    Preface

    The material contained in this Design Guide was compiled and verified by theOffice of the Surgeon General, Health Facilities Planning Agency and theCorps of Engineers, Medical Facility Design Office. This document is intendedto be used as a guide and a supplement to current Technical Manuals and otherpublished criteria, in the planning and design of Army Health Facilities.Information contained in the Design Guide is subject to changes in medicaltechnology and revisions to Technical Manuals or other criteria. The use ofthe Schedules or Room Designs shall not preclude compliance to current

    criteria of any applicable codes and standards. The Medical Design Guide wasprepared by the Joint Venture of Harwood K. Smith & Partners Inc./Wingler &Sharp Inc., and was updated September 1994 by JMGR Inc.

    Each Room Design series is composed of five individual plates:

    Plate "A" - The Matrix Data Provides specific information on technicalrequirements including architectural, electrical, communication andmechanical requirements. Plate A also includes a specific equipment programincluding Joint Schedule Number (JSN), nomenclature, quantity and thelogistical responsibility category as defined in Military Standard 1691. AJoint Schedule Number with a sub-script is a variation of the basic JSN asdescribed in Military standard 1691.

    Plate "B" - The Functional Diagram Illustrates the specific spatialutilization including dimensional requirements, maximum, minimum and/orpreferred dimensions.

    Plate "C" - The Floor Plan Locates the equipment scheduled in the equipmentprogram with respect to equipment clearances and functional requirements.Some portable equipment not critical to room layout or utility requirements.may not be indicated in this plan, but can be found in the Medical FaciltyRoom Contents List (MFRCL), which is supplied separately for each project.

    Plate "D" - The Reflected Ceiling Plan Coordinates architectural materials,ceiling located equipment, and locates lighting, communications, mechanicaland electrical components with proper relationship to room equipment andfunction.

    Plate "E" - The Composite Plan Combines multidisciplinary information fromPlates "C" and "D" to reflect a total room requirement for equipment andbuilding systems.

    Refer to the following schedules for general notes and graphic "keys" whichapply to all Room Designs and for specific notes referenced in the MatrixData.

    GENERAL NOTES

    1. Doctor's Office and Examination Room are to be identical in all respects with the exception of the equipment and room designation.

    2. Ceiling height to be a minimum of 8'-6" with a minimum of 9'-0" to

    structural support for ceiling mounted surgical light.

    3. A minimum of 6 is required between room walls and prefabricated Audiometric Testing Room. The Audiometric Testing Room will require a recessed slab and special structural design due to the weight of the rooms

    4. Add 6 NSF for Nurse Server and 5 NSF for Wardrobe to Bedroom Area.

    5. Patient Service Console M5541 shall be provide in all Labor Rooms in Medical Centers and in 25% of the Labor Room in MEDDACS.

    6. Area includes 80 SF for Plaster Preparation.

    7. Add 12 NSF for Nurse Servers and 10 NSF for Wardrobes to Bedroom Area.

    8. Add 24 NSF for Nurse Servers, 20 NSF for Wardrobes and 20NSF for a lavatory to Bedroom Area.

    9. Walls for Seclusion Bedroom shall be reinforced with plywood or steel backed GWB. All items to be tamperproof and installed with tamperproof fasteners.

    10. Add 8 NSF for undercounter Nurse Server and 5 NSF for Wardrobe to Bedroom Area.

    11. Add 16 NSF for Nurse Servers, one undercounter and one full height, 10 NSF for Wardrobes and 20 NSF for a lavatory to Bedroom Area.

    12. Add 32 NSF for Nurse Servers, two undercounter and two full height, 20 NSF for Wardrobes to Bedroom Area.

    13. Add 8 NSF for one undercounter Nurse Server to Anteroom and 5 NSF for a Wardrobe and 20 NSF for a lavatory to Bedroom Area.

    26. The columns shown in patient bedrooms are for purposes of establishing the minimum clear width of these rooms. The selection of a structural system that requires columns along the corridor wall will establish a

    minimum clear dimension of 13 in the patient bedrooms. A structural system without columns on the corridor wall will establish a minimum clear dimension of 12'-6". These dimensions are set, as indicated on the room design for the patient bedroom, by the items to be installed along the corridor wall and entrance to the room. A 13 clear dimension will require a 27' column spacing on the exterior wall and a 12'-6" will require a 26' column spacing. The one patient bedroom indicates the preferred column location when only one exterior column is required.

