board of Directors. In addition, Applicants will be reviewed by the Background Check Committee and may be disqualified if they have been found guilty or entered a plea of guilty or nolo contendere, regardless of the adjudication, for any of the following within the past seven (7) years:

D. Any Felony Conviction

E. Any Drug Related Conviction

GCYC Background Screening Policy In addition, Applicants shall be reviewed by the Background Check Committee for possible disqualification if they have been found guilty or entered a plea of guilty or nolo contendere, regardless of the adjudication, for any of the following within the past five (5) years:

F. Any crimes of moral turpitude or lack of integrity and/or character of an Applicant, as

determined by the Background Check Committee

G. More than one alcohol related conviction

4. The Background Check Committee and/or the background screening service shall notify the affected Applicant in writing (or electronically) that a disqualifying entry was reported on the criminal background check and present the background report. From the date that a determination is made that the applicant is disqualified, he or she can no longer perform the duties of that position. In the event the Applicant feels a mistake has been reported in their criminal background check, it is the Applicant’s responsibility to contact the reporting agency and resolve any issues. The Applicant has the right to appeal the decision made by the Background Check Committee. Included with the notification letter will be a copy of the Appeal Process. Failure to adhere to the process set forth as indicated in the Appeal Process will render the Background Check Committee’s decision as final. 5. All information in response to the criminal background check or disclosed in the review process shall

be kept confidential and not disclosed or discussed outside of the review process. The GCYC and its members are not responsible for errors or omissions that may be reported on background checks. The Background Check Committee of the GCYC shall maintain all authorizations and records or reports in a confidential manner.

6. All background checks will expire one year from the date of submission to the reporting agency. They

will automatically recheck on the one year anniversary, making the background check good for 2 years.

7. The Applicant may, at his option request a hearing of the Background Check Committee. The request

for a hearing shall be made on the approved APPEALS REQUEST FORM (either manually or electronically filed) and tendered to any member of the Executive Board within forty-eight hours of receiving the notification that a disqualifying event has been discovered. Failure to timely request an appeal will cause the determination of the Background Check Committee to become final.

Upon receipt of the APPEALS REQUEST FORM, the Secretary shall deliver the complete background check to the Background Check Committee and the APPEALS REQUEST FORM. The Background Check Committee shall schedule a hearing within 14 days of the request and inform the Applicant of the time and date of the hearing. At the hearing, there will be a time limit of fifteen (15) minutes for the Applicant to present any argument that the determination of the BACKGROUND SCREENING COMMITTEE should be modified. The Background Check Committee will review the information and render a final decision. If the Chairman is not available, the Co-Chair will take his/her place. For an appeal to be successful, a majority vote of the Committee is required. In the case of a tie, the appeal is denied. Any determination made by the Background Check Committee shall be final.

LWYFL Team Day Background: LWYFL is a 100% Volunteer Organization. Each game day requires volunteers to make the day run smoothly. Approximately 320 children will participate in the LWYFL 2020 season. We all want to watch our kids play. Does not include the chain-gang, clock, spotter, and announcer volunteers required for each game.

Team Day Concept:

Each team is responsible for (1) Team Day during the season. The term ‘Team’ includes BOTH football players and cheerleaders. A Signup genius will be posted online, and parents are responsible for filling the volunteer positions.

Football and Cheerleader parents/guardians MUST volunteer (1) 3 ½ hour shift per child participant during the season. Head Coaches are required to be present for the duration of their assigned Team Day. There will be a minimum of two LWYFL Board Members present during all shifts.

A shift may be completed by anyone over 18 years old on behalf of the player/cheerleader. The adult volunteer doesn’t have to be the child’s parent. Coaches are also required to complete a Team Day shift if they have a child in LWYFL.

Those parents that do not sign up for their assigned Team Day will be Responsible for finding another shift with another team. If a team does not have enough participants to fill all the volunteer slots, other volunteers will be assigned by the LWYFL Board. If a player/cheerleader volunteer does not show for their scheduled shift, the player/cheerleader will not play or cheer on game day until the 3 ½ hour shift commitment is met.

Team Day assignments will be assigned once the GCYC publishes the Season Schedule. Teams are required to submit their Team Day Roster with contact info to the LWFYL.

Team Day schedule to be provided as soon as GCYC schedule is posted.

Required Positions: (7 per Shift) *Positions assignments are NOT guaranteed. Adjustments may be necessary and directed at the discretion of the LWYFL shift board member.

Concession Stand – 4 (Children are NOT allowed in the concession stand.) Grill – 2 Gate – 1

Shift Schedules: (Times are approximate and will be set when game schedule comes out) Sample schedule based on a 5 game day 1st Shift 10:30am to 2:00pm 2nd Shift 3rd Shift

Incident Reports In order to better serve the youth of the Lebanon Warrior Football League, the LWYFL has made an incident form available to make your comments heard. The LWYFL will use the form as an official notification for any incident that may be deemed as in appropriate concerning coaches, players and spectators due to a breakdown in sportsmanship or due to rules violations.

This form is available on the LWYFL website: www.lwyfl.org

Sideline Announcing Sheet You will want to have an announcing sheet made available for all games. Make several copies of the announcing sheet. One copy to leave at the home field and copies for the away games. Make sure there are NO nicknames. Helpful hint, it is beneficial to have both the cheer and football on one sheet and use phonetic spelling as needed.

