Prior to taking the SAT, students will be asked to complete a voluntary Student Data Questionnaire that will collect information about their grades, interests, intended major and college plans, among other things. Students are not required to answer this questionnaire, but those that fill it out will have the opportunity to opt in to the College Board’s Student Search Service . This service allows participating colleges and nonprofit scholarship organizations to better find specific student groups to share information about the opportunities their organizations provide. The College Board will only share data with eligible colleges, universities, and scholarship & educational opportunity programs that have signed a license agreement, and consistently monitors their use of student information for compliance. Student scores are not shared through the Student Search Service, and students may cancel their participation in this service at any time. A copy of the Student Data Questionnaire has been included to give parents an opportunity to review and discuss the questionnaire with their child prior to the April 11 th test date. Students and families have complete discretion regarding any voluntary information they provide. Completion of the Student Data Questionnaire and participation in Student Search Service are optional and voluntary for all students. School DayStudent Data Consent FormThe Colorado Department of Education is offering all 11th grade students attending public schools inColorado the opportunity to participate in the SAT® exam.The SAT is aligned to the Colorado AcademicStandards and measures the skills and knowledge that are essential for college and career readiness.

The College Board, the provider of the SAT, gives students and families complete discretion regarding anyvoluntary information they provide. Either before or after testing, you will have the opportunity to completethe Student Data Questionnaire (SDQ).The SDQ consists of basic information about you (name, registrationnumber, school, state student ID, sex, and date of birth) that is required to match your questionnaire to yourSAT registration, along with optional questions regarding yourself and your interests. Information gatheredfrom the optional questions will be used for a variety of purposes, including analysis to ensure that theexams are fair for all groups, for group reporting, to connect you with potential colleges or scholarshipopportunities, and for research.

One of the primary benefits of completing the Questionnaire is to opt into Student Search Service®, whichhelps connect you to college and scholarship opportunities that align to your interests. If you choose tocomplete the survey, a minimum number of fields are required.The data you provide will be added to yourCollege Board student record, even if you choose to not participate in Student Search Service. Additionalinformation about each field can be found in the Registration and Questionnaire Booklet distributed by yourschool and located at collegeboard.org/colorado.Your responses will:

• Give your counselors and college admission officers information to use in assisting you with makingplans for the future.The more information you provide, the more assistance they may be able to give you.

Provide information to the College Board that helps to ensure that the SAT is a fair and accurate test forall students.

Return the completed and signed letter to your school.Your teacher will assist in identifying the optional questions when completing the SDQ, and you will receivea copy of the SAT Student Data Questionnaire Supplemental Instructions for Students again when youcomplete the SDQ.

This document is to assist you in completing the SAT Student Data Questionnaire. Completing the Student DataQuestionnaire is voluntary and optional, but it is recommended that you consider doing so.To participate in thequestionnaire, you must complete the Student Data Consent Form and return it to your school along with theseinstructions, indicating that you have reviewed the supplemental instructions and agree to share your information.If you consent to completing the questionnaire, you will receive a copy of these instructions again for use whencompleting the questionnaire.
