Working diligently to bring the highest quality cancer care close to home

We believe that when facing a serious medical condition like cancer, the last thing anyone should have to worry about is traveling long distances to get superior quality care. This philosophy is the driving passion behind the work we are doing today at Mercyhealth, to bring the world’s best oncology specialists, the most advanced cancer-fighting technology available today, and a compassionate team of cancer care professionals.

Since our humble beginnings in 1990, the Mercyhealth Cancer Center has helped thousands of cancer patients and their families meet the challenges of cancer head on, caring for the whole person—from detection to treatment to life as a survivor.

In 2016, we added gynecologic cancer care, and continued to push ourselves to new heights, expanding this practice area so women diagnosed with gynecologic cancers no longer needed to travel for surgery, chemotherapy and other cancer-related services. Our newest gynecologic oncology specialist is the best-of-the-best—an expert in minimally invasive, cutting-edge surgical techniques and so much more.

In 2016, we also added new 3D mammography capabilities at the Mercyhealth Breast Center in Janesville. 3D mammography is clinically proven to significantly increase the detection of breast cancers while simultaneously decreasing the number of women asked to return for additional testing. We also implemented Survivorship Care Plans to help patients thrive after treatment, and partnered with the Wisconsin Tobacco Quit Line, an initiative that helps individuals access free smoking cessation support and aids.

From cancer prevention and detection to a wide array of advanced treatment options to providing compassionate care and education, we do whatever it takes to help our patients achieve the best possible outcomes so they can live life to the fullest. It is my sincere hope that the articles in the following pages demonstrate our unending passion for making lives better for our cancer patients and their families.

Message from the Mercyhealth Cancer Committee ChairmanIt is my privilege to present the Mercyhealth 2016 Cancer Committee Annual Report. We are committed to improving the quality of cancer care through a multidisciplinary approach. Treating a complex disease such as cancer requires expertise, compassion and collaboration. Working with a dedicated team of specialists allows us to help cancer patients cope with the multiple dimensions of their disease.

Our cancer program is accredited by the American College of Surgeons Commission on Cancer as a Comprehensive Community Cancer Program. To maintain accredita-tion, our Cancer Committee, comprised of members across several disciplines, meets regularly throughout the year to monitor the program and to establish goals to improve care. We strive to meet and exceed standards put in place by the Commission on Cancer.

The Cancer Committee oversees the multidisciplinary cancer care conferences that are an integral component of our program and provide a forum for collaboration among specialties. We have a general tumor conference that meets every two weeks and an additional breast cancer conference that meets monthly to review and discuss specific cases. Morbidity and mortality conferences provide supplementary educational opportunities for all cancer providers.

This past year we expanded our services to include Gynecologic Oncology. Dr. Ali Mahdavi, who is board certified in gynecologic oncology and fellowship- trained in laparoscopic and robotic surgery, joined our program to offer women with gynecologic cancers treatment closer to home. In addition, we hired Kathy van Veldhuisen, ANP-BC, ACHPN, an oncology-trained palliative care nurse practitioner, who sees oncology patients in both the inpatient and outpatient setting. She collaborates with physicians and other providers to help support cancer patients during treatment and beyond, focusing on improving their quality of life. Lastly, we initiated a complimentary oncology massage program for our patients who are undergoing treatment. Kathy Bjerke, Licensed Massage Therapist, offers massage therapy to patients free of charge in the medical oncology clinic one day per week.

Through expert staff and a comprehensive array of services, we strive to provide our patients with the highest level of cancer care, from diagnosis and treatment, through supportive care and survivorship. It gives me great satisfaction to share with you the highlights of our program as a testament to our commitment to offer the best possible care to our patients.

Hospitalists ensure exceptional inpatient cancer careDoctors who devote most of their practice to providing care to hospitalized patients are called hospitalists. They are experts in treating very sick patients requiring comprehensive inpatient care and are available to patients around the clock each day.

Mercyhealth’s cancer team works closely and collaboratively with hospitalists at Mercyhealth Hospital and Trauma Center-Janesville, offering the best possible care for our patients. In many cases, Mercyhealth hospitalists take responsibility for the care of our cancer patients during their hospital stay.

When a patient requires hospitalization, the oncologist contacts the hospitalist team to make arrangements. After discussing the patient’s history and treatment details, they work together to formulate the patient’s inpatient treatment plan.

During the patient’s hospitalization, Mercyhealth hospitalists offer evaluation and treatment, and frequently consult with the patient’s oncologist. While the patient’s oncologist stays involved during the entire hospital stay, most of the patient’s ‘round-the-clock care is provided by hospitalists.

The patient’s complete electronic medical record is accessible to all collaborating doctors at all times; this ensures timely and accurate communication between doctors and nurses, and the best possible treatment for the patient.

Once the patient is discharged from the hospital, he is scheduled for follow-up care with his primary care doctor and oncologist.

Pathology’s important role in cancer careWith the rising rates of many different types of cancers being diagnosed each year, early detection has become a primary tool for the diagnosis, treatment and management of cancer. At the forefront of early detection are the pathologist and the laboratory.

Imagine you are getting your annual checkup, and your doctor finds a small mass. You’re referred to a specialist for a biopsy. The biopsy sample is delivered to the laboratory, where it is prepared and analyzed to determine the presence or absence of disease.

Information from the laboratory is used to make approximately 70% of the choices in patient care. Pathologists are physicians who are specifically trained in the use and interpretation of laboratory tests, as well as in tissue examination, both under the microscope and by other methods. They deal with the nature, causes, and class- ification of cancers and other diseases, and provide the information necessary for effective cancer treatment. A cancer diagnosis is made and confirmed by the pathologist, using a number of testing methods.

Mercyhealth’s pathology and laboratory department consists of a group of experienced, well-trained and highly qualified pathologists and laboratorians, who work to ensure that all patients are cared for correctly, based on accurate laboratory testing results.

Supporting smoking cessation

Each year, Mercyhealth joins other community partners in evaluating a Community Needs Assessment to determine the most pressing health issues facing our service region. Smoking is clearly where attention is needed.

While nearly everyone understands that smoking is bad for your health—and a significant lung cancer risk factor—you may be surprised to learn that the risk is even greater for Rock County residents, where the percentage of adults who smoke (18.1%) exceeds the national average.

This is alarming, as lung cancer is the second most common cancer and the leading cause of cancer death for men and women. In March 2016, in an effort to help reduce this risk, Mercyhealth’s Medical Oncology group partnered with the Wisconsin Tobacco Quit Line.

The Quit Line is a Rock County program that helps patients safely kick the habit. The program is managed by the University of Wisconsin Center for Tobacco Research and Intervention (UW-CTRI) and funded by the Wisconsin Department of Health Services.

Today, all Mercyhealth patients are actively screened for tobacco use at each appointment. Those who express an interest in quitting are offered the services of the Quit Line. This free, 24/7 service offers a host of benefits, including:

• Individualized coaching and support calls providing personalized advice on how to quit

• Assistance with choosing a quit date and creating a quit plan

• Information on medications

• Two weeks’ worth of free nicotine patches, nicotine gum or nicotine lozenges

• Secure web forum to share experiences with other tobacco users trying to quit

• Self-help materials

Patients may call the Quit Line as often as they like. The Quit Line also offers referrals to other local quit-tobacco resources and services. Most importantly, the program helps encourage the number-one way to prevent lung cancer—by helping individuals avoid tobacco smoke. Since inception, 35 participants have enrolled in the program.

2015 Cancer prevention and screening focus Mercyhealth’s multi-faceted outreach to promote prevention and awareness

Men’s health education

Mercyhealth included men’s health education at community events throughout the year where there was a strong breast health focus. A prostate cancer fact sheet was created and distributed to all Mercyhealth locations.

• Sandy Mascari-Devitt, Breast Health Navigator, provided men’s health/prostate cancer information at the booth at Project Hope and Central Christian Church in Beloit.

• Dr. Kozak gave a presentation on prostate cancer at his monthly free cancer discussion group at Mercyhealth North.

