INTRODUCTION

Woodlands Health Centre is dedicated to providing health care to patients in theHither Green area. We are located on the old Hither Green Hospital site, which wasopened in March 1871, and closed in September 1997. The Surgery moved fromsmaller premises in Lewisham to this site in February 2005 with 2,600 patients. Thepractice now provides services to circa 8000 patients and continues to grow steadilyyear on year.

STAFF ROLES AND RESPONSIBILITIES

PrACTICe NUrSINg STAff/HeAlTH CAre ASSISTANTWe have two practice Nurses who are trained to do an extensive number ofprocedures and tests. They work alongside our doctors, and they run chroniccondition clinics such as asthma, diabetes, heart disease as well as a range ofother services besides. We have a Health Care Assistant who assists the PracticeClinical team in the care of patients. She conducts New Patient Checks,phlebotomy, NHS Health Checks etc. She advises on preventative care, screeningand patient education in accordance with agreed practice protocols.

PrACTICe PHArMACISTThe Practice Pharmacist is responsible for medicine management and optimisation,medication reviews, hospital prescription reconciliations, complex communitypharmacy queries, medicine audits, MHRA/NICE updates as well as otherpharmaceutical related duties. You can book to see the practice pharmacistbetween 1pm and 2pm on Monday, Tuesday and Thursday.

DISTrICT NUrSeSWe have a number of district nurses attached to the practice. They are based atHonor Oak Health Centre. You can contact them directly by phoning 020 83147777. The district nursing team provide services to patients that are housebound.Patients can be referred to a distract nurse by their GP, Practice Nurse or by thehospital discharge planning team. If you have any questions about the services orhow to obtain the services please speak us.

HeAlTH VISITorSThe Health Visitors provide support and advice to parents of children under five. Regularbaby clinics are run at the practice from 1.30pm-3.15pm every Wednesday. The clinicis for well children only and provides an opportunity to discuss your child's development.If your child is unwell you should bring them to normal surgery If you want to weigh yourbaby or just have a chat then please come along as no appointment is required. Youcan call our Health Visitors on 0203 049 1900.

MIDWIVeSThe practice is linked to the Community Lewisham and Greenwich NHS TrustMidwives. They attend the practice every Monday and Thursday for pre-bookedantenatal clinics. You can book with the Midwives directly by calling the DirectAccess Number: 07787841986 without having to contact your GP.

PrACTICe ADMINISTrATorThe Practice Administrator deals with all aspects of the practices administative andsupport services necessary to run the practice and also oversees the maintenanceand upkeep of the premises.

reCePTIoN AND ADMINISTrATIVe STAffAll our staff are here to help you and strive to offer a professional service at all times.Receptionist and are your link with the rest of the practice. They may need to askfor your medical details in order to work effectively. All information given to them istreated in the strictest confidence and they are bound by the same code of conductas Doctors and Nurses. The more information that you share with the receptionists,the better they will be able to assist you. The reception desk and telephone arealways extremely busy, so please be patient. We ask that if you need to contactthe surgery regarding a non-urgent matter that you call us after 10.30am.

NoTe SUMMArISerThe Note Summariser summarises all new patient records on to the practice system,so when you come to see your GP or nurse your past information is available.

HOW TO USE OUR SERVICESYou can pre book an appointment with a clinician by calling the practice on020 8698 6664, or by just popping in to see us. Most appointments can be bookedup to two weeks in advance to accommodate those who may not have an urgentneed to see a doctor or may need to plan their doctors visit in advance. We offerearly morning and late night clinics on Mondays and Tuesdays and the lastSaturday of the each month, and you can book appointments online via our website. If you need to see a GP as a matter of urgency we offer a walk in service Monday-Friday from 8.30am-10.30am. All our staff have been trained to a high standard tomaintain patient confidentiality.Please let us know as soon as possible if you are unable to attend an appointmentso that we can rebook the appointment to a patient who needs it.

WAYS To BooK AN APPoINTMeNT• Online – Before you can use the online appointment booking system you mustregister at reception to obtain a username and password.• Telephone – You can book an appointment by telephoning the practice during practice hours or if you would like telephone advice leave a message with the receptionist and a doctor will call you back at the end of the morning or afternoon surgery.

• Face-to-face – You can visit the practice in person and book an appointment via reception.• Urgent – Protected appointments are offered every day strictly for medical emergencies.• Telephone Consultation Service – Our practice offers telephone consultations if you do not need to be examined by a GP. Request a phone consultation and you will be given an approximate time that the GP will call you back.

PATIeNT ACCeSSWe have now implemented Patient Access. This allows patients to bookappointments online, order repeat prescriptions and edit their details from thiercomputer, tablet or smart phone. Patients can even download an app to accessthis service. In order to register for this service please attend the sugery withphotographic ID and you will be issued with a password to enable you to completethe on-line registration.

