They are three areas that you can
address on a support plan
1- BehaviouralTask2- Centred approach3- Therapeutic Groups
Behavioural Task
Approachidentifying behaviour thatneeds to be changed.
and the desired behaviour thatthe individual is hoping to achieve.
Do this my praising or rewarding good behavior.
Task CentredThis approach involves identifying
particular tasks orskills that the individual needs to
achieve. The task centered approach centers on supporting the individual
to achieve the task for themselves and therefore
develop independence in that particular area.
Therapeutic Groups
These are often used when a number of individuals
require similar support in a particular area. 1- to improve self-confidence
2- for more specific care as such asovercoming alcohol addiction or learning anger
management skills.

Case Study 1 - 10th september 2016
Mary is 84 years old and lives in a care home full time. She is a very lively and happy lady. Her general health is
great she has no problems doing daily
activities. Her care plan is minimal with just doctors involved for general check ups.
Case Study 1- 10th November 2016
Mary is 84 years old and lives in a care home
full time. She has stopped eating and
socialising so the carers are extremely worried.
Therefore they are going to update and change Mary’s care plan to prevent her
becoming ill.
First step
would be to
assess Mary to find out how at risk she
is? (How much
weight has she lost?)

1- What is the resident's normal
weight?

From the information the resident can then be identified as low, moderate or high risk of malnutrition (undernourished).
2- How tall is the resident?
3- Has the resident lost weight unintentionally in the past 6 months?
4- Has the resident had an extremely poor appetite recently?

The case study has all information that is required for the MUST screening tool
Your job is to:
1. Identify previous BMI 3- 6 months ago2. Work out current BMI of individual
3. Identify any disease or illness4. Use the tool to add the scores
5. What is the total score?6. Action points (Use table below)
