    5 workers were in the

    process of dismantling a

    tower crane from a

    height of 16 stories

    During preparatory

    works for jacking down,

    the crane jib section with

    its slewing unit toppled

    to the ground

    One erector fell together

    with the crane jib section

    & died at the scene. 3

    others were injured.

    5 workers were installing a trolley

    platform to the jacking cage of tower

    crane to prepare for jacking up of

    crane.

    Trolley platform was being hoisted intoposition when it hit the adjacent work

    platform with a worker on it.

    Work platform became dislodged and

    fell 55m to the ground with the worker.

    The worker fortunately landed on a

    waste plastic bin which cushioned his

    fall. He survived the accident and is

    undergoing treatment for his injuries.

    Only Approved Crane Contractors are allowed to perform any

    erection, jumping/jacking & dismantling of Cranes

    Adequate Risk Assessment must be carried out prior to the

    commencement of work

    Risks & SWP must be communicated to all involved Must be carried out in accordance with manufacturers instructions

    as provided in the manufacturers manuals

    Must be carried out only by competent erectors under the direct

    supervision of a competent supervisor. Other works and vicinity at affected areas must be no-entry zone

    Demolition work;

    Excavation and trenching work in a tunnel or hole exceeding 1.5m in depth;

    Lifting operations involving tower, mobile or crawler crane;

    Registration of Crane Operator

    No person shall operate a mobile crane or tower crane in a

    factory unless he is registered as a crane operator under these

    Regulations and is the holder of a valid certificate.

    No person shall employ or cause or permit any person who isnot registered as a crane operator under these Regulations to

    operate a mobile crane or tower crane in a factoryNote:

    a) To drive a mobile crane - must have the relevant driving license

    b) To operate a mobile crane must be registered crane operatorc) Lorry loader operator not required to be registered if purpose is

    mainly to transport materials or goods. However, operator must

    still be trained and competent to operate lorry crane safety.

    (a) co-ordinate all lifting activities;

    (b) ensure that only registered crane operators, appointed riggers and appointed

    signalmen participate in any lifting operation involving the use of a mobile crane or a

    tower crane;

    (c) ensure that the ground conditions are safe for any lifting operation to be performed by

    any mobile crane;

    (d) unless he is present to supervise a lifting operation, ensure that there is a set

    ofsafe lifting procedures for any lifting operation by a mobile crane or a

    tower crane;

    (e) briefall crane operators, riggers and signalmen on the safe lifting procedures referred toin sub-paragraph (d); and

    (f) if any unsatisfactory or unsafe conditions are reported to him by any crane operator or

    rigger, take such measures to rectify the unsatisfactory or unsafe condition or

    otherwise ensure that any lifting operation is carried out safely.

    Findings of Joint Ops Woodpecker

    on 28 July 2010 Workers carrying out tree-pruning activities in man-bucket

    were provided with necessary safety equipment (eg safety

    harness, safety gloves, glasses etc).

    All the lorry crane operators checked had undergone either a

    structured training course or OJT.

    Operators had also extended the outriggers of the lorry cranes

    to ensure stability.

    Contractors were generally aware of the need to demarcate the

    work zone through the placement of appropriate cones,

    barriers and warning signages, as stipulated in LTA's Code of

    Practices for Traffic Control at Work Zone. However there

    were sites inspected where the cones, barriers were found to

    have been placed too close to the work area.

    Some sites inspected did not conduct RA implement a PTW for

    lifting operations involving lorry cranes.
