BONN/KREMS – Zwei Elemente kennzeichnen die GKV-Statistik zurEin nahmen-Ausgaben-Entwicklung im1. Quartal 2014: Besonders die Ersatz-und Betriebskrankenkassen haben einDefizit eingefahren, die Ausgaben für dieZahnheilkunde sanken von bisher sie-ben Prozent auf nur noch sechs Pro zent,davon für „zahnärztliche Be hand lung“fünf Prozent und für „Zahn ersatz“ einProzent. Ursache sind überproportio-nale Zuwächse der Ausgaben für dieKrankenhäuser, die um fünf Prozentstiegen und von den Gesamtausgaben34 Prozent ausmachen.
Die AOKen und die Knappschaft-Bahn-See erzielten Überschüsse vonrund 149 beziehungsweise 62 MillionenEuro. Bei den Ersatzkassen überstiegenhingegen die Ausgaben die Einnahmenum rund 317 Millionen Euro; bei denBetriebskrankenkassen um 128 Millio-nen Euro und bei den Innungskranken-kassen um 29 Millionen Euro. Zur „Panikmache“ der Krankenkassen zuden steigenden GKV-Ausgaben erklärteBundesgesundheitsminister HermannGröhe: „Die Kassen tun gut daran, ihrehohen Finanzreserven und ihre neuenBeitrags-Gestaltungsmöglichkeiten im
Sinne der Versicherten zu nutzen.“ Und weiter das BMG: „Die gesetzlichenKrankenkassen haben im 1. Quartal 2014Prämien in Höhe von 236 MillionenEuro an ihre Versicherten ausgezahlt.Damit haben einzelne Kassen begonnen,ihre Versicherten an den hohen Finanz-reserven von 16,8 Milliarden Euro teil-haben zu lassen.“ Gröhe: „Die Kassen er-halten auch für 2014 vom Gesundheits-fonds genügend Finanzmittel, um ihreAusgaben zu 100 Prozent zu decken.“
Nach dem vorläufigen Bericht stan-den Einnahmen von rund 50,7 Milliar-
den Euro im ersten Quartal Ausgabenvon rund 51 Milliarden Euro gegenüber.
Je Versicherten gab es im 1. Quartal2014 nach den vorläufigen Schätzungeneinen Ausgabenzuwachs von 5,3 Pro-zent. Die Leistungsausgaben stiegen um5,5 Prozent je Versicherten; die Verwal-tungskosten um rund 2,1 Prozent.
Im Bereich der vertragsärztlichenVergütung sind die Ausgaben um 3,8 Prozent gestiegen, bei den Ausgabenfür zahnärztliche Behandlung undZahn ersatz um 3,6 bzw. 2,9 Prozent.Dass es nach Wegfall der Praxisgebühr
vor allem bei AOK-Versicherten einenhöheren Bedarf an zahnärztlichen Leis-tungen gibt, zeigen die Zahlen: So stiegendie Ausgaben je Versicherten gegenüberdem ersten Quartal 2013 für die zahn-ärztliche Behandlung um fünf Prozentund für Zahnersatz um 4,5 Prozent. DerAnteil für zahnärztliche Behandlung undZahnersatz an den Gesamtausgaben derGKV ist angesichts des höheren Ausga-benanteils unter anderem für die Kran-kenhäuser, für Arzneimittel, Heil- undHilfsmittel von bisher sieben Prozentauf nur sechs Prozent gesunken. ��

KREMS (jp) –Die rechtliche Situationzum Aufklärungsgespräch sei schwie-rig, so Prof. Dr. Maria Kletecka-Pul-ker, MedUni Wien, beim 60. Gesund-heitspolitischen Forum Ende Mai inWien. Jedes Aufklärungsgespräch seiindividuell, genauso sind es die Patienten, Eingriffe und Situationen.Das Selbstbestimmungsrecht sei inÖsterreich ohne Beschränkung auchim negativen Bereich Realität, jederPatient habe das Recht, unvernünftigzu handeln. In manchen Bereichenseien die Bestimmungen zur Aufklä-rung besonders streng, das betrifft vor allem jene Bereiche ohne medizi -nische Indikation. „Aufzuklären“, soKletecka-Pulker, „ist der Patient bzw.Angehörige durch einen dienstha-benden Arzt – es muss jedoch nichtder behandelnde Arzt sein. Die Auf-klärung umfasst die Diagnose, Thera-pie und den Verlauf sowie Risiken undNebenwirkungen und Behandlungs-alternativen. Gegebenenfalls müssenauch die Folgen bei Ablehnung derBehandlung besprochen werden“, soin der Diskussion unter Leitung vonHR Prof. Dr. Robert Fischer, DanubePrivate University (DPU) und Leiterdes Gesundheitspolitischen Forums.
