SIM is a federal initiative funded by the Centers of Medicare and Medicaid Innovation (CMMI) to help states test alternative payment models and achieve the Triple Aim. Susan E. Birch, MBA, BSN, RN, Executive Director

As Executive Director, Sue Birch oversees the operations and strategic direction of the Department of Health Care Policy and Financing. The Department’s budget is more than $8 billion and is the single state agency administering Health First Colorado (Colorado’s Medicaid Program) and the

Child Health Plan Plus (CHP+) program. Birch was appointed the executive director of the Department by Governor Hickenlooper in January 2011. Birch directs the Department’s efforts to improve the health of Health First Colorado and CHP+ clients and increase access to care across the state while containing costs.

Under Birch’s leadership, the Department has successfully expanded coverage to more low income Coloradans while focusing on containing costs and improving service delivery. The uninsured rate decreased from 14.3% in 2013 to 6.7% in 2015 and the state saved approximately $285 million (gross savings) through the Health First Colorado Accountable Care Collaborative over the past four years. The Department has been awarded numerous grants and performance bonuses, including the highest bonus of any state in 2012 and 2013 in recognition for its innovation and modernization of eligibility and enrollment systems by the Centers for Medicare and Medicaid Services. Birch serves on the Connect for Health Colorado Board and has been integral in modernizing and integrating the state’s health information technology systems. Birch continues to promote person-centeredness and client engagement and led efforts to develop one of the nation’s first mobile applications for Health First Colorado members. Birch is leading the implementation of federal and state health care reform, while working with stakeholders to integrate care across the continuum and transform the Department’s health care delivery systems. The Department is driving clinical innovations across the state through collaborative projects and initiatives such as the Colorado Opportunity Project, the State Innovation Model (SIM) grant, and launching the Chronic Pain Disease Management Program which leverages telehealth to help connect primary care providers treat pain and reduce prescription painkiller abuse.

Director Birch has been honored at both the state and federal level for her commitment to improving Coloradan’s access to care. Most recently Birch received the 2014 Dr. Nathan Davis Awards for Outstanding Government Service in the category Outstanding Career Public Servant at the State or Local Level from the American Medical Association (AMA).

Director Birch has completed appointments to the National Advisory Committee on Rural Health and Human Services and served as the Bonfils-Stanton Foundation Livingston Fellow and the Robert Wood Johnson Executive Nurse Fellow.

Birch earned a nursing degree and a master’s degree in Business Administration, both from the University of Colorado.

Donna Lynne DrPH, Lieutenant Governor and Chief Operating Officer

Donna Lynne, DrPH, was sworn in as Colorado’s 49th Lieutenant Governor and Chief Operating Officer on May 12, 2016. She is widely respected as both a leader in the private sector and an expert in the health field.

Prior to assuming her roles in Colorado state government, Dr. Lynne served as the executive vice president of Kaiser Foundation Health Plan Inc. and Kaiser Foundation Hospitals, and as group president responsible for its Colorado, Pacific Northwest and Hawaii regions – overseeing an $8 billion budget, 1.4 million members and 16,000 employees.

From 1998 to April 2005, Dr. Lynne was the executive vice president and chief operating officer for Group Health Inc., a $2.5 billion managed care organization. In the mid-1990s, she was the executive director of the New York Business Group on Health.

Dr. Lynne also spent 20 years working in various positions in New York City government, including first deputy commissioner, Office of Labor Relations; director, Mayor’s Office of Operations; and senior vice president, New York City Health and Hospitals Corp.

In addition to her role as an International Executive Fellow with the International Federation of Health Plans, Dr. Lynne is the recipient of numerous honors and accolades, including the Denver Business Journal’s 2008 Outstanding Women in Business award; the 2009 Distinguished Coloradan Award from the University of Colorado Denver School of Pharmacy; the President’s Award from Women in Health Management; and the Health Care Leadership Award from the New York Business Group on Health.

She formerly served on the boards of the Denver Metro Chamber. Colorado Education Initiative, Colorado Mountain Club, Colorado Regional Health Information Organization, Denver Museum of Nature and Science, Denver Public Schools Foundation, Teach for America-Colorado, U.S. Bank Colorado Advisory Board, and is the former chair of the Denver Metro Chamber of Commerce.

