o The array of services started with the screening/ assessment process, continued through treatment to conclusion of care and separation from provider. Screening/Assessments started with the evaluation of the psychosocial

level of functioning and the individual’s needs, and then progressed to the treatment planning/goal setting stages. This stage included the identification of the patient’s problems, strengths, weaknesses, coping skills and what assistance was needed. Collaboration with the patient, family, referral source or other pertinent agencies to further identify the individual’s psychosocial needs occurred, as allowed by HIPAA rules.

By utilized the knowledge of human development and behavior (physical and psychological) combined with the understanding of the different influences of environment, society and culture a mental health diagnoses, employing the most current DSM, was made. A person’s disabilities, which included acute/chronic and traumatic illnesses/injuries (using the formal medical terminology of the DSM) influenced the plan of services.

Treatment plan was designed using all the information gained in the assessment phase, adding in the person’s developmental status, the DSM diagnosis and the person’s goals.

Eclectic approach was used with focus on Behavioral, Cognitive, Cognitive Behavioral, Supportive and Structural therapies with families, groups and individuals.

Prevention. Family education regarding Substance Abuse and Addiction in the family Individual and group counseling to veterans, and their family, for veterans

diagnosed with PTSD and other readjustments to civilian life. Case management services including advocating and assisting clients and

their families address various social issues such as housing, financial, health and legal issues by helping them complete forms, write letters, make phone call, etc. required to obtain assistance from other agencies that were responsible for providing assistance in these matters. Also made follow up phone calls to clients to ensure that their needs were being met, and to the other agencies to provide information when necessary.

Collaboration with Department of Veteran Affairs, community agencies, and community/government social services agencies to provide treatment to military families

on marital, family issues and bereavement issues when needed. Group domestic violence intervention program using The Duluth Domestic

Abuse Intervention Project model. Program consisted of 26 weekly programmed sessions with optional 26 week follow up supportive program.

Anger management groups lasting 27 weeks with optional follow up supportive groups

Worked with judicial system referrals (probation, parole, incarcerated) with positive outcomes of treatment measured by officers of the above divisions based on recidivism.

Developed a resource list for all clients utilizing community resources and available literature

Provided education and health promotion related to mental illness and self care in community outreach programs and for existing clients.

For all clients, an individual crises intervention plan was developed to reduce emergency room visits or police interventions

Crisis interventions were available and did include situations in domestic violence and suicide prevention as part of the clients individual crises plan

All DSM diagnoses, except eating disorders, pedophilia, and pyromania, were treated.
