Understanding and diagnosing hearing loss

The ears are sensitive sensory organs of the body that are suscep-tible to damage, includ-ing hearing impairment or loss.

The National Insti-tute on Deafness and Other Communication Disorders says approxi-mately 15 percent of the United States popula-tion between the ages of 20 and 69 has high frequency hearing loss due to exposure to loud sounds or noise from work and leisure activi-ties. School-aged chil-dren and even babies can have varying stages of hearing loss. Such loss might have been pres-ent at birth, but it also can be a byproduct of environmental factors. Statistics Canada reports more than one million adults across Canada have a hearing-related disability.

Types of hearing loss

Any person can experience hearing loss and at varying degrees. Medical professionals distinguish different levels of hearing loss as

mild, moderate, severe, and profound. The deafness can be catego-rized in two main types: conductive and sensory-neural.

If sound is blocked from reaching the inner ear in some way, then that person is said to have conductive hear-ing loss. If the problem stems from impairment of sensory cells inside of the ear or neurological impairment that com-promises reception of sound or understanding of language, the person is said to have sensory-neural hearing loss.

Hearing loss testing

Tests will determine the type and the sever-ity of hearing loss. Hear-ing tests may be part of routine examinations performed by pediatri-cians as children age. But once people reach adulthood, hearing tests are not common unless they are requested in response to a specific medical issue. The Amer-ican Hearing Research Foundation identifies six types of hearing tests.

* Bedside testing: Doctors can use screening proce-dures to identify symptoms of hear-ing loss. A doctor may use some sort of common sound, such as whispering, snapping or a tick-ing watch, to quan-tify a patient's ability to hear sounds. He or she also may use tuning forks to test at chosen frequen-cies. The Rinne

test is a common tuning fork test that identifies hearing discrepancies when the tuning fork is placed on the mas-toid bone and then one inch from the external ear canal.

• Audiometry: This test is conducted using an audiometer that gives off a vari-ety of pitches. When higher levels of decibels are needed, this indicates hear-

test is a common tuning fork test that identifies hearing discrepancies when the tuning fork is placed on the mas-toid bone and then one inch from the external ear canal.

• Audiometry: This test is conducted using an audiometer that gives off a vari-ety of pitches. When higher levels of decibels are needed, this indicates hear-

ing impairment. The source of the hearing loss then needs to be identified.

• Tympanometry: This is a measure of the stiffness of the eardrum to evaluate middle ear function. Tympanometry can identify fluid in the middle ear, perfora-tions and abnormal middle ear pressure.

• Brainstem auditory evoked responses: This test, often referred to as a BAER test, measures the timing of electri-cal waves from the brainstem in response to clicks sounded at the ear. Both ears are tested and then delays in one side to the other may be indica-tive of a problem.

• Electrocochleog-raphy: This test is a variant of the BAER and uses an electrode placed on or in the ear drum to amplify sound waves.

• Otoacoustic emis-sion: This is a relatively new pro-cedure used to as-sess hearing loss in newborns and also verify if the cochlea, which is the spiral-shaped hearing organ inside of the

inner ear, is working correctly. A small probe is inserted into the ear, and quiet tones are sent to simulate move-ment of the hairs on the cochlea. Should these hairs move, a tiny microphone will record the hairs' own sounds and produce a response that indicates the cochlea is working.

Identifying hearing loss

Tests are only func-tional if hearing loss is identified and people take steps to confirm diagnosis. Often it is up to parents, spouses and caregivers to recognize the signs of hearing loss

and suggest testing. Some signs of hearing loss are more noticeable than others. Here are some indicators.

• Asking people to repeat themselves

• Turning up the radio or television volume to a level that is loud for others

• Difficulty hearing on a telephone

• Difficulty hearing when women or children speak

• Problems following a fast-moving conver-sation

• Feeling like others are always mum-bling

• Difficulty making out conversations in noisy places

If you have repeat-edly experienced any of these signs, the Ameri-can Speech-Language-Hearing Association suggests scheduling an appointment with your doctor. He or she can determine if an appoint-ment with a certified audiologist is necessary.

Although there is a strong relationship be-tween age and hearing loss, anyone can experi-ence hearing impair-ment. Being informed and seeking treatment can help improve hear-ing and communication in affected individuals.