The efforts to educate the community on prostate cancer weren’t as successful as hoped. The committee agreed to expand and improve on the efforts for 2016.

Prostate cancer screenings

Mercyhealth continues to pursue innovative and effective ways to promote prostate cancer aware-ness and the importance of screenings that can lead to early diagnosis and higher survival rates.

• Our goal was to increase prostate screening offerings throughout the organization. On April 25, 2015, Mercyhealth South hosted an open house and offered prostate cancer screenings.

• Multi-site screenings were offered in Janesville and Lake Geneva with five men screened.

Mercyhealth staff screened seven men in 2015 which was an increase from one man screened in 2014.

Obesity and breast cancer

Mercyhealth continued its focus on obesity in relation to breast cancer.

• Sandy Mascari-Devitt presented breast health information with a focus on obesity at 13 events in 2015. In excess of 500 participants received this information.

• In addition, Sandy was featured on the WCLO radio show in October. She discussed obesity as a risk factor for cancer, guidelines for breast cancer screening, particularly in the aging population, and mammograms.

Multi-site cancer screenings

We are always evaluating our screening events and improving them from year to year. In 2015, we offered screenings in Janesville and added a location in Lake Geneva. We doubled our screening numbers from 2014.

Based on patient feedback at previous screenings, the following changes were made:

• An evening screening time was scheduled, instead of morning sessions that had been offered in the past.

• Participants were offered the choice to schedule a mammogram at the time of the free screening. Dr. Jane Anderson volunteered to be the physician champion for any patient who requested a mammogram but didn’t have a primary care provider.

• All screening results were forwarded to the primary care physician at the request of the participant.

Data

• 37 screenings were done

• 7 patients were scheduled for a mammogram

• 1 patient was scheduled for an aspiration of a breast nodule

• 4 patients were referred to a Dermatologist for further follow-up

• 7 patients were referred to a Primary Care Physician for care

• All findings resulted in benign findings

Mercy Walworth event

• 16 patients

• Dr. Kevin Burns screened 20 patients during a skin cancer screening offered in Oc-tober.

Mercyhealth’s clinical trials programMercyhealth continues to demonstrate a strong commitment to using clinical trials in providing cancer care. Our clinical trial program has continued to grow since its start in 1997. We have had a variety of clinical trials available here for the treatment of many different types of cancer. We offer supportive care trials, symptom manage-ment trials and treatment trials. We provide the latest treatments in a coordinated care approach to ensure the best possible patient experience and to provide patients access to advances in cancer care close to home. In September 2015, our program underwent a routine three-year audit through the ECOG-ACRIN Audit and Quality Assurance Committee and no deficiencies were identified.

When patients with a newly diagnosed cancer are seen in Mercyhealth’s medical oncology or radiation oncology clinic, they are evaluated for participation in clinical trials. Mercyhealth’s medical oncology physicians are members of the ECOG-ACRIN Cancer Research Group (Eastern Cooperative Oncology Group and American College of Radiology Imaging Network) and the Wisconsin Oncology Network (WON). Our radiation oncologist is a member of the Radiation Therapy Oncology Group (RTOG). Through these organizations, and with the collaboration of the National Cancer Institute’s National Clinical Trials Network, the Clinical Trials Support Unit (CTSU), and the independent Centralized Review Board (CIRB), we have access to a large number of clinical trials, and are able to make them available more efficiently to better serve our patients.

The clinical trial program works closely with Mercyhealth’s own Institutional Review Board (IRB) to make sure the needs of patients who are participating in clinical trials are being met. The IRB includes physicians, community representatives and clergy. The IRB oversees the clinical trial program to protect the rights of research participants and to ensure the safety of those patients participating on trials.

Clinical trials offer an opportunity for patients treated at the Mercyhealth Cancer Center to contribute to the improvement of cancer treatment and to benefit from advances in cancer care. One advantage of participation is the extra attention patients receive before, during and after their treatment on a clinical trial.

Through our community-based program, Mercyhealth Cancer Center patients can receive the newest treatments close to home, without having to travel to a larger city. We also help interested patients navigate their way to a referral center for trials that might not be available here.