SeeINg THe DoCTorWe feel that it is important to provide you with continuity of care, this means seeingthe same doctor when you attend the surgery. When booking an appointmentplease tell the receptionist who your usual doctor is, if you do not know we can findthis out for you. We will endeavour to provide a suitable appointment with anotherpractitioner if the doctor of your choice is not available.

TelePHoNe ACCeSSThe surgery sometimes gets very busy due to circumstances beyond our control.We can only apologise for this inconvenience and ask you to be patient or call back.

INTerPreTINg SerVICeSPlease inform reception if you require an interpreter or a British Sign LanguageInterpreter when booking your appointment.

SERVICES WE PROVIDEBABY ClINICHeld every Wednesday between 1.30pm and 3.30pm. The clinic is for pre-schoolchildren only and provides an opportunity to discuss your child's development. Ifyour child is unwell you should bring him/her to normal surgery. If you want to weighyour baby or see the Health Visitor no appointment is necessary. We do

developmental checks on all children at six weeks. The 6 to 8 week checks areusually done at the mothers post natal examination with your GP. You will receivean appointment for this by letter, phone call or text from the surgery.

ANTe/PoST NATAl ClINIC/NeW PregNANCIeSThe Antenatal clinic is held on Monday morning 9.30am-12.30pm and Thursday8.30am-5.00pm with our midwife. We also hold Antenatal clinic with our doctors3pm-5pm and Thursday 4pm-7.30pm. Most antenatal care can be undertaken atthe practice. Mother and baby should be seen 6 to 8 weeks after the birth by theDoctor. Once we receive your discharge letter from the hospital you will receive anappointment for this clinic from our practice. You should book to see the doctor ina routine appointment during surgery hours or you can book directly with ourmidwife. You should be seen before you are 11 weeks.

SeXUAl HeAlTH ClINICWe want to ensure that all our female patients are up to date with their smears andbreast awareness. You should have a smear every three years unless told otherwiseby a healthcare professional. Please ask the receptionists to book you anappointment with the practice nurse. We offer a full range of contraceptive services.Appointments but must be booked. We also offer coil fitting, removal andadjustment on Tuesdays from 5.30pm-6.30pm.

CHIlD IMMUNISATIoNBaby immunisation clinics are available by appointment only please contact reception.

MeDICATIoN reVIeW ClINICWe offer a Medication Review clinic run by our Practice Pharmacist on Mondays,Tuesdays and flexibly on Thursdays & Fridays between the hours of 1pm and 2pm.If you are due for a medication review, kindly book an appointment with our PracticePharmacist.

YelloW feVer VACCINe CeNTreWe are a nominated Yellow Fever Vaccine Centre. The vaccine is offered on aprivate only basis at a cost of £60. If you are not a registered patient please bringphoto ID with you.

rePorTS/leTTerS/forMSCertain requests are made for reports, letters, and forms etc.; which do not comeunder the NHS and are therefore chargeable. A current list is displayed at reception.Form are available at reception and you should allow atleast 28 working days forall letters/forms/reports before collection. Forms should be handed in at thereception desk. We will not release any of your medical history without yourauthorisation.

APPlYINg for ACCeSS To YoUr MeDICAl reCorDSDepending on your health records you want to see, you can submit your request byfilling in our Access to Medical Records, and Personal Data forms. There will be anadministration fee for this service. The health records manager, GP or other healthcareprofessional will decide whether your request can be approved, Under the DataProtection Act, requests for access to records should be met within 40 days.

forMAl reqUeSTS UNDer THe DATA ProTeCTIoN ACTUnder the Data Protection Act 1998, you have a legal right to apply for access toyour health information held about you. This includes your NHS or private healthrecords held by a GP Optician or Dentist, or by a hospital. For information regardingcharges please refer to the charges List displayed at reception. A health recordcontains information about your mental and physical health recorded by ahealthcare professional as part of your care.

OUR PRESCRIPTION SERVICErePeAT PreSCrIPTIoNSRequests for repeat prescriptions can be made in several ways:

• In writing with an SAE included• By using our online request form (please register at reception)• By ticking the relevant boxes on your repeat prescription form and submitting it

at reception. We do not accept prescription requests over the telephone.

Your prescription should be ready within 48 hours from the time of receipt by thePractice. This does not include weekends and Bank Holidays so please rememberto bring your requests in earlier.

