Traitements
Méthodes Thérapeutiques au cours du SHU/PTT
• Traitements symptomatiques +++
– antihypertenseurs, IEC+++
– hémodialyse itérative (bilan hydroélectrolytique et poids)
• Traitements étiopathogéniques
– diarrhée infectieuse: abstention
– plasma frais: perfusion ou échanges plasmatiques (20-60 ml/kg/jour)
– Si PTT confirmé: • corticoïdes (1mg/kg/jour)
• rituximab 1 injection par semaine x4 (PTT)
– Si SHU atypique (ou typique sévère?): eculizumab; intérêt des immunoadsorptions ?

Eculizumab dans le SHU atypique résistant au plasma (Ch Legendre, NEJM, 2013)
• Eculizumab: 900 mg /semaine puis 1200mg toutes les 2 semaines; blocage complet du Complément ;
• 17 patients inclus, 15 Tt complets (sorties: 1 lupus, 1 mauvaisetolérance)
• Patients: âge moyen: 28 ans; 30% hommes; 40% transplantés; 24% sans mutations, 6 EP avant Tt
• Résultats: – normalisation des plaquettes en 1 à 2 semaines++++
– 77% sur 17 sont guéris (13/15 en fait) ; pas de nécessité de plasma pendant eculizumab
– 65% améliorent leur DFG; 5 sur 7 arrêtent la dialyse
– Nette amélioration de la qualité de vie
Eculizumab dans le SHU atypique plasma-dépendant
• Eculizumab seul sans plasma
• Patients:
– SHUa sous plasma au long cours, au moins 2 fois par semaine;
– malades stables sous plasma depuis en moyenne 9 mois
– 70% avec mutations ; 40% transplantés
– Âge moyen 28 ans , 63% femmes,
• Résultats:
– 87% des cas sont stables sous eculizumab seul ;
– GFR stable à 30 ml/min
MAT diagnostiquée = traitement en urgence +++
EP 60 ml/kg plasma quotidiens++++ folates+ Réanimation±±±± SteroïdesTsf plaquettes = 0
Conclusion – Perspectives: Traitement des MAT en 201 3