Dr. Lynne was a co-chair for the Denver Education Compact Executive Advisory Board for Mayor Michael B. Hancock. In 2010, she was one of 16 statewide transition co-chairs and the Personnel and Administration Committee co-chair for Governor John Hickenlooper.

She holds a Bachelor of Arts (Economics and Political Science), a Masters of Public Administration, and Doctor of Public Health from Columbia University. She also holds the designation of Certified Employee Benefits Specialist from the International Foundation of Employee Benefit Plans/University of Pennsylvania Wharton School. In 2014, Dr. Lynne received an honorary doctorate of public service from the University of Denver. Since 2005, she has been an adjunct associate professor of Public Health at Columbia University.

Dr. Lynne resides in Denver and has three children and two step-children. She enjoys skiing, mountaineering, cycling and reading. She has climbed all 58 of Colorado’s Fourteeners.

Value-Based Care PanelMark A. Levine, Centers for Medicare & Medicaid ServicesMark A. Levine, MD, FACP, is the Chief Medical Officer in the Denver office of the Centers for Medicare and Medicaid Services. A quality improvement coach for the agency, he is active in developing and maintaining agency initiatives in clinical quality, payment reform, value-based payment and delivery system innovation.

An alumnus of Rutgers College and Temple University School of Medicine, Dr. Levine completed residency training in Internal Medicine at the Hospital of the University of Pennsylvania and fellowship in Clinical Immunology at the University of Colorado.

Dr. Levine founded the Colorado Patient Safety Coalition. He serves as Clinical Professor at the University of Colorado School of Medicine and the Colorado School of Public Health, teaching in areas of clinical geriatrics, quality, health systems and ethics. Dr. Levine served for many years as a delegate to the American Medical Association and as member and past chair of its Council on Ethical and Judicial Affairs, steward of the AMA Code of Medical Ethics and is currently a faculty associate at the University of Colorado’s Center for Bioethics and Humanities.

Dr. Levine practiced general internal medicine in the Denver area for over 20 years in a variety of practice settings; solo, small group, large group and integrated group. He co-founded and developed a large physician group practice. He has broad experience in managed care with service as an IPA and HMO medical director. Through this varied, deep and practical knowledge and experience, he recognizes the many challenges and opportunities facing American health care in the evolution of safe, effective, timely, equitable, patient-centered and efficient medical care.

Aaron Van Artsen, MLRHR, CHC, Administrative Director | Compliance & Privacy Officer, Office of Value Based Performance | University of Colorado MedicineAaron Van Artsen is the Director of Value Based Performance for CU Medicine, the faculty practice plan representing the University of Colorado School of Medicine School of Medicine. In addition to his

operational Directorship, Mr. Van Artsen serves as UPI’s Organizational Compliance Officer and HIPAA Privacy Officer. Having attended Michigan State University, he holds a Master’s Degree in Labor Relations and Human Resources complimented by a BA in Communications and Certificate in Healthcare Compliance. Mr. Van Artsen offers a unique blend of experience, inclusive of talent acquisition, organizational development, regulatory compliance, and population health management.

Mark J. Carley, Vice-Presidnet of Managed Care and Risk Products | Centura HealthMark J. Carley is Vice-President of Managed Care and Risk Products at Centura Health, the largest health system in Colorado with 17 hospitals, 500+ employed physicians, 150+ ambulatory and ancillary locations

and over 4,000 aligned community physicians. In his role Mark is responsible for all managed care activities and strategies including the organization’s managed care revenue strategic plan, preparing for future payment models, implementing transparency initiatives, establishing partnership and ACO strategies with payers and employers, and assisting the organization in the management and performance of its clinically integrated provider network.

