With the use of predictive models and enhanced data sources, we’ve been able to eliminate the telephone interview process and random holdouts, increase the maximum face amount and decrease most rates. Accelerated Term 2.0 also features some of the best pass-through rates in the industry.

• Life Promises Term is a traditionally underwritten term product for those who do not qualify for accelerated underwriting or require face amounts higher than $1,000,000. All six underwriting classes are available, and standard nontobacco rates include chewers, pipe users and cigar smokers. It’s ideal for clients seeking a customizable life insurance solution; multiple riders can be added including an Additional Insured Term Rider, Term Insurance Rider for Children, Accidental Death Benefit Rider, Accelerated Death Benefit Rider, Terminal Illness Rider, Seat Belt Benefit Rider, Unemployment Benefit Rider and Disability Income Rider in states where approved.

EXCHANGE OR RENEWAL OPTION

The policyowner can elect to exchange or renew the policy at any time during the first term period described below:

EXCHANGE OPTION

If the insured can provide full evidence of insurability, the policy can be exchanged for a new term policy. The premium will be based on the then current rate at the attained age. Exchange is not available if the insured’s age exceeds the maximum issue age of the plan at the end of the first term. If the exchange is not applied for, the original policy can be continued by paying the renewal premium, as outlined in the Renewal Option.

RENEWAL OPTION

After the first term period, one year renewable term periods will be allowed until the policy anniversary nearest age 95 without evidence of insurability.

CONVERTIBLE

This base term policy may be converted without evidence of insurability to a permanent whole life or flexible premium adjustable life policy designated by the company on the date of conversion. The amount cannot exceed the face amount of the term policy. Conversion may be made prior to the earlier of the end of the first term period or attainment of age 75. If the insured is receiving disability waiver of premium benefits, the policy will automatically convert to an ordinary life policy at the end of the conversion period and premiums will continue to be waived.

REINSTATEMENT

The policy may be reinstated within five years after lapse while the insured is living subject to the following conditions:1. A completed application for reinstatement is received. 2. The insured is insurable at the same underwriting class as originally issued. 3. All requested premium is paid upon approval of reinstatement. Premium is not to be submitted with the application

for reinstatement.4. The suicide exclusion and two year contestable period will both commence again with the date of reinstatement.

If applying for Standard Plus, Preferred or Super Preferred rates, a paramedical exam is required in addition to the full blood profile and urinalysis. RIDERS (AVAILABLE IN STATES WHERE APPROVED)

RIDERS MUST BE ADDED AT THE TIME OF APPLICATION; NO RIDERS CAN BE ADDED ONCE THE BASE POLICY IS IN FORCE.

ACCELERATED DEATH BENEFIT RIDER – ELR246In the event the insured is diagnosed with a terminal illness, the policyowner can receive benefits up to 75% of the face amount to a maximum of $250,000. This rider is only available in CA, CT and FL; it is automatically available at no charge in these states.

ACCIDENTAL DEATH BENEFIT RIDER – ICC16ELR020/ELR020This benefit will provide additional funds in the event of accidental death. Not approved in CA.The company will pay:

1. An amount equal to the face amount of the life insurance policy but not more than $500,000.

2. An additional amount equal to the lesser of the face amount or $50,000 if death occurs while riding as a fare-paying passenger in a taxicab, street car, bus, train, airplane or other vehicle operated as a public conveyance by a common carrier.

Issue ages are 18-69. Benefit expires on the policy anniversary following the insured’s 75th birthday.

ADDITIONAL INSURED TERM RIDER – ICC19ELR026/ELR026An insurance benefit can be added for the same plan as the base term plan. Additional insured can be the primary insured’s spouse, children age 18 and over, partner/business owner or other individual where there is an appropriate insurance relationship to the policyowner.

The same premium rates and features as the base plan are applicable except no policy fee is added to the rider.

If the death of the primary insured occurs or the policy matures while this rider is in effect, coverage for any additional insureds may continue as individual policies.

