Added two new fields • NEW_PATIENT_PO_FLAG
– indicates if a patient is new on the current month’s patient list compared to the previous month’s list for the PO
• NEW_PATIENT_PU_FLAG– Indicates if a patient is new on the current month’s
patient list compared to the previous month’s list for the Practice
These fields are included at the end of the records (prior to the Payer pass-through fields)

Dropped Patients List• Includes a list of patients who appeared on the previous
month’s list, but do not appear on the current month’s list.
• This file includes patients for the entire PO. (There are no practice-specific dropped patient lists.)
• The record layout of the dropped patient list is the same as the other patient list files.

Highest Risk to Lowest Risk. The order will be:

Sort by the 9 Chronic Condition flag fields, in the following order:
