MTM is a clinicalservice provided by li-censed pharmacists, theaim of which is to optimizedrug therapy and improvetherapeutic outcomes forpatients. MTM services in-clude medication therapyreviews, pharmacotherapyconsults, monitoring effi-cacy and safety of medica-tion therapy, and otherclinical services.TennCare's MTM benefit
