PharmacoEconomics & Outcomes News 361 - 4 May 2002 A barcode medication administration (BCMA) system can substantially reduce the incidence of medication errors, says an editorial in the American Journal of Health-System Pharmacy. The BCMA system was implemented at the Veterans Affairs (VA) Medical Center in Topeka, US. The system scans each medication prior to administration and a visual alert identifies the wrong barcoded patient identification wristband, administration time, drug dosage or no active medication order, and documents the avoided error. Since implementation of the BCMA system, the number of wrong medications dispensed at the VA centre has been reduced by 75%. In addition, patient mix-ups have decreased by 93%, dosage errors by 62%, missed doses by 70% and wrong administration times by more than 87%. Young D.