LWYFL Fundraiser - Warrior Cards

Team moms will be responsible for the distribution and collection of the cards/money. ALL WARRIOR CARDS MUST BE SOLD. REGISTRATION FORM CLEARLY INDICATED SALE OF WARRIOR CARDS IS REQUIRED.

Volunteers - Home Games It is the responsibility of each home team to provide 6 volunteers at each home game for the following positions: 3 for chain gang 1 spotter 1 announcer 1 to run the clock/scoreboard

Football Paperwork - Binders: Before our season starts, the players paperwork, team roster, coaches roster and hardship letters will be completed and placed in a Team Parent Binder. DO THIS AS EARLY AS POSSIBLE! Coaches will receive items 2 & 3 below electronically and should fill out and print forms. Physicals must be handed in prior to the start of practice. Paperwork to be included is as follows: 1. Hardship letters (attach letters from each player listed) 2. Team Roster (Jersey # order, NOT ALPHABETIC), 3. Sideline Roster (List Head Coach first) 4. Players Paperwork- physical & copy of birth certificate 5. Signed Volunteer Policy, Concussion and USA Football Certificates for coaches. 6. Form with Parent Signature acknowledging watched Lindsay's Law video, read

SCA form, Concussion form and parent code of ethics.

It is imperative that physical forms are filled in correctly. NO child may participate if the forms are not handed in to the coach prior to the start of practice. PLEASE ENSURE the “Date Performed” of the child’s physical is AFTER January 1 of the current year. No physicals from the previous year will be accepted.

Once you have checked each physical form to ensure it is completed correctly, you will need to write that players game jersey number in the upper right-hand corner and circle it. Check to make sure the players number is clear and legible. Do this for every player on the team.

Do the same, Jersey number written/circled, with the 1 copy of birth certificate. Birth certificate should have a state seal or stamp, “live birth”/church or announcement type documents will not be accepted.

Once you have each players paperwork paper clipped you will put them in order by game jersey number from the least to greatest. The team will need 1 copy of their player roster and Sideline roster. Sideline roster must also be signed by coaches and team mom’s, with USA Football date of completion.

All teams must have their team binders by July 22nd. The player physical form and coaches USA

Football number and background check MUST be completed prior to the first day of practice. NO PLAYER MAY PRACTICE WITHOUT A PROPERLY COMPLETED PHYSICAL FORM, PERIOD. NO COACH MAY BE ON THE FIELD WITH KIDS WITHOUT BACKGROUND CHECK and CURRENT USA Football number, concussion certificate, signed conduct policy. Board Members will be at every practice site the first couple of days to assist with parents and paperwork questions or issues.

Physical Form (Football and Cheer) • WE must have on hand for first day of practice! or NO PRACTICE - NO EXCEPTIONS • All spaces/blanks need to be completely filled in • Check grade and school - School District is Lebanon City Schools. The Name of the school will very

Birth Certificate (Football Only) • Each player will need to have 1 copy of an original birth certificate. Please check the name on birth

Once all forms AND jersey numbers are assigned:

• Place the player’s number in the upper RIGHT corner of the corresponding physical form and birth

certificate • Place in numerical order - lowest number to highest

You will eventually have a copy of each set on hand at all games. It is suggested to have a notebook that will contain the player’s physical forms, birth certificate, sideline roster and team roster. Any team can request to check a player’s information during a game.

Hardship Letters/ Weigh ins Each and every football player will be weighed at Weigh ins!

In the event that a player cannot be at the mandatory weigh in meeting, a hardship letter will need to be written by the parent of the player and turned in by the day which your team is scheduled for pictures. You can turn these in with your paperwork.

Hardship weigh ins are listed on Important Date handout. The Head coach will accompany the player to the weigh in and take the team roster with them.

Recertification weigh ins are listed on Important Date handout (for those players close to weight for ball carrying).

Sideline Notebooks Team Rosters: Due one week prior to weigh ins. • All information will come from the individual player’s physical forms. • Put all information by player jersey number - low to high. (Make sure your coach is settled with each

jersey number, once the team roster is filled out you cannot switch numbers without redoing all paperwork.)

Sideline Roster: Due by your team’s picture time. • This roster includes any individuals that will be on the sideline during games as per coach request

(coaches, assistant coaches, water boys, team moms, play counter etc.) • Limited to 10 • All need to have a background check (exception for under the age of 18) • All need to have a USA Football number (exception for under the age of 18) • Signatures required

Physicals and Birth Certificates: • When completed this will be added to your notebook • Forms need to be available at all games

Minimum Play Sheets 1. A minimum play sheet must be complete for your team each and every game you play.

2. You must write your complete team name at the top of the form including grade (i.e. Lebanon 4th

Maroon).

3. Each player on your roster must be listed on your minimum play sheet even if they have quit or they are not playing due to absence, injury, etc.

4. All players must be listed on the minimum play sheet in NUMERICAL JERSEY ORDER - low to

high.

5. If a player quits or is not playing you must draw a line through his name and extend the line through all ten play boxes. In the comment section to the right you must state the reason the player will not be playing.

Tracking and Submission 1. The head coach is required to sign the minimum play sheet at the conclusion of the game

acknowledging that it is accurate. 2. FINAL score is to be written on the minimum play sheet. 3. The sheet must be turned in at the location where you are playing. (The home field for the day) 4. Failure to turn in your minimum play sheet will result in an automatic suspension from the LWYFL.