We also offer a “Collect at The Chemist” scheme please check with your pharmacist forfurther details Repeat prescriptions are only issued with the agreement of the GP andyou will be asked to attend your GP practice for a medication review every six months.

eleCTroNIC PreSCrIPTIoN SerVICe (ePS)You will not have to spend time going to see your GP practice every time to pick upa paper prescription. Instead your GP will send it automatically to the pharmacy ofyour choice, with no chance of it getting lost. You will have more choice about whereyou get your medications from, because they can be collected from a pharmacywhere you live, work, or shop.

Our Practice Pharmacist will review your medication normally every 6 months. Youwill find the date your medications are due for review on your repeat prescriptionslip. Please ensure that you attend the practice in order for your GP to continue tosafely prescribe your medications.

INFORMATION FOR NEW PATIENTS

HoW To regISTerYou can register as an NHS patient if you live within the practice catchment area.You may still register with us if you live outside of our catchment area howeverplease note that our doctors will not visit you at home. Please check with receptionif you are not sure. You can download a registration documents from our websiteor pop in to the surgery and collect a form and accompanying documents. Beforeyou are fully registered you will need to attend the surgery for a new patient healthcheck.

HoW THe HeAlTH SerVICe WorKSThe NHS is the world’s largest publicly funded Health Service. The NHS hasundergone huge changes in recent years including the introduction of ClinicalCommissioning Groups (CCG) in April 2013. NHS Lewisham CCG is led by cliniciansand is responsible for planning, buying, and monitoring most of the health servicesyou will use whilst in Lewisham. This means that all GP practices in Lewisham aremembers and so assist in making these decisions and plans for your local NHS.

CoMMUNITY HeAlTH SerVICeSKindly ask at reception for the contact details of health visitors, district nurses,school nurses, therapists and others.

MeNTAl HeAlTH SerVICeSThere are a range of mental health services available. These are provided by theSouth London and Maudsley NHS Foundation Trust. If you are worried about yourmental health and need help please speak to your GP who will refer you to thecorrect service.

DrUg AND AlCoHol SerVICeS Offer counselling and advice to all. If you areworried or need help please speak to your GP or contact 020 8314 5566.

SelDoC - South East London Doctors on call is a service offered when your GPPractice is closed. If you are unwell but the matter is NoT AN eMergeNCY pleasedo not go to A+E. Contact NHS 111 or SELDOC on 020 8693 9066.

STAff TrAININgThe practice will close for training of all doctors and staff usually half a day everytwo months. Prior notice of the closure will be advertised in the practice.

TREATING MINOR ILLNESS AND ACCIDENTS AT HOME

Many common complaints can be simply and successfully treated at home without the need to consult a doctor.

MINor BUrNSImmediately submerge area in cold water until the pain subsides. This can take 10-15 minutes but is invaluable in diminishing the damage done. Unbroken blistersshould be left alone or protected if necessary by a loose, dry dressing. Large burnsor those in which the skin is broken should be seen by a doctor or nurse. It can behelpful to cover the burn with cling film until medical help is available . This protectsthe wound and prevents infection and it will not stick.

SPrAINSA cold compression (e.g. Crushed ice or frozen peas) is invaluable if applied assoon as possible for 15-30 minutes. Apply a firm crepe bandage and rest the sprainin an elevated position. Sprains may take several weeks to heal so try not to provokethem during this period. Simple painkillers such as paracetamol or ibuprofen(Nurofen) purchased from the chemist is usually all that is required.

NoSe BleeDSTry to keep calm in this alarming situation. Sit in a chair, lean back with your mouthopen. Firm pressure applied with fingers and thumb below the bridge of your nosemust be maintained without letting go for ten minutes. Be very gentle afterwardsand do not blow your nose dab or pick it! Vaseline can soften the clots in a fewdays time as they harden.

MINor CUTS AND grAzeSClean the wound thoroughly with soap and water. Firm pressure for five minuteswill stop the bleeding. A clean, dry dressing can then be applied.

DIArrHoeA AND VoMITINgADUlTVomiting- Take nothing by mouth for 12 hours then try small, frequent sips of waterevery 15-20 minutes, progressing to a light diet for the next day or two. Most casessettle within 24-48 hours. If symptoms persist, please contact your GP by telephone.Please do not attend the surgery unless requested to by a GP.

DIArrHoeAThis is usually caused by a virus. Your body is doing its best to eliminate the virusso do not take diarrhoea remedies as they may actually prolong the illness. A fluidor very light diet should be taken avoiding any meat based greasy foods or citrusfruits, If problem persists please contact the GP. Please do not attend the surgeryunless asked to do so by your GP.