Mercyhealth Cancer Center: Hematology/Medical OncologyMercyhealth’s hematology/medical oncology clinic specializes in providing care to our patients with hematologic (blood-related) and cancer diagnoses. Our experienced care team includes medical oncologists, a physician’s assistant, registered nurses (RNs), medical assistants, case managers, a dedicated oncology dietitian, patient financial counselors and numerous support staff including a medical records clerk and several receptionists.

This extensive team works together with the guiding principle of providing seamless, compassionate care to the patients needing our services. Conveniently located on the second floor of the Sister Michael Berry Building on the Mercyhealth Campus in Janesville, the clinic also offers satellite services at Mercyhealth Hospital and Medical Center-Walworth in Lake Geneva, Wisconsin.

Individualized chemotherapy treatments are provided by specially trained RNs under the supervision of our medical oncologists. Many of our RNs have advanced certifi-cations through the Oncology Nursing Certification Corporation (ONCC). This certifi-cation recognizes their experience and expertise in providing care for patients who experience the complex problems associated with a cancer diagnosis.

Treatment plans are individually structured based on factors such as the patient’s type and stage of cancer, the patient’s response to treatment, and National Comprehensive Cancer Network (NCCN) guidelines that are based on nationally recognized protocols. Of course, new and cutting-edge treatments are always considered for patients meeting the strict criteria of research studies and national clinical trials. These treatments are set up through the Eastern Cooperative Oncology Group of the National Cancer Institute, as well as the Wisconsin Oncology Network. Clinical trial treatments are monitored on-site by our research nursing staff under the direct supervision of our medical oncologists. More information about clinical trials can be found on page 10.

Additional supportive care provided by our nursing staff includes lab draws, injections, dressing changes, mediport flushes and immunizations. These services are all given through our new Fast Track program. This program was designed with an eye on improving patient flow and access to care. It ultimately reduces wait times for our patients and has led to improved patient satisfaction.

Recently, the clinic added a dedicated oncology dietitian who specializes in educating and counseling our patients on the importance of nutrition. Our dietitian directs general informational meetings for all patients and also conducts one-on-one counseling with patients seeking further guidance.

Additionally, Mercyhealth Behavioral Health Services counselors are available to help patients and their family members with their mental health and emotional needs. This important service can be arranged through our department.

Our nurse navigator acts as a single point of contact for our patients. The nurse navigator acts as a liaison between the patient and all members of the multi- disciplinary care team, including the patient’s primary care physician, radiation oncologists and/or surgical oncologists, and any support services that may be required or requested. The navigator functions, as all of our team members do, to ensure continuity of care and a collaborative approach to care for our patients.

Mercyhealth Cancer Center: Gynecologic OncologyCommitted to offering the best in cancer care, Mercyhealth Cancer Center is staffed by a multi-specialty team that includes medical oncologists, a radiation oncologist, oncology certified nurses and navigators, a social worker/counselor, dietitian, and financial navigator. The oncology team provides compassionate, patientcentered care through personalized treatment plans across the continuum of care, planning each patient’s individual treatment and administering therapy using state-of-the-art treatment plans and equipment. The Center is dedicated to improving cancer care through community outreach services, community education, prevention, screening programs and wellness events.

The most common procedures Dr. Mahdavi performs are related to the treatment of uterine and ovarian cancers.

“I’m one of only a few gynecologic oncologists in the country trained in advanced laparoscopic and robotic surgery techniques,” Dr. Mahdavi says. “For patients who qualify, I provide minimally invasive surgical options for these cancers. That translates into a patient experience that typically includes faster recovery, shorter hospital stay, reduced pain level and improved postsurgical quality of life.”

Dr. Mahdavi was part of the first group in Wisconsin—and one of the first groups in the country—to perform a novel procedure called the sentinel lymph node biopsy for uterine cancer. A safer alternative to traditional dissection, this procedure allows physicians to examine pelvic lymph nodes to determine whether or not cancer has metastasized, rather than removing the lymph nodes entirely.