    27. Add 30 NSF to room for x-ray generating equipment.

    28. Area requires computer flooring.

    29. Square footage must be added to this area as required to meet requirements of largest computer equipment.

    30. If Halogen is used for Fire Protection, special exhaust may be required.

    31. This room must be located on building exterior, occupied areas should not be located above or below this room.

    32. Verify floor tolerances with equipment manufacturer.

    33. Provide ICU type patient service consoles for 25% of recovery beds.

    34. The quantity and type of equipment, infant incubators and/or infant care centers, will be determined by the user.

    NOTES:

    1. Manufacturer's Name and Number are furnished to serve only as examples of the type of products desired and not a proprietary item.

    2. All metal kick plates, stretcher plates, armor plates, pulls and push plates shall be stainless steel.

    3. All door trim protection plates shall be 2" less than the width of a single door and 1" less for pairs of doors.

    4. All closers shall be provided with the capability for regular or parallel arm mounting applications. Delay action type closers will be provided on doors where cart traffic and handicap accessible is a consideration.

    FIRE PROTECTION NOTES

    GENERAL NOTES

    A. Noncombustible construction with one-hour corridor walls are required for unsprinklered buildings.

    B. ICU and CCU patient rooms may be open to a nurse's station as one large room without corridor, provided the space has it s own exits and is not a required exit for another space.

    C. Nurse's station may be open to a corridor provided it is staffed 24 hours/day; does not reduce width of corridor; contains only communication and monitoring equipment, counter and seating, space for charts and one day s supply of forms.

    D. No more than one day s supply of flammable liquid permitted in a room at one time.

    E. One hour enclosure for oil-cooled transformers. Refer to NFPA No. 70 for enclosure ratings for other types and sizes of transformers.

    F. Provide smoke detectors only when waiting areas are open to corridors. Open waiting cannot exceed 250 square feet in patient sleeping areas and 600 square feet in non-sleeping areas.

    G. When several laboratories are contiguous an. d within applicable total area limitations, the labs may be considered as one room.

    H. Engineered Smoke Control/Evacuation System will be considered on a case- by-case basis, depending on building design and other fire protection features utilized. Generally, there will be no equivalences given for such systems.

    I. Local fire department support for the building design must be considered. Generally, totally sprinklered buildings will be the most

    economical design.

    4. HEPA filters having DOP test efficiencies of 99.9% shall be used on air supply systems serving rooms used for clinical treatment of patients with a high susceptibility to infection from leukemia and burns. They

    shall also be used on the exhaust discharge air from fume hoods or safety cabinets in which infections or highly radioactive materials are processed and shall be designed and equipped to permit safe removal, disposal and replacement of contaminated filters.

    For radioactive exhaust, use nuclear grade HEPA filters with DOP efficiency at 97.97% at 0.3 micron.

    SPECIFIC NOTES

    6. Air Supply will in general, be obtained from adjacent areas, mechanicalexhaust will be provided.

    7. The Oral Surgery Operating Rooms will be treated as medical operating rooms. Normal treatment other than oral surgery will be performed in dental treatment rooms.

    8. Hood exhausts and equipment exhausts will be considered in balance of supply and exhaust air quantities.

    9. Four low air exhausts will be provided for improved distribution of clean air and removal of contaminants in operating room.

    10. Audiobooth air supply and return will be through sound attenuating ducts and discharge silencers supplied by the manufacturer. The building supply and return ducts will be connected directly to the audiobooth silencers. These connections will be coordinated with the audiobooth manufacturer.

    11. The Bedroom shall be negative to the anteroom, positive to the toilet and the anteroom shall be negative to corridor for a normal isolation room. For positive isolation room, reverse air flow and increase filtration to 95% efficiency except for toilet, which will remain negative.

    12. Not Used

    13. Temperature and humidity control required.

    14. Separate packaged air conditioning unit may be required.

    15. The soiled side of all undercounter Nurse Servers must e supplied with

    on exhaust connection form beneath, when ceiling exhaust cannot be utilized.

    d. Manufacturers are to meet a plus or minus five percent supply voltage regulation specification. The requirement for power conditioning equipment should be determined for each project.

    e. Provide one 120/208v, three-phase panel per room with minimum of five circuits, located with breaker behind control booth.

    f. Provide shunt trip breaker control switch located on recessed riser duct, 5 feet below ceiling height (see Enclosure - Elevation B).