Es stelle sich nicht die Frage, obaufgeklärt werden soll, sondern im-mer nur, wie die Aufklärung statt -findet. Falls Kosten nicht von derKrankenkasse getragen werden, muss das ebenfalls unbedingt be -sprochen werden. Die mündlicheAufklärung reicht aus rechtlicherSicht aus, ein „Auf klärungsbogen“ hat
jedoch Beweis- und Dokumenta-tionsfunktion. Das persönliche Auf-klärungsgespräch ist zwingend, esgibt jedoch viele Mö glichkeiten, dieseAufklärung zu unterstützen. Für nichtdeutschsprachige Patienten geltenkeine Sonderbestimmungen, son-dern ebenfalls die allgemeinen Re -gelungen. Falls der Patient den Arztgar nicht versteht, besteht keine Be-handlungspflicht, außer im Falle un-bedingt notwendiger erster ärztlicherHilfe.
Univ.-Prof. DDr. Porteder sprichtvom Kommunikationsmangel als Ursache für den Arztwechsel. Oftmalsist zu wenig Zeit, um die Aufklärungumfassend durchführen zu können.Patienten sind häufig durch Medieninformiert, kritisch und selbstbe-
wusst. Gleichzeitig haben sie einehohe Anspruchshaltung und vieleRechte. Aus Sicht der Ärzte ist es wich-tig, diese Rechte auch zu kennen. Ver-fahren gegen Ärzte betreffen häufigvermeintliche Behandlungsfehler, diesich im Laufe des Verfahrens auf Mängel in der Aufklärung zurückfüh-ren lassen. Die Aufklärungspflicht istim Ärztegesetz, dem Zahnärztegesetz,der Patientencharta und dem KAGverankert – vorrangig sind es aber hu-mane Gründe, die für die Verpflich-tung zur Aufklärung sprechen. Früherwar es das Wohl des Patienten, das imVordergrund stehen sollte – heute istes der Wille des Patienten.
Der Arzt haftet in jedem Fall fürdie Methode, jedoch nicht für denHeilerfolg. Die Dokumentation der
Aufklärung ist vor allem auch fürmögliche Gerichtsverfahren relevant.Anders als im Strafrecht liegt im Zivil-recht die Beweislast beim Angeklag-ten, dem Arzt. Kann er nicht zweifels-frei nachweisen (über Dokumenta-tionen), umfangreich aufgeklärt zuhaben, wird er das jeweilige Verfahrenmit großer Wahrscheinlichkeit ver -lieren.
Frau Dr. Kletecka-Pulka stellteauch ein Pilotprojekt „Videodolmet-schen“ vor. Professionelle Dolmet-scher können per Video innerhalbvon zwei Minuten zum Aufklärungs-gespräch zugeschalten werden. DasPilotprojekt soll unter anderem dieQualitätssicherung bei der Behand-lung nicht deutschsprachiger Patien-ten erhöhen. ��
ELIXHAUSEN – Zahlreiche Teil -nehmer waren der Einladung des Vor-standes des ODV gefolgt und nahmenam Vorabend der Generalversamm-lung am gemeinsamen festlichenAbendessen teil. Das Hotel Gmachl istfür seine exzellente Küche bekannt undverwöhnte die Gäste mit einem erst-klassigen Abendmenü.