“With my expertise and additional training in minimally invasive surgery, I am able to perform most complex gynecologic surgeries using a technique called ‘keyhole’ surgery,” Dr. Mahdavi says. “Because I operate through a small incision site rather than the large one required for traditional surgery, the healing time for the patient is shortened.”

Dr. Mahdavi emphasizes the importance of providing personalized medicine, participating in national clinical trials and using cutting-edge technology to achieve best outcomes in cancer treatment.

“My philosophy of care is to treat the individual as a whole — body, mind and spirit — and to value my patient’s needs,” Dr. Mahdavi says. “I also believe in taking a multi-disciplinary approach to the treatment of gynecologic cancers through collaborations with other specialists, including radiation oncologists, medical oncologists and genetic specialists.”

Mercyhealth Cancer Center: Radiation OncologyWe are excited to offer the highest possible quality radiation therapy to residents of Janesville and surrounding communities. Treatment options available at the Mercyhealth Cancer Center include external beam radiation therapy, intensity- modulated radiation therapy (IMRT), image-guided radiation therapy (IGRT), and stereotactic body radiotherapy (SBRT).We are committed to always raising the bar. In fact, the Mercyhealth Cancer Center’s radiation oncology department had some exciting changes in 2016.

Several new developments focus on efforts to bring stereotactic radiosurgery to Mercyhealth Cancer Center. We have acquired and commissioned the BRAINLAB iPLAN radiation planning system and the VisionRT patient tracking system. Coupled with the exceptional imaging capabilities of our Siemens® linear accelerators, these devices ensure the highest possible accuracy in radiation delivery.

These developments provided the foundation for our efforts to bring stereotactic radiosurgery to the Mercyhealth Cancer Center—something we accomplished in 2016.

One of the most significant assets of the radiation oncology department is the staff. Our team members come from a wide variety of health care settings and bring di-verse experiences and expertise to the department. Our team consists of a radiation oncologist, a medical physicist, dosimetrists, radiation therapists, plus a registered nurse, case manager and patient financial counselor. Support services are provided by our friendly receptionist and cancer registry staff.

Our radiation oncologist, Dr. Kevin Kozak, joined the department in 2013. Dr. Kozak completed medical training and a PhD in biochemistry at Vanderbilt University, where he finished first in his class. He then went on to receive his radiation oncology training at the Harvard Radiation Oncology Program.

Dr. Kozak served as a cancer researcher and academic clinician prior to joining Mercyhealth and has more than 70 peer-reviewed articles, book chapters and patents to his credit. He has been recognized as a “Face of Komen” by the Susan G. Komen Race for the Cure Foundation and as a “Top Doctor” by Madison Magazine.

The radiation oncology staff is deeply committed to both community outreach and continuous quality improvement. Along those lines, the department has initiated a monthly Morbidity and Mortality conference to review side effects of cancer treatment and develop system-wide strategies to minimize these effects. More ambitiously, the department has embarked on the process to receive the American College of Radiology Radiation Oncology Practice accreditation. Finally, the department offers a Cancer Facts community discussion group where cancer topics of interest to community members are reviewed in an informal setting.

During the course of radiation treatment, our patients visit every day, Monday through Friday, for five to seven weeks. Our staff takes pride in putting patients first and providing the highest quality care available. Our patients have very positive feelings about the care they receive and the relationships they form:

3D Mammography at Mercyhealth Breast CenterLast year, Mercyhealth began offering 3D mammography exams that uses advanced breast tomosynthesis technology developed by Hologic, a worldwide leader in women’s health. 3D mammography exams using this technology are clinically proven to significantly increase the detection of breast cancers, while simultaneously decreasing the number of women asked to return for additional testing.

How It Works

The breast is a three-dimensional object composed of different structures located at different heights within the breast. These structures can overlap and cause confusion when viewed as a flat two-dimensional image. This confusion of overlapping of tissue is a leading reason why small breast cancers may be missed and normal tissue may appear abnormal, leading to unnecessary callbacks. Using breast tomosynthesis technology, the Hologic system creates a 3-dimensional image of the breast structure, which enables radiologists to more clearly see through overlapping tissue to detect cancers, often at an earlier stage.

