My name is…………………and I work for a research agency called Karvy Insights Pvt. Ltd. I am member of a research team that is conducting a study to understand the recall of campaigns around child health and family planning created and aired by our client ABT Associates. This study is being conducted as part of a project called SHOPS Plus that seeks to promote the use of family planning, child health and tuberculosis treatment products and services among the urban poor. As a part of this study we will be interviewing caregivers to children under 5 (or young married couples), to gather data on their understanding of, beliefs, attitudes and behaviors with regard to health areas being covered by the project. This data will help our programming team to understand the impact and retention of the campaigns messages among the project’s target audience. You have been selected for the Child Health (or Family Planning) module for the current study. We are asking for 45 minutes of your time. There will be no monetary or other compensation that we will provide for your time. Please understand that your participation is voluntary and subject to your approval . The choice of whether to participate or not, is yours and yours alone. If you agree to participate, you have the choice to stop participating in the interactions at any time you feel uncomfortable — all you have to do is tell me that you do not want to continue. You may also refuse to answer any question. There will be no penalty for your refusal to answer any question. The information you discuss will be kept private. We will NOT publish your name, phone number, or any personal information. Only the research team will ever know that you participated in this study or what your answers were. The answers you give will be kept secure and will be combined with several other respondents in a summary report. You will not be identified in any manner in this report. Per USAID requirements, the datasets developed using this survey will be uploaded to USAID’s data warehouse for use by authorized researchers. Good……………….! My name is…………………and I work for a research agency called Karvy Insights Pvt. Ltd. I am member of a research team that is conducting a study to understand the recall of campaigns around child health and family planning created and aired by our client ABT Associates. This study is being conducted as part of a project called SHOPS Plus that seeks to promote the use of family planning, child health and tuberculosis treatment products and services among the urban poor.

As a part of this study we will be interviewing caregivers to children under 5 (or young married couples), to gather data on their understanding of, beliefs, attitudes and behaviors with regard to health areas being covered by the project. This data will help our programming team to understand the impact and retention of the campaigns messages among the project’s target audience.

You have been selected for the Child Health (or Family Planning) module for the current study. We are asking for 45 minutes of your time. There will be no monetary or other compensation that we will provide for your time.

Please understand that your participation is voluntary and subject to your approval. The choice of whether to participate or not, is yours and yours alone. If you agree to participate, you have the choice to stop participating in the interactions at any time you feel uncomfortable — all you have to do is tell me that you do not want to continue. You may also refuse to answer any question. There will be no penalty for your refusal to answer any question.

The information you discuss will be kept private. We will NOT publish your name, phone number, or any personal information. Only the research team will ever know that you participated in this study or what your answers were. The answers you give will be kept secure and will be combined with several other respondents in a summary report. You will not be identified in any manner in this report. Per USAID requirements, the datasets developed using this survey will be uploaded to USAID’s data warehouse for use by authorized researchers. Your name or any other identifying information will not be a part of this dataset.
