Introduction

In 2009, Meeker McLeod Sibley Community Health Services (MMS) began receiving funding from the Minnesota Department of Health through the Statewide Health Improvement Program (SHIP). SHIP aims to improve the health of Minnesotans by reducing tobacco use and obesity through policy, systems and environmental change strategies. MMS has continued to receive funding through each subsequent funding cycle to date. In 2013-14, funds also became available through SHIP to conduct a community health survey to gather county level data from adults on behavioral health and overall health. This report contains the results from the first iteration of the survey. The survey will be repeated in future years to gather additional county-level and tri-county data and measure changes in health behaviors and overall health over time.

Survey Methodology Survey Instrument

SKtaff from the public health agencies representing Meeker, McLeod and Sibley counties along with representatives from Hutchinson Hospital and Meeker Memorial Hospital developed the questions for the survey instrument with technical assistance from the Minnesota Department of Health Center for Health Statistics. Existing items from the Behavior Risk Factor Surveillance System (BRFSS) survey and from recent county-level surveys in Minnesota were used to design some of the items on the survey instrument. The survey was formatted by the survey vendor, Survey Systems, Inc. of New Brighton, MN, as a scannable, self-administered English-language questionnaire.

Sample

A two-stage sampling strategy was used for obtaining probability samples of adults living in each of the three counties. A separate sample was drawn for each county. For the first stage of sampling, a random sample of county residential addresses was purchased from a national sampling vendor (Marketing Systems Group of Horsham, PA). Address-based sampling was used so that all households would have an equal chance of being sampled for the survey. Marketing Systems Group obtained the list of addresses from the U.S. Postal Service. For the second stage of sampling, the “most recent birthday” method of within-household respondent selection was used to specify one adult from each selected household to complete the survey.

Survey Administration

An initial survey packet that included a cover letter, the survey instrument, and a postage-paid return envelope was mailed October 23-24, 2014 to 4500 sampled households (1500 from each county). Two weeks after the first survey packets were mailed (November 4), a reminder postcard was sent to all sampled households, reminding those who had not yet returned a survey to do so, and thanking those who had already responded. Two weeks after the reminder postcards were mailed (November 17), another full survey packet was sent to all households that still had not returned the survey. The remaining completed surveys were received over the next six weeks, with the final date for the receipt of surveys being January 7, 2015.

Completed Surveys and Response Rate

Completed surveys were received from 1423 adult residents of the three counties; thus, the overall response rate was 31.6% (1423/4500). County-specific response rates can be found in the next section.

Data Entry and Weighting

The responses from the completed surveys were scanned into an electronic file by Survey Systems, Inc. To ensure that the survey results are representative of the adult population of each of the three counties, the data were weighted when analyzed. The weighting accounts for the sample design by adjusting for the number of adults living in each sampled household. The weighting also includes a post-stratification adjustment so that the gender and age distribution of the survey respondents mirrors the gender and age distribution of the adult populations of the three counties according to U.S. Census Bureau 2010 estimates.

Reading the Data Tables The tables are organized based on the order questions were asked in the actual survey. The table headings contain the question as asked in the survey and the corresponding question number, except when the variable was calculated based on one or more survey questions. In these instances, it is noted in the table heading that the table variable is calculated. Most tables include responses for one question. The response options for the questions are listed in the top row; however each option is not always included as a separate column in the table, sometimes they were combined. In addition to the overall weighted response percent for all three counties combined, each table contains results for the following demographic characteristics as follows:

All results reported in the tables are weighted percentages and are reported across the row (each row adds up to approximately 100% except when respondents were asked to check all that apply). Most questions were asked of all survey respondents but some were asked of a subset of the survey sample based on response to a previous question; these subsets are indicated in parentheses at the beginning of the table heading. Often the number of respondents for these variables is much smaller and can be compared by the n number reported at the top left corner of each table. This number indicates the actual number of survey respondents and is not weighted. Other variations in this number are due to respondents not answering all questions on the survey.