Seniors who opt for Independent Living must be able to manage their personal needs without intervention

from the facility. In an Assisted Living facility, by contrast, residents re-quire and receive some custodial care, such as help with bathing, dressing, and grooming. Medical care is limited and those who require medical assistance would have the oppor-tunity to move into the CCRC’s skilled nursing unit within the same building. The residents are able to move back and forth between vari-ous facilities in the same general location, as their needs for care change over time. Shift work is a vital and growing part of the modern economy. It’s been estimated that 20% of the work force is engaged in some form of shift work. While extending work outside traditional daytime hours offers obvious productivity advan-tages it also comes with a price. Shift work has been linked to a broad range of prob-lems including: fatigue, sleepiness, insomnia, decreased concentra-tion, depressed mood, gastrointestinal distur-bances, cardiovascular disease, infertility, ab-senteeism, errors, etc.

Shift work causes these problems by mis-aligning what the body wants with what it gets. The body and its internal workings (e.g. hormone production, neurotrans-mitter function, etc.) run on an approximately 24 hour cycle that is tuned to generate alertness during conventional daytime hours and rest during traditional sleep time. In shift work this

gets flipped, and the body is exerted when it is priming for sleep and induced to sleep when it is bracing for activity.

There are a number of

strategies that can help mitigate the effects of shift work. Most of these are centered either on trying to control one’s sleeping environment or

helping reset the body’s clock to facilitate better and more refreshing sleep. They are:

1. Try to avoid frequent shifting between day

and night schedules.

2. Planned sleep sched-ule: a disciplined sleep schedule helps promote an increased amount of

sleep. Napping before work and after the main sleeping period is a common helpful strategy.

3. Protect your sleep environment: A dark, quiet environment can go a long way to promoting sleep during the day. Other helpful tips include avoidance of TV/computer or other stimulating activities be-fore sleep, avoidance of caffeine 6 hours before sleep, etc.

4. Timed light expo-sure: Light is the most powerful way to set your body’s clock and exposure or avoidance at certain times can have a powerful effect in improving alertness, mood, work time tasks and sleep quality. In general, restriction of light in the early morn-ing after a night shift will help facilitate sleep, mood and alertness.

5. Timed melatonin: Melatonin before daytime sleep may help promote sleep in some

7. Prescription sleeping tablets and stimulants: These can be helpful in some but should only be used in the setting of careful attention to

the above strategies and under a physician’s supervision.

8. Be aware of sleep ap-nea: this is a very com-mon disorder that will make it harder (or im-possible) for the above strategies to work.

We’ve all had that mo-ment when you think, “Should I go to the ER for treatment?” Whether it’s your child suffering from an injury, your parent who can’t shake a bad cold, or a spouse who got carried away on home improvements, things happen. There are two options for most medical issues.

Option 1: Go to the emergency room

Hospital emergency rooms (ERs) are set up to focus on medical emergencies. So how do you know when to go to the ER? Severe symp-toms would necessitate a trip to the emergency room. These symptoms may include, but are not limited to, chest pain that could be a heart

attack, having difficulty breathing, high fever, numbness in limbs, severe bleeding, stroke symptoms, allergic reactions (anaphylaxis), and any trauma to major organs.

And what do you bring with you to the ER? A photo ID, list of medications, list of al-lergies, and any other information about the incident will be helpful to the physicians treat-ing you.

Option 2: Visit a doc-tor’s office or clinic

If you feel your symptoms aren’t severe enough to visit the emergency room, you will want to make an appointment with your doctor as soon as possible. West Valley

Maintaining or adopting a healthy lifestyle in-volves a host of factors. Getting off the couch to be more active is one such factor, as is taking control of your diet so it's helping you and not hurting you.

While adopting a more active lifestyle is a rela-tively simple step for many people, modifying a diet is oftentimes a much steeper hill to climb. Dietary habits can be hard to break. But altering your selections so the food you consume is helping you instead of hurting you is possible, and you won't have to abandon your favorite foods, either. The

following are a few tips aimed at helping you make your diet work for you.

