hand holes, vaults and laterals with wires and cables to be placed therein, under
the surface of the following public way. Said locations to be placed in accordance
with plan titles proposed Comcast Conduit installation P1725 Hall Ave.
Place 1-2 PVC conduit from P1725 Hall Ave across Hall Ave 30 west to a
Comcast vault to be placed on a private property of 75 Governors Ave
..also for permission to lay and maintain underground conduits, manholes,
handholes, vaults, laterals, cables and wires in the above or intersecting public
ways for the purpose of making connections with such poles, equipment and
buildings as it may desire for distributing purposes
ApprovedChief Engineer w/provision that1) No City owned or private utilities
or other structures are adversely impacted. The applicant shall ensure that allsewer, water and drain lines are marked prior to any excavation 2) For roadrestoration, the applicant will match the existing pavement thickness with asphaltbase course and following the 2014/2015 winter season , grind and installpermanent trench pavement 3) Tracer tape shall be placed over the conduit andas built plans shall be delivered to Medford Engineering.
Before beginning work the applicant shall notify Dig Safe and shall obtainapplicable permits from the Engineering Division. The contractor shall utilize Cityof Medford regulations and standards for restoration as well as remove all debrisrelated to its work.

buildings as it may desire for distributing purposes