The Hologic 3D mammography system was the first FDA-approved breast tomosynthesis system in the U.S. To date, more than 8 million women have been screened with this advanced screening system.

Screening Saves Lives

3D mammography is recommended for women of all ages and breast densities. It allows Mercyhealth radiologists to more effectively pinpoint the size, shape and location of abnormalities. This leads to better detection and greater peace of mind for Mercyhealth patients.

Mercyhealth Cancer Center: Surgical OncologyPatients whose cancer treatment regimen requires surgery look to the Mercyhealth Cancer Center’s surgical oncology department, staffed by board certified surgeons and support professionals with years of experience. Our surgeons’ offices are located at Mercyhealth Terrace, just across the parking lot from Mercyhealth Hospital and Trauma Center-Janesville, where the surgical procedures are performed. The surgical oncology department employs:

Technologically advanced procedures, including minimally invasive techniques that use a small scope. The small incision needed for the scope to enter the body offers the advantages of faster in-hospital recovery, faster healing, less pain and minimum risks as compared to traditional surgical techniques. Screening, surveillance and diagnostic upper and lower gastrointestinal fiber optic endoscopy is also provided by the department.

A wide variety of cancer-related surgeries for all stages of cancer treatment. These include removing cancers completely in the majority of patients to permanently cure them; removing cancer that has spread (metastasized) beyond the original tumor; removing the majority of malignant tissue (called debulking) in preparation for chemotherapy; and reconstructive surgeries that help patients look and feel their best, leading to emotional healing.

Relief From the Pain: Marlene’s StoryFor more than 30 years, the professionals at the Mercyhealth Cancer Center in Janesville have worked diligently to help patients and their families meet the challenges of cancer through a personalized approach. Part of this personalized approach means going beyond the ordinary to help patients manage the very broad range of symptoms and side effects a patient might experience before, during and after treatment. Today, one unique way the Center is achieving this goal is through massage therapy.

Extensive research demonstrates that massage therapy can help cancer patients find relief from pain, nausea and vomiting. As a result, certified massage professionals can specialize in cancer massage, receiving training specific to managing cancer patient pain and anxiety via massage therapy. Thanks to a generous donation from the Mercyhealth Foundation, the Mercyhealth Cancer Center now offers twice monthly massages to Mercyhealth Cancer patients. The program has been booked solid since it began.

Starting in March, Marlene underwent four weeks of radiation therapy at the Mercyhealth Cancer Center. The combination of chemotherapy, surgery, and radiation therapy did the trick: By spring 2016, Marlene was cancer-free and able to shift her focus from fighting the disease to regaining her strength. However, she still was battling neuropathy. If anyone touched her feet, she recoiled in pain. Then she heard about Kathy’s services from her Radiation Oncologist at Mercyhealth, Kevin R. Kozak, MD, and booked an appointment.

Marlene’s first appointment with Kathy wasn’t exactly relaxing—at least, not at first. Kathy started by simply touching the tips of Marlene’s toes, which felt like a sharp knife to Marlene’s hyper-sensitive feet. However, Kathy kept working at it, eventually moving to a kneading motion on Marlene’s feet, as well as massaging the side of Marlene’s chest that had not received the radiation. By the end of the session, Marlene was feeling much more relaxed. “I actually had no symptoms of tingling or pain for more than five hours after the massage,” says Marlene.

“That’s the first time I felt that good since I started experiencing neuropathy.” After that first session, Marlene touched her own feet at home to gradually become used to contact.

Marlene began seeing Kathy on a regular basis. Massage therapy has become immensely popular with the cancer patients, and Kathy’s schedule quickly filled up.

With the help of the massage therapy and the exercises she performs at home, Marlene has been steadily feeling better. She hopes to return to work once she has regained enough strength to handle the physical demands of her job. “Neuropathy is one of those mysteries,” she says.

“You never know how the nerves are going to repair themselves. But I am grateful for all the treatment I have received at Mercyhealth.”

Mercyhealth Palliative Care ProgramSince January 5, 2009, Mercyhealth Hospital and Trauma Center has had an active in-hospital palliative care consultative service.