* Choose the right foods. Many people feel the culprit behind their unhealthy weight is the por-tions they consume. While portion size might have something to do with it, the foods you're eating are likely the primary suspect. If your diet is devoid of fruits, vegetables and whole grains and the dairy products you choose are not fat-free or low-fat, then this could be why you have difficulty maintaining a

Dark spots, specks, "webs," and other shapes that seem-ingly dance around in a person's field of vision are often referred to as "floaters." Such floaters are typically harmless, but there are instances when they can be indicative of something more serious.

The perception of floaters in the eye is called myodesopsia. They are particularly noticeable when look-ing at a light-colored background, such as the sky.

What causes floaters?

Floaters are formed when very small pieces of the eye's gel-like substance, called vitre-ous, break loose in the inner back portion of the eye. Vitreous is the part of the eye between the lens and the retina and contains a clear jelly called the vitreous humor. The vitreous has a gel-like consistency initially, but as a person ages the vitreous begins to dissolve and liquefy. Some of the undissolved

gel particles can break off and float around in the more watery center of the vitreous, forming floaters.

Shadows from the

floating particles, and not the debris itself, are what you actually see in the eye. As light passes through the eye, shadows from the particles are cast onto the retina. They seem to move around in the eye following the rapid movement of the eye.

When should I be alarmed?

Most eye floaters are quite common and not dangerous at all. Howev-er, consult an eye doctor

if the floaters interfere with your quality of life or impair your ability to read or drive a car.

If floaters suddenly increase or are accom-panied by light flashes or losses in peripheral vision, this could be a sign of a serious condi-tion known as retinal de-

tachment. The National Eye Institute offers that a retinal detachment occurs when any part of the retina is lifted or pulled from its normal

position at the back of the wall of the eye. Left untreated, a retinal detachment can lead to permanent visual im-pairment or even blind-ness in a matter of a few days. Surgery is required to seal any holes in the retina or to reattach the retina to the back of the

Are eye 'f loaters' dangerous?

Routinely visiting the dentist for cleanings and other checkups is one way to prevent periodontal disease, which is usu-ally unrecognizable in its early stages.

Eye floaters are caused by pieces of vitreous gel that break loose in the back portion of the eye.

Many eye conditions, including floaters, are a hereditary condition. If someone in your family is prone to severe eye floaters, there is a good chance you may experi-ence the same. However, taking some precau-tions can help improve eye health and reduce your risk of developing floaters.

• Avoid excessive rub-bing of the eyes.

• Do not sleep face-down where the eyes will be rubbed in sleep.

• Recognize some medi-cations carry side effects that can affect the eyes. Talk to an optometrist about medications you take and discuss poten-tial alternatives.

• Avoid risky sports. Head and eye trauma can lead to new eye floaters. Blows to the head and face can cause vitreous material to detach.

If you have float-ers that are dense and numerous, a procedure called a vitrectomy may be recommended. This procedure removes the vitreous gel along with the floating debris, replacing the gel with a salt solution. A vitrecto-my is a potentially risky procedure, so it's often reserved only for those suffering from severe floaters.

Experimental laser therapy to dissolve the floaters is another po-tential treatment option, though this procedure is still being studied and isn't widely used.

Most floaters are left untreated, and patients are advised to simply live with them and learn to cope.

Eye floaters are com-mon and usually carry no risks. If they become prevalent or are ac-companied by flashes of light, visit your eye doc-tor as soon as possible.

Many individuals have the wrong idea about hospice care. These misconceptions usu-ally stem from personal experiences, good or bad, or that of a friend. Hospice care is a com-monly misunderstood field of care that people think you use at literally the very last days of life. This could not be further from the truth. Every hospice is different. Call the hospice and ask questions to make sure you are getting the best

care and service. Know the facts about hospice care so that you and your loved ones can get the best care possible, when you need it most.

Myth: Hospice care should begin in the last days of a person’s life.

The earlier a person is referred, the more hospice can do to help. Sadly, many people believe that hospice is unavailable until liter-ally the final days of life.

Through hospice, pa-tients can enjoy a high quality of life. Hospice is a high-quality care that focuses on comfort and quality of life.

Myth: Hospice care is expensive.