DISABILITY INCOME RIDER – ELR009The Disability Income Rider is available when the base plan plus primary insured term rider face amount is at least $50,000. Issue ages are 18-60. Benefits will be paid if insured is totally disabled and unable, due to sickness or injury, to perform all of the material and substantial duties of his or her regular occupation. Disability income benefits are not coordinated with Social Security or Workers’ Compensation. Benefits are payable for up to two years with elimination period options of 30, 60 or 90 days before benefits are paid. A Terminal Illness Benefit is also included. Refer to Disability Income Rider Agent Guide (EMM122) for complete rider information and rates.

TERM INSURANCE RIDER FOR CHILDREN – ICC16ELR022/ELR022An amount of term insurance for each dependent child 14 days to 17 years old may be added to any parent’s policy by addition of this rider. Dependent children include legally adopted and stepchildren. Each additional child is automatically insured when 14 days old at no increase in premium. Insurance continues until the child’s 25th birthday or the policy anniversary following the 65th birthday of the insured, at which time conversion is available up to $5,000 of each $1,000 of rider coverage without evidence of insurability on or within 60 days prior to the 25th birthday of the covered child. Please notify the Home Office when each additional child is 14 days old and eligible for coverage under this rider.

TERMINAL ILLNESS RIDER – ICC15ELR018/ELR018Under the terms of this rider, the company will pay the policyowner up to 75% of the benefit basis, less the same percentage of the policy loan, all discounted for one year using the accelerated death benefit interest rate as of the date we receive written proof from the policyowner. The company will pay upon receipt of due proof that the insured’s life expectancy is 12 months or less from the date the acceleration is requested. The benefit basis for the primary insured is defined as the policy death benefit of the policy to which this rider is attached, excluding any increases to the death benefit after the policy issue date; and, the death benefit provided by any additional insured benefit rider covering the primary insured of the policy to which this rider is attached. For any other insureds covered by an additional insured benefit rider of the policy to which this rider is attached, the benefit basis is defined as the death benefit provided by an additional insured benefit rider covering that other insured person. The accelerated benefit is only paid once and is a lump sum. This rider is automatically included without charge in states where approved. Not approved in CA, CT and FL. Have the proposed insured sign and keep the Disclosure Statement ICC15EUN053/EUN053.

SEAT BELT BENEFIT RIDER – LP740An additional benefit amount will be paid if the company receives satisfactory proof the insured died while driving or riding in a private passenger vehicle in a covered accident. The vehicle must be equipped with seat belt(s) and the seat belt(s) must have been in actual use by the insured at the time of the accident. The additional benefit amount will be 10% of the amount of insurance shown on the policy specification page up to a maximum of $10,000. This rider is automatically included without charge in states where approved. Not approved in AK, CA, CT, DC, GA, HI, MD, MA, MT, OR, PA, TN, UT, VA and WA.

UNEMPLOYMENT BENEFIT RIDER – AML8856Monthly premiums for a period of up to six months will be waived if the insured is unemployed for a period of 90 consecutive days, receiving state unemployment compensation and the period of unemployment begins at least six months after the policy effective date. The maximum lifetime benefit for this rider is an accumulation of 12 months; maximum benefit period for each period of unemployment is six months; at least six months must separate each period of unemployment. This rider is automatically included without charge in states where approved. Not approved in CA, CT, FL, MD, MA, MI, NH, RI, SC, TN, UT and VT.

ASSISTANCE TO BENEFICIARIES IN APPLYING FOR DEATH BENEFITSPlease provide the Claims Department notice of death as soon as possible, so we can research the policy file to determine the current beneficiary. Include the full name of the insured, along with the date and cause of death. A letter along with the appropriate claim form will be sent to the beneficiary or representative of the estate.

CLAIMS WITHIN THE CONTESTABLE PERIODThe statements made in an application materially affect the company’s acceptance of coverage, and if a claim should occur within two years of the effective date, the Claims Department will conduct a full review of medical history. If there has not been full disclosure of material facts, the company may void the contract as of the issue date and refund all premiums paid, placing the insured and the company in the same position as if a policy had never been issued.

When the Claims Department is conducting a medical review, it is important that the policyholder or beneficiary complete medical history at time of claim. Delays in obtaining medical information normally occur with the medical provider, so it is important that we know of all medical providers at the onset of the claims process and not learn of additional providers as records are received and reviewed.