Am Samstagmorgen hieß der Prä-sident des ODV, Dr. Gottfried Fuhr-mann, die anwesenden Mitglieder desVerbandes willkommen, stellte die Beschlussfähigkeit fest und ließ überdie Tagesordnung abstimmen. Nachdem Tätigkeitsbericht des Präsidenten,der über die Aktivitäten des Verbandesinformierte und den Jahresbericht2013/14 in gedruckter Form vorstellte,übergab er das Wort an Roman Reich-holf, Kassier des ODV.
Dessen Beitrag über die Kassenlagedes ODV und der daraufhin folgendeBericht des Rechnungsprüfers, Mag.Gerhard Mrak, waren die nächsten Tagesordnungspunkte.
Mit herzlichen Worten des Dankesverabschiedete Dr. Fuhrmann im An-schluss daran Herrn Mag. Mrak aus seiner Funktion als Mitglied des Fach-beirates, da Mag. Mrak sich ab 1. Juli an deren beruflichen Herausforderun-gen widmet.
Nach erfolgter Neuwahl des Rech-nungsprüfers sowie der Diskussionund Abstimmung über den Antrag eines Mitgliedes endete der erste Teilder Zusammenkunft. Im Anschluss andie Generalversammlung sprach Ste-fan Smyczko, MSc, über den ODV-Kodex, das „Legal Compliance Audit“

Wieeine repräsentative Umfrage imAuftrag des Gesundheitsmaga-zins „Apotheken Umschau“ her-ausfand, können sich viele Men-schen in Deutschland eine teureZahnbehandlung von beispiels-weise mehr als 500 Euro für Zahnersatz schlichtweg nichtleisten. Zwei von fünf der Befragten(41,5 %) betonen, ihnen fehle fürsolche Eingriffe das Geld. JederAchte (12,1 %) hat entsprechendauch einen oder mehrere feh-lende Zähne, deren Lücken nichtdurch Zahnersatz oder -prothe-sen ersetzt sind. Wie die aktuelleAusgabe der „Apotheken Um-schau“ (6A/14) berichtet, ist esoft eine Frage des Preises, fürwelche Art der Versorgung sichder Patient entscheidet. Diemeisten bevorzugen günstigereBrücken oder Prothesen, derenKosten den Festzuschuss derKrankenkassen möglichst we-nig übersteigen. Ein Implantatsei hingegen kostspielig: „Mansollte zwischen 2.000 und 3.000
Euro einkalkulieren“, informiertder Präsident der Deutschen Gesellschaft für Implantologie,Dr. Gerhard Iglhaut, in der „Apo-theken Umschau“. Es wundertdaher nicht, dass sich bisher laut Umfrage nur 4,6 Prozent der Deutschen einen fehlendenZahn durch eine implantatge -tragene Konstruktion ersetzenließen. Die große Mehrheit derBundesbürger (87,3 %) findet die Kostenstruktur ungerechtund kritisiert, dass die Kranken-kassen beim Zahnersatz nur be-grenzte Zuschüsse zahlen.

