Project #12-334

TO ALL BIDDERS OF RECORD:

This Addendum supplements and amends the original Drawings and Project Manual, dated April 1, 2013,

and shall be taken into account in preparing proposals, and shall become a part of the Contract

Documents.

This Addendum shall hereby be done and become part of the Contract Documents the same as if

originally bound thereto.

The following clarifications, amendments, additions, revisions, changes, and modifications change the

original Contract Documents only in the amount and to the extent hereinafter specified in this Addendum.

Acknowledge receipt of the Addendum on the Bid Form.

Bidders are responsible for becoming familiar with every item of the Addendum.

PROJECT MANUAL

A. All references to Bid Due date shall change from April 29, 2013, 2 p.m. to May 3, 2013, 2 p.m.

B. Add Alternate GC6 to Document 00 10 00 Solicitation (Standard Requirements) and Documents

00 41 13 – Bid Form.

a. Change Code Note 4 identifying work to be completed at stairs to Upham Hall

“Breezeway” to Code Note 5.

b. See Sketch SK-L19 for an “Alternative and Acceptable” means to the repair and /

or replacement of the mid-level limestone landing (Code Note 4). Intention to the

Alternative solution is to remove, clean and reduce the size of existing large slabs

into manageable sizes as per the sketch. New cut limestone may be required.

Special attention to seam alignment and aesthetics to original design is required.

c. Change Code Note 5 from: “…precast concrete riser and tread…” to “…new cut

Limestone riser and tread to match original pieces”. Change Detail 1 – Sheet

C2.1 from “Concrete Stone Step” to “Cut Limestone Step” as per sketch SK-L14

including steps identified on Sheet L2.0 located at North and South ends of plaza

area.

d. Revise Code Note 7 to read as “Remove existing Bluestone paver veneer from

walk where indicated. Replace damaged stones with new bluestone paving

stones as detailed in Specification Section 32 14 40 Stone Paving. Existing stone

may be salvaged, cleaned and re-used as desired. Contractor to note existing

Bluestone paver thickness varies. Bluestone pavers with a thickness of 1” to be

used in mortar setting bed applications (landings and breezeway). All pavers

thicker than 1 ½” to be reset in aggregate setting bed applications (walkways on

terrace), per Detail 1 – Sheet L2.0.”

e. Change Code Notes 10 & 11 from “See Civil Engineering Drawings” to “See

Sheet C2.0”.

f. Add Coded Note 18: “Circle in breezeway to be reconstructed to match the

original Existing design” as per sketch SK-L14.

C. YAGER STADIUM LAYOUT PLAN

a. Revise concrete pad to new layout as per sketch SK-L15

b. Revise Code Note 5 to read as: Contractor to provide new 6” dia. Galv. End Post

to abut newly installed decorative metal end post. Pull existing fabric taut and

D. CAMPUS AVENUE SIDEWALK REPLACEMENT PLAN

REPORT: These minutes represent the author’s understanding and interpretation of issues discussed and decisions reached. Errors, omissions or disagreements with the information contained herein, should be addressed to the attention of the writer, within two days of the date received.

1. Pre-Bid Meeting was opened by Terrance Ponder – MU Contract Administrator. Terry introduced MU project Manager – Vinny Cirrito and the A&E Team

2. Everyone was encouraged to register through the MU website for latest addendums, access to plans and specifications. www.pfd.muohio.edu/fco/ - FCO Main – Campus Hardscape Improvements 2013

3. All questions regarding the drawings and specifications to be directed to Clete Benken – MKSK at: [email protected], cc all correspondence to: Vincent Cirrito – MU at: [email protected]

4. All addendums to be issued through MU Contract Administrator’s office: Terrance Ponder.

5. Bid specifics for project:

a. Project Duration: Approx. 90 days after notice to proceed and prior to the start of Fall classes.

b. Budget: $2,050,000 w/ several Alternates. MU noted the award of project will also consider cost of Alternates which may be selected in any combination as determined by MU.

c. Ohio Rev. Code 153 (“Buy American” Provisions) applies to this project.

d. 5% EDGE Compliant is preferred but not a requirement. As per Section 6.1.11 of the Instructions to Bidders, an EDGE Affidavit sheet for each EDGE-Certified Business Enterprise is required as part of the bidding process.

e. Prevailing Wage Rates & EEO requirements are applicable to this Project. Changes to Prevailing Wages during the contract period will not be accepted as a Change Order.

f. Any or all Bids, including Alternates, in whole or in part, and in any order may be accepted or rejected by the University’s Contracting Authority.

g. Sealed Bids to be received by 2 pm on April 29, 2013 at MU Contract Administrators office. (NOTE: Since the Pre Bid Meeting on April 15, the Bid Opening deadline has been extended to May 3, 2013, 2 pm.)

h. Unit prices are required as part of the bidding process. Unit Prices will be used to inform the value and acceptability of Change Orders but will not be used to determine low bid.

i. Project Number – project does not have a number. Designated space on Bid Security Form to remain blank or bidders may enter “N/A”.

j. Bid Guarantee of 10% including base bid and all Alternates is required with Bid.

6. Project Site Review: Clete Benken, MKSK

a. General Comments: i. Contractors advised to read Summary of Work and General Conditions very carefully. Prime

Contractors must have prior relevant experience performing paving and resurfacing projects under demanding scope and schedule requirements.

ii. All work is expected to be of “highest” quality requiring experienced sub-contractors with established records of performance on similar projects. Experience and references will be checked.

iii. The Bishop Woods project is designed to create a more pedestrian oriented, quality landscaped environment. The finished quality of all paving work is paramount. The alignment of joints and the tooling and finish of the concrete work must meet University standards.

b. Bishop Woods (BW) i. Addendum for Electrical/Lighting will be issued for BW area. ii. Wood line is off limits to equipment and storing of materials. iii. Due to new field information an Addendum on drain inlets will be issued. iv. Armstrong Student Center Work, Deliveries and Various “other” projects ongoing at same time.

Coordination of access to areas will be required with other contractors. Sequencing of work in and around Bishop Woods circle required .

c. Upham Hall (UH) i. The project involves the renovation and replacement of Pennsylvania Quartzite / AKA ” Blue

Stone” on sand and mortared-down on slabs. Contractors to match designated University Standard as exemplified by finish quality of recent work near Upham Hall and the Sundial Plaza.

ii. Circle inlay at breezeway to be reconstructed to match the original “Existing” design. iii. No heavy equipment to be used in breezeway to UH. Steam tunnel is located at west entrance

to tunnel and not capable of HD equipment. iv. 3-Day “No Work” period must be adhered to during commencement in Mid-June. Actual dates

to be announced by University. v. Site Walk – Hand rails to be removed, repaired, refinished and replaced. Refurbished hand

rails are not to be modified . vi. Site WalkContractors advised to research and confirm procurement schedule for brick and

brick caps. Time is of Essence if reproduction is required. vii. Site Walk – Limestone medallions in cheek walls to be salvaged, protected and then re-

installed. viii. Site Walk - Large limestone slabs may be replaced with smaller pieces. Addendum to be

issued as to alternative / acceptable method.

d. Yager Staduim (YS) i. Various “other” projects ongoing at same time. Coordination of access to areas will be

required with other contractors. ii. Alternate for Brick Columns to be same as existing columns located at the Main Entrance

(South) to Stadium.

e. King Library (KL) i. Site Walk – Contractors must keep one door active at all times.

7. Questions by Contractors:

a. Is the City coordinating to secure permits for Roadway work with Oxford? Answer: Yes,
