INTRODUCTION:

SUBMISSION INSTRUCTIONS:

1. An electronic version (Word format) of the narrative report with associated appendices (using alphabetic labeling) is due no later than 4 p.m. on Friday, November 3, 2017.

Electronic reports will be submitted through the michigannutritionnetwork.org website:a. Navigate to michigannutritionnetwork.org . b. Hover over the “Support” heading at the top right of the screen. c. Hover over “Requirements” in the drop-down menu. d. Click on “Year-End Report” in the pop-out menu.

The EARS Report and financial report will be completed and submitted online.

2. One complete printed copy of the narrative report, including appendices, should be submitted to Sarah Panken at the address below, postmarked by 4 p.m. on Friday, November 3, 2017.

1) Executive Summary

Provide a one-page (not more than 500 words) Executive Summary of SNAP-Ed activities during the reporting fiscal year. Please remember that this is reviewed by MDHHS and USDA, in addition to your FY17 MFF Project Manager. Include the following for both DE and PSE:

2) Process Evaluation

Process Evaluation is required for all funded SNAP-Ed program components. Types of evaluations are defined in the Evaluation section of the MFF Programming and Operations Manual.

Please limit this section of the report to no more than (5) single-sided pages, excluding materials, photos, etc. Copies of program materials, photos, quotes, or short vignettes that illustrate your program are encouraged and can be attached to this report as appendices.