Simon Hambidge, MD, PhD, Chief Ambulatory Officer, Denver HealthSimon Hambidge, MD, PhD, is Chief Ambulatory Officer at Denver Health and the Chief Executive Officer of Denver Community Health Services (DCHS). A pediatrician by training, Dr. Hambidge was previously the Director of General Pediatrics for Community Health Services for 8 years. He is a Professor of Pediatrics at the University School of Medicine, and has practiced at Denver Health since 1997.As DCHS CEO, Dr. Hambidge oversees a network of 9 community health centers and 17 school based clinics, which provide health care to over 150,000 patients, approximately one-fourth of the population of Denver. Denver Health’s patients include over half of the Medicaid patients in the city.Dr. Hambidge still provides limited clinical care in the Webb Center for Primary Care’s Kids Care clinic. He has published numerous articles on topics such as vaccine safety and pediatric obesity.Dr. Hambidge serves on the Board of Directors of the Denver Health Medical Plan, the Colorado Community Health Network, the Community Health Association of Mountain and Plains States, and Reach Out and Read Colorado.

During his tenure he has been involved in many technical aspects of the industry; including system changes and implementations, financial modeling, analyzing clinical effectiveness, investigating cost structures, and reforming payment methodologies.

Barbara Martin, RN, MSN, ACNP-BC, MPH, SIM Director, Colorado State Innovation ModelBarbara is a nurse practitioner with more than 15 years of clinical and leadership experience. She received her master’s degree in Public Health in 2013. Barbara has been engaged with SIM since 2013. She joined the SIM Office in 2015 as Director of the Transforming Clinical Practices initiative (TCPi) and was promoted to the position of SIM Director in October 2016.

Ellen Kaufmann, MA, PMP, Evaluation Specialist, Colorado State Innovation ModelEllen Kaufmann is the Evaluation Specialist for the Colorado Department of Health Care Policy and Financing and the Evaluation Program Manager for SIM. In this role, Ellen is creating a more comprehensive approach to evaluation of Department and cross-agency initiatives, including SIM and the Accountable Care Collaborative (ACC) program. Prior to joining the Department, Ellen worked at the Illinois Public Health Institute, implementing robust stakeholder engagement processes for state-level health transformation initiatives. She also led project management and communications for a national program to connect information systems and share data across sectors to improve community health. Ellen’s background is in community and economic development, with previous experience at an HIV program serving primarily Latinos in the Denver area, a water sanitation and hygiene project in a Nairobi slum, a microenterprise development non-profit, and Peace Corps service in Nicaragua. Ellen is well-versed in incorporating stakeholder perspectives into state health planning and implementation efforts and will ensure SIM’s progress toward all aspects of the Quadruple Aim is captured and communicated to stakeholders. Ellen received her MA in International and Intercultural Communications with a concentration in global health from the University of Denver and a BA in Business Administration from Truman State University.

Strategic Implications of MACRA Payment ReformSteve Smith, Managing Consultant, BKD, LLPSteve is a member of the Physician Services Center of Excellence and payment reform teams. He provides consulting services to provider groups and healthcare organizations related to payment reform initiatives, revenue cycle, process improvement, and compliance.

Steve has experience planning and completing consulting projects which utilize big data and historical performance to predict future outcomes, as well as identifying trends which may indicate areas of opportunity for process change and improved financial and clinical outcomes. He has provided detailed presentations on the Medicare Access and CHIP Reauthorization Act of 2015 (MACRA), Comprehensive Care for Joint Replacement (CJR), Comprehensive Primary Care Plus (CPC+), and numerous other payment reform initiatives.

The Triple Aim: Is it Possible?Dr. Bill Burman, Interim CEO, Denver Health

Bill Burman is the current Interim CEO of Denver Heath. Previously, he served as Director of Denver Public Health, the arm of Denver Health that provides data on health in Denver, works in partnership to improve the health of the entire community, and helps prepare the community for health emergencies. Dr. Burman also chairs the Mile-High Health Alliance – a forum that brings together medical, behavioral, and social service providers in Denver to collaborate on common problems.

Health Equity in ColoradoJoe Sammen, MPH, Executive Director, Colorado Coalition for the Medically UnderservedAt his core, Joe believes the desire for good health is the closest thing our society has to a universal goal, making the promise of health equity an incredible call to action for people from all walks of life.