Hypertension, often referred to as high blood pressure, affects mil-lions of people across the globe. Understanding high blood pressure, or HBP, may encourage adults and children alike to take steps to maintain a healthy blood pres-sure, which can reduce their risk of heart attack, stroke and kidney dis-ease.

What is HBP?

HBP is often associ-ated with men and women who are consid-ered to be constantly on edge, nervous or hy-peractive. But even laid back men, women and children can suffer from HBP. The AHA notes that HBP is typically devoid of symptoms. So people suffering from HBP may not know it if they have not routinely had their blood pressure checked. Over the years many myths have prevailed regarding HBP. Such myths can be danger-ous, giving people a false sense of security regarding their blood pressure. For example, in the early 1900s it was assumed that people with high blood pres-sure experienced more frequent headaches than those with healthy blood pressure. How-ever, a study published

in the journal Neurology found that people with high blood pressure were significantly less likely to have headaches than those with healthy pressure readings. Nose-bleeds have also been linked to high blood pressure, but one study found that 83 percent of patients treated for high blood pressure did not suffer from nosebleeds.

Debunking such myths is important, as it further emphasizes the need to receive routine blood pressure read-ings as the only reliable means for men, women and children to deter-mine if they have high, low or healthy blood

pressure. Anyone experi-encing frequent head-aches or nosebleeds should consult a physi-cian but should not as-sume such occurrences

are a byproduct of high blood pressure.

For example, fried foods are typically loaded with salt and contain more calories than foods that are baked or grilled. Calcium and vitamin D important throughout life

Calcium and vita-min D are frequently touted for their ability to improve bone health. While consuming foods rich in vitamin D and calcium is especially important for aging women, both vitamin D and calcium also help to keep the body strong and vital at any age.

Calcium and vitamin D help fight bone loss, and not just in older women. Younger, active women need it to pre-vent stress fractures and other bone injuries. A 2008 study reviewed the health of 5,000 female U.S. Navy recruits, and those who did not take additional calcium and vitamin D were 25 per-cent more likely to suffer a stress fracture. A stress fracture is a small frac-ture of a bone caused by repeated physical strain. Gymnasts, runners and even marching soldiers can suffer such fractures even if they are other-wise healthy.

Calcium is also needed for other parts of the body. The National Institutes of Health Osteoporosis and Related Bone Dis-eases National Resource

Center says calcium is necessary for the heart, muscles and nerves to function properly. It also helps blood to clot. Furthermore, pregnant women need ample calcium in their diets in order to supply calcium to a growing fetus. If a pregnant woman does not get enough calcium through diet, the baby will draw it from Mom's own bones, threaten-ing the mother's health while leading to bone fragility and increasing her risk of fractures.

Calcium and vitamin D work in conjunc-tion. Although a bal-anced diet may provide enough calcium, many times high levels of salt and protein in one's diet can increase calcium excretion through the kidneys. Also, people who have an intoler-ance to lactose may not be getting the calcium they need. Vitamin D is necessary for the body to absorb calcium. With-out vitamin D, a woman cannot produce enough calcitriol, impairing calcium absorption from her diet. In turn, the body will take calcium from stores in existing bone, weakening them

and preventing the for-mation of strong, new bone.

Women who may have been deficient in vitamin D and calcium can develop osteoporo-sis and other bone-loss conditions as they age. Thanks to osteoporosis, half of all women over the age of 50 will have a fracture of the hip, wrist, or vertebra during their lifetime, accord-ing to the U.S. National Library of Medicine. A drop in estrogen at the time of menopause can contribute to bone loss. Drinking large amounts of alcohol, maintaining a low body weight and smoking can each cause osteoporosis.

In order to maintain bone and body health, there are certain dietary

intake recommenda-tions regarding calcium and vitamin D. Adults under age 50 should consume between 1,000 and 1,300 milligrams of calcium and roughly 600 IU of vitamin D daily. Calcium can be found in dairy products as well as in salmon, shellfish, brazil nuts, dried beans, and green, leafy vegeta-bles. Vitamin D is found in fish, eggs and fortified milk. Vitamin D can also form when the body is exposed to sunlight. Even as few as 10 min-utes of daily exposure to sunlight can produce vitamin D.

Consuming enough calcium and vitamin D can help a woman's body stay healthy as she ages.
