Community Learning Disability Teams

• 7 CLDT’s and one county wide Transition Team provide services across all of Hertfordshire

• Core team employed by HCC

• Extended team employed by HPFT

• Referrals for all specialist learning disability community services managed through CLDT

• All community learning disability nurses work towards nationally agreed health agenda’s

Community Learning Disability Nurses (CLDN’s)

• CLDN’s were transferred from HPFT to HCC by TUPE arrangements in January 2005.

• The role of the CLDN has remained health focused whilst being managed by local authority.

• CLDN’s role has evolved in line with the national health agenda for people with Learning Disabilities.

• HCC now employ 26 WTE qualified nurses and 6 health care assistants.

Supervision and Management

• Since September 2009, our Nursing agenda was enhanced with the appointment of a lead nurse who has a vital role within the senior management team.

• CLDN’s receive clinical supervision from one of 3 senior learning disability nurses, in line with the requirements of their registered body (NMC).

• CLDN’s are operationally supervised within the management structure of the CLDT.

• KSF outlines, training and development requirements and continued professional development is maintained as in health due to shared knowledge and opportunities within our partnership arrangement with HPFT.

Challenges for the future

• Whilst CLDN’s remain on health’s Agenda for Change terms and conditions, HPFT HR sharing with HCC HR will be crucial.

• Within HCC, CLDN’s use HPFT’s electronic recording system, further consideration will be given to the systems ‘talking’ to each other.

• Full integration of South East Herts, service provision repatriated over past year.

Partnership Working

The government has chosen partnership working as a key means through which much of its policy programme is to be delivered at a local level.

Effective partnerships have clarity of purpose. This means that everyone in the partnership is clear about what the partnership is trying to achieve.
