ARH team members from each : community were invited to step right up to the carnival midway,

where classic games such as Duck Pond, Ring Toss. and Wheel of Fortune were accompanied by sweei and salty treats

All attendees received a special Team ARH T-shirt with "Just for Ihe Heallh of II," the theme of thiS year's wellness fair/camival crossover, printed across the back.. With wellness as a major locus of the events, screenings such as blood pressure, blood' glucose, and grip strength were provided on s1te. Comes now the Affiant, Billy O'Brian (Brian) Frasure, and after first being duly sworn, states

and deposes as follows:

1. My name is Billy O'Brian (Brian) Frasure and I am employed as Accounting and Finance

Manager for Big Sandy Rural Electric Cooperative Corporation ("Big Sandy'').

2. Pursuant to 807 KAR 5:001 Section 17, Big Sandy provided notice to its customers of the

general rate adjustment that serves as the basis of its Application in the above-styled matter,

Case No. 2017-00374. A copy of the notice (''Notice") is attached to Big Sandy's

Application at Exhibit 5.

3. Big Sandy caused the Notice to be published in newspapers of general circulation in the

utility's service area, as evidenced by the newspaper publishers' affidavits filed

contemporaneously herewith.

4. Big Sandy also caused the Notice to be mailed to certain customers that are known or

believed to be outside ofthe circulation area of local newspapers. The customers that were

mailed the Notice total thirty-five (35) in number and are located in areas of Breathitt

County, Kentucky, and Morgan County, Kentucky. TheN otice was placed into the custody

and care of the U.S. Postal Service on October 23, 2017.