CHIlDreN AND BABIeSIf your child is experiencing the above symptoms do not hesitate to contact thedoctor for advice. If breast fed, continue to feed as usual. If bottle fed, replace withhydration fluids (Dioralyte) For 24 hours. For babies under six months see the doctorif diarrhoea occurs in more than two nappies a day. In older infants and childrensee the doctor if symptoms do not improve after 24 hours.

feVer IN YoUNg CHIlDreNIt is usual for children to have high temperatures with minor illnesses. It may be that thisis natures way of doing some harm to the virus. You can try to cool the child down by• Dressing them lightly, reducing blankets on cots and beds• Maintaining high fluid intake• Giving paracetamol (Calpol) or Nurofen• If the child is also coughing, ban smoking in the house• Sponging with tepid water may only have a minor effect and can be unpleasant

for the child.12

INDIgeSTIoN AND CoNSTIPATIoNIndigestion is best treated with some antacid tablets or mixture. For constipationyour pharmacy can supply a laxative treatment although it is a good idea to eatplenty of fibre rich foods and to drink around two litres of water per day.

CHOOSING THE RIGHT SERVICE WHEN YOU ARE ILL

gP’sYour GP is your first port of call if you have any routine/urgent illness and ongoingconditions. Using your GP saves time as they know your medical history. GPservices are available 24/7.

UrgeNT CAre CeNTreThe Urgent Care Centre at University Hospital Lewisham is available to treat injuriesand illnesses which require immediate help when you are unable to get anappointment with your GP. It is located in the same area as the A+E departmentand on your arrival you will be treated by the relevant department depending onyour clinical need. Please consider what other services are available to you beforeattending the urgent care centre.

A+e (ACCIDeNT & eMergeNCY)When you have a serious injury such as blacking out, chest pain, choking, shortnessof breath or bleeding you cannot stop, you need to go to A+E. The service providesurgent treatment for serious and life threatening illness and injury. You should travelto A+E yourself if you can, but if someone is too ill, has collapsed or is unable tobreath call 999 for an ambulance.

HoMe VISITSPlease reserve requests for home visits for cases that genuinely cannot attend thesurgery such as the elderly or housebound. If you require a home visit phone before10.30am if possible. Please note that we are only able to visit you at home if you livewithin the practice catchment area. The GP will ring you back to discuss the problem.Please ensure you leave a working telephone number for the doctor to contact you.A doctor can often have up to four visits daily, these take time so please be patient.

TeST reSUlTSThe receptionist will be able to give you your test results once the doctor hasreviewed them. Most test results are normally back after seven days. Results aregiven out between 11.00am and 2.00pm Please do not ask for results outside thesetimes as we will be unable to help you. We will ask you to confirm your details beforewe release any confidential information.

CoMPlAINTSWe welcome feedback from our patients. All complaints/feedback should be made tothe practice manager in the first instance and should be made in writing or by email.We will endeavour to resolve your complaint. PATIENT PARTICIPATION GROUP (PPG)

All patients are welcome to join our Patient Participation Group to help have an inputinto the service we provide. We hold our PPG meetings every quarter on the 1stMonday of the month at 6.30pm at the practice. If you are unable to attend our PPGmeetings then we will be happy for you to become a virtual member providing inputonline and responding to surveys sent. If you are interested in becoming a memberof our PPG, please ask at reception.

DISABleD ACCeSSAccess for wheelchairs is through the main entrance of our surgery. If you needassistance or experience difficulties climbing stairs please inform the reception staffwhen you book your appointment and arrangements will be made to assist you.The surgery has a ramp for wheelchair access, toilet facilities for the disabled anddisabled parking.

The NHS belongs to all of us. There are things that we can all do forourselves and for one another to help it work effectively,

and to ensure resources are used responsibly.14

Please recognise that you can make a significant contribution to your own, and yourfamily’s, good health and wellbeing, and take personal responsibility for it.Please register with a GP practice - the main point of access to NHS care ascommissioned by NHS bodies.Please treat NHS staff and other patients with respect and recognise that violence,or the causing of nuisance or disturbance on NHS premises, could result inprosecution. You should recognise that abusive and violent behaviour could resultin you being refused access to NHS services.Please provide accurate information about your health, condition and status. Pleasekeep appointments, or cancel within reasonable time. Receiving treatment withinthe maximum waiting times may be compromised unless you do.Please follow the course of treatment which you have agreed, and talk to yourclinician if you find this difficult. Please participate in important public healthprogrammes such as vaccinations. Please ensure that those closest to you areaware of your wishes about organ donation.Please give feedback - both positive and negative - about your experiences andthe treatment and care you have received, including any adverse reactions youmay have had. You can often provide feedback anonymously and giving feedbackwill not affect adversely your care or how you are treated. If a family member orsomeone you are a carer for is a patient and unable to provide feedback, you areencouraged to give feedback about their experiences on their behalf. Feedbackwill help to improve NHS services for all.