Die demografische Entwicklungin Deutschland ist geprägt vonder doppelten Altersdynamisie-rung, die Menschen werden im-mer älter und gleichzeitig wächstder Anteil alter Menschen an derGesamtbevölkerung. Es kannalso nicht überraschen, wennauch in Ihrer Praxis schon heutejeder vierte mögliche Patient be-reits über 65 Jahre alt ist. Hinzukommt, dass sich die zahnärzt -liche Arbeit mit dem alterndenPatienten in den vergangenenzehn Jahren grundlegend verän-dert hat. Wie aber sehen Kon-zepte zur Zahnpflege im altenMund aus? In Deutschland leben heute mehrMenschen, die 60 Jahre und ältersind, als solche, die jünger als 20 Jahre sind; eine Entwicklung,die sich bereits seit den frühen1970er-Jahren abgezeichnet hatund die nicht im Sinne eines vor-übergehenden Phänomens einrasches Ende finden wird (Abb. 1).Natürlich muss sich auch dieZahnmedizin mit dieser wach-senden Patientengruppe ausein-andersetzen. Auch der betagtePatient hat zunehmend eigene,
echte Zähne, die es zu erhaltengilt. Die Ansprüche und Bedürf-nisse dieser Patienten führen in der therapeutischen Konse-quenz nur in den seltensten Fäl-len zu einer Versorgung mit her-ausnehmbarem Zahnersatz. Hochwertige zahnmedizinischeLösungen aber erfordern eine re-gelmäßige Pflege, wobei gleicher -
maßen Patient und auch der Profieingebunden werden müssen.
Die Zahl der eigenenZähne im Alter steigt
Zunächst ist – neben einer Viel-zahl von Vorurteilen gegenüberdem Alter – auch die Mundhöhle
oft Gegenstand einer undifferen-zierten Betrachtungsweise, häu-fig wird diese als „Elendsgebiet“abqualifiziert. Eine objektive Aus -einandersetzung hingegen zeigt,dass das Alter nicht unmittel -bar zu einer physiologischen Veränderung der Strukturen im

erhalten. Die durch die Ent -zündung entstandenen Abbau-produkte versorgen wiederumdie dysbakterielle mikrobischeGemeinschaft mit essenziellenNährstoffen. So entwickelt sichein Teufelskreis von Entzündungund Dysbakterie, die die Paro-dontitis verschlimmern.Das amerikanische Forscher-team beschreibt die Spezies P. gingivalis als einen Schlüssel-krankheitserreger, der vermut-lich eine große Rolle bei Reiz-darmsyndrom, Darmkrebs undEntzündungskrankheiten spielt.Deshalb sind die Untersu-chungsergebnisse nicht nur be-deutend für die Parodontitis -behandlung, sondern deckendarüber hinaus eine bakterielleStrategie auf, die einen Schlüs selzu anderen Krankheiten liefernkann.

Ist eine kieferorthopädische Be-handlung indiziert, verwendetein Großteil der Kieferorthopä-den zweidimensionale intra- undextraorale Aufnahmen, ein OPGsowie FRS für die Dokumenta-tion und Behandlungsplanung(Rischen RJ et al.).1 Sind festsit-zende Apparaturen das Thera-piemittel der Wahl, werden in derRegel Straight-Wire-Brackets ausMetall oder Keramik, welche be-
reits in der Praxis vorhanden sind,sowie ein Standard-Set vorge-formter Bögen für die initialeKorrektur der Zahnposition ver-wendet. Nach dem Ausrichtenentscheidet der Kieferorthopädedann, welches Bracket ersetztwerden muss und biegt abschlie-ßend die Bögen für die Finishing-phase, um den vorliegenden Fallabzuschließen. In solch einem Behandlungspro-zess werden das Behandlungs -ergebnis sowie die erforderlicheTherapiedauer durch den Kiefer -orthopäden bestimmt, das heißt,sie hängen von dessen Fertigkei-ten und natürlich auch von derMitarbeit des Patienten ab. Company profileOver the last 21 years, Oemus Media AG has become an established,leading global publisher specialised in dental medicine. In addition toa range of more than 20 specialised publications only on the Germanmarket, the publisher is also one of the major providers of congressesand seminars for dentists, dental technicians and dental assistants.In 1994, OEMUS MEDIA AG emerged from a freelancing educationalinstitute. At the time of the company’s establishment, nobody expectedor imagined that this idea would grow to become a publishing houseoperating worldwide and employing more than 100 people. Facing thehealth-political changes in the country, company founder Torsten Oemus created a logical but at that time little-understood publishingconcept. Today’s “state of the art” intention of dentists with regard totheir business activities faced a strong resistance in the middle of the1990s.