The World Health Organization succinctly stated, “Palliative care affirms life and regards dying as a normal process, neither hastens nor postpones death, provides relief from pain and other distressing symptoms, integrates the psychological and the spiritual aspects of care, offers a support system to help patients live as actively as possible until death, and offers a support system to help the family cope during the patient’s illness and in their own bereavement.”

In light of that definition, the Mercyhealth Palliative Care Program strives to offer a high-quality service for patients, and their families, with serious or life-limiting illness who are seeking life-prolonging or curative care, or are nearing the last stages of life.

The Mercyhealth Palliative Care Program accomplishes these goals by using an interdisciplinary team approach. This team includes the patient’s primary care physicians, medical and nursing palliative care specialists, plus direct care nurses, clergy and discharge planners.

Mercyhealth’s hospital-based palliative care team includes physicians and nurse practitioners, including Dr. Shekhani who is board certified in hospice and palliative medicine by the American Board of Internal Medicine. In addition, palliative care services are available in the Mercyhealth Outpatient Palliative Care Clinic in Janesville.

By visiting Mercyhealth’s interdisciplinary team, our patients and their families have the opportunity to discuss and plan the complicated goals of care, code status, transition to hospice, symptom management and their care at the end of life.

Many studies have shown that effective palliative care can manage fatigue, anxiety, breathlessness, nausea, depression, constipation and other distressful symptoms. A recent study demonstrated patients with advanced lung cancer actually lived longer when they were involved with early palliative care intervention in their diagnosis when compared to those patients who were not offered palliative care until later. Palliative care can benefit patients with cancer, congestive heart failure, emphysema, AIDS and other serious illnesses.

The Mercyhealth Palliative Care Program strives to coordinate complicated medical decisions through communication and excellent medical care to provide the highest quality of life possible for our patients with serious and life-limiting illnesses.

And after leaving the hospital, our patients can continue palliative care by using the at-home or at patient palliative care services. See p. 18 for more information.

Massage therapy — This gentle technique helps with stress and circulation. Our certified massage therapist uses pressure points in the hands and feet to relieve pain and symptoms. This therapy is used for relaxation and comfort.

New Access to Information for Rock County Patients

The American Cancer Society, Mercy Health System, the Rock County Cancer Coalition and the Wisconsin Well Woman program of Rock County joined together in a new undertaking. Mercy volunteers received training about American Cancer Society programs and services. The goal is to make it easier for patients to access cancer information, and to have volunteers help patients find programs and services, removing some of that burden from staff.

Survivorship Care PlansHelping survivors move forward with hope and joyAbsorbing a cancer diagnosis and then undergoing treatment poses physical and psychological challenges for patients, no matter the situation. A critical part of treatment includes a Survivorship Care Plan to help guide patients as they complete treatment and determine the best course of follow-up for their unique situation.

A report from the Institute of Medicine titled “From Cancer Patient to Cancer Survivor” stresses the importance of giving cancer survivors a comprehensive treatment summary and follow-up plan to ensure optimal health and quality of life going forward. Cooperatively drafting a Survivorship Care Plan is an integral piece of the overall treatment journey for Mercyhealth patients.

What does a Survivorship Care Plan include?

The Survivorship Care Plan, or SCP, is a record that summarizes and communicates what transpired during active cancer treatment. It includes:• Care team information• Diagnostic work-up• Disease characterization• Family history• Treatment referrals• Recommendations for follow-up care and surveillance testing and examinations• Referrals for support services the patient may need going forward • Any other information pertinent to the survivor’s short- and long-term care

Who is eligible?

The Mercyhealth Cancer Committee developed and implemented a process to formulate and disseminate Survivorship Care Plans for patients with Stage 0, I, II and III cancers that are treated with curative intent. Patients typically receive planning services after they have completed initial therapy.

At present, Survivorship Care Plans are provided for patients with breast, lung, colorectal, head and neck, prostate, non-Hodgkin’s lymphoma or general cancers; a gynecologic care plan is in development. Patients who have taken advantage of care planning thus far have welcomed the process, communicating how much they appreciate the information and peace of mind it provides.
