EHC plan Review Form 2018–2020This form must be word processed and emailed as a Word document securely to [email protected]

For each child or young person for whom the Local Authority maintains an Education, Health and Care (EHC) plan the education provider should gather advice, hold a meeting and complete this review form on at least an annual basis, recommended six monthly for a child under 5. Reviews must focus on the child or young person’s progress towards achieving the outcomes specified in their EHC plan.

This form is to be used for all reviews of the EHC plan.

Before the EHC plan review meeting

The education provider must complete Part 1: sections 1 to 9 (boxes in green marked ) of this EHC plan review form in advance of the review meeting.

Ideally three and at least two weeks before the EHC plan review meeting date. The educational setting must collate and send to all those invited to attend the EHC plan review meeting copies of:

The completed Part 1 of the EHC plan review form

The child or young person’s contribution to their EHC plan review The parental contribution to the EHC plan review The reports provided by all the professionals invited to the EHC plan review

Following the EHC review meeting

Once the EHC plan review meeting has been held, the educational setting must ensure the whole document is fully and clearly completed (boxes in yellow). A copy of the completed EHC plan review form must be sent to everyone invited to the review meeting within two weeks of the meeting. A copy of the completed EHC plan review form and a copy of all of the collated reports, contributions and supporting evidence must be sent to the Statutory SEN Service within two weeks of the meeting.

So that the EHC plan review form contains the required and appropriate information, please refer to and follow the EHC plan review guidelines which can be found in the Special Educational Needs File on the Cornwall Council website: http://www.cornwall.gov.uk/education-and-learning/schools-and-colleges/special-educational-needs-file/ Throughout this document the term parent/s is used to refer to the person/s with legal parental responsibility and may include carers or guardians.

Post 16 transport (information only)

Beyond Year 11, the Local Authority does not have a statutory duty to provide home to school transport free of charge for children and young people. However, the Local Authority provides a subsidised transport scheme for young people to access post-16 education. All young people, including those with an EHC plan must apply for Post 16 transport and make the required contribution.