    (1) Must meet NFPA 70 requirments.

    (2) To assure front clearance, this breaker may optionally be installed on Wall D through vertical riser D-1.

    g. Provide main breaker with 150 amp rating located on Wall D behind panel*, and

    (1) Include for selective circuit restoration.

    (2) Assure for 3-1/2" feet "clear" in front of box face (NFPA 70).

    (3) Locate 4'-0" below ceiling height.

    * This feature may be combined with the shunt breaker on Wall B.

    h. Provide duplex receptacles at 8'-0" on center, 18" above finished floor, with the following exceptions:

    (1) Double duplex (4 outlets) located behing the control booth.

    (2) Duplex located behind the table in open space of Wall A, and

    (3) Duplex located at foot of table.

    i. Where single phase is anticipated,k design the raceway system and breaker back box for 80 KW, three-phase capacity. Where three-phase is used, also install single phase for additional flexibility in the future.

    j. Normally emergency power for X-ray equipment, illumination and duplex receptacles shall be provided through a manual transfer switch to one or two X-ray rooms (including one fluoroscopic unit, if applicable). One X-ray film processor is also included on the emergency power.

    k. Dedicated power:

    (1) Where required, size for the smaller of the loads - two largest load or by the Line-Amperage method (largest load plus 22.5 percent of the remaining unit loads).

    (a) Substations require nomninal unit loading of 10 to 15

    percent of substation capacity.

    (b) This substation sizing does not apply where only two units are supported, in which case the designer should investigate the history of brown-outs and the location of the transformers to determine the need for the dedicated power.

    (2) Where not required, substation being utilized must not support motor loads but amore constant loading (i.e., lighting), and line losses shall be minimized to avoid X- ray contrast problems.

    (3) Required for providing additional cooling to handle

    transformer heat load where substations are located in electrical rooms to avoid build-up of excessive temperatures.

    l. Illumination

    (1) Illumination levels and dimmer devices for all Radiographic rooms shall be in accordance with DOD 4270.1M.

    (2) Location of luminaires and X-ray viewing boxes are at the discretion of individual service or agency.

    (3) Dimmers shall be nearly RFI propogation free.

    3. Case Work:

    a. Case work shall as shown on Guide Plates.

    (1) A pass box shall be specified normally only where the dark room is adjacent to the radiology room. It may be installed for film storage at user's discretion.

    (2) Refer to latest edition of DOD MIL-STD-1691 for the lost of other category "A" equipment.

    b. Universal design shall include rough-in for hand sink, hot/cold water with drain, location as shown.

    c. Warning lights and interlocks should be provided to meet safety regulations.

    4. Structural Support System:

    a. Mechanical supporting devices on both sides of each vertical riser or vertical studs, shown on the Enclosure, shall:

    (1) Provide floor-to-ceiling coverage in all cases with closure strips.

    (2) Be capable of supporting static loads of 220 lbs. vertical-to-horizontal pull.

    b. Ceiling grid, unistrut or equal:

    (1) Spacing is 26" OC with 1-5/8" member thickness resulting in 2'-0" clearance between members and flush with accoustical ceiling.

    (2) Ceiling grid channels shall be provided with closure strips.

    (3) Dead Load bearing capacity.

    (a) Vertical point loading is 800 to 900 pounds.

    (b) Uniformly distributed load is 50 lbs.square foot.

    c. Support system for ceiling mounted X-ray equipment (i.e., C arms) shall be designed to actual support requirements to suit the particular situation. Specify anchor supports and grid elements to suit situation.

    5. Radiology Shielding

    a. As a minimum:

    (1) Comply with the design requirements of NRCP Nos. 33, 34, and 49, and certify accordingly.

    (2) Lead shielding shall be 1/16 " thick up to 7' high above finished floor.

    (3) Where possible, lead shielding shall be applied to exterior side of wall partitions behind gypsum board for protection.

    b. Use of modular shielding (Ray Bar or equivalent) for operator's booth is encouraged.

    c. Specify the operator's view window at 18" by 24" or nextavailable standard larger size.

    d. All duct work, grilles, registers and diffusers shall belocated at a height higher than seven feet clear above thefinished floor. Thermostats with associated transmission lines

    shall be surface mounted.
