Introduction
This manual shows potential applicants and agents on how to use the MM2H Online
Application System. By using this new system, you will get to generate forms online and
download for printing. Applicants and agents will then submit their supporting documents
together with the printed forms at the MM2H counter.
To access this online application system, you must go to mm2honline.motac.gov.my

New Application
How to apply:
1) Go to mm2honline.motac.gov.my. Click on the “New Application” link or icon, as
highlighted below.
2) You will come to an online New Application form. You will need to fill up all the
information and click on the Generate Application Forms, just as shown below:

3) You will now have registered with the online system, and will need to fill up more
details on your spouse, children, financial, medical. If you are an agent, you will have
to fill up additional information in the agent form. You will now see your passport
number with status Incomplete Form in the Applicant form, as below:
4) Notice that there are green, orange, red and no-entry symbols at the top of the page.
Green – you have filled up and saved this particular form previously.
Orange – you are currently filling up this particular form
Red – you have not filled up this particular form yet
No-entry – you are not allowed to fill up this particular form

From the screenshot example, you can see that you are not allowed to fill up the Spouse,
Children and Sponsor (for agent) forms. To enable these forms, click on the green
symbol for the General Info, as shown below:
5) You are now in the General Info form, and you can see some of the information that
you have filled up before. Check the boxes for Spouse and Children if you want to fill
up these forms. If you are an agent filling up for the applicant, you must choose the
submission method “Through MM2H Agent”. After that, click on the “Save &
Continue” button to save the form and continue to the next form, i.e. the Applicant
form.
6) You are now in the Applicant form. You can continue filling up the form and the next
ones. In case that you cannot complete the forms for now and need to continue the

application at a later time, you can click on the “Exit” button at the bottom. You will
be back to the front page of the system.
Caution:
Make sure that you have saved the filled up information by clicking on the “Save”
buttons located at the bottom of the forms, or you will end up losing the filled up
information in the forms.

Complete and Submit
If you have not completed your application and want to continue, follow the steps below:
1) Go to mm2honline.motac.gov.my. Click on the “Complete and Submit” link or icon,
as highlighted below.
2) Fill up the necessary information and click on the “Search for My Application” button

3) You are now back into your online application forms. Fill up the information in the
Applicant form and click on the “Save & Continue To Next Section”. Clicking on the
Save button without filling up the mandatory fields will trigger warning messages, as
shown below:
4) Make sure that you have filled up all the required information. Click on the “Save &
Continue To Next Section”, and you will arrived in the next form, which is the Spouse
form.

5) Fill up all the details for the spouse. In the event if the applicant has more than one
spouse and legally married, additional spouse can be added by clicking on the “+ Add
Spouse” button at the bottom of the form.
6) Doing the previous step will save entered information of the first spouse and refreshes
the page for the next spouse. You can see in the picture below that you are now about
to enter information for the second spouse. You can add more spouses by repeating the
previous step. Clicking on the “Cancel Spouse Addition” button will stop the addition
of another spouse.

* Once you save the details of the spouses, you can always come back to the Spouse
form and view any of the spouses’ details by clicking on the spouses’ links; e.g. Spouse
1, Spouse 2, Spouse 3, etc.
7) Click on “Save & Continue To Next Section” button at the bottom to go to the next
form, which is the Children Below 21 form. Your child must be under 21 years old to
be eligible for this program.
8) Fill up all the details for the child. In the event if the applicant has more than one child
under 21 years old, additional child can be added by clicking on the “+ Add Child”
button at the bottom of the form.

9) Doing the previous step will save entered information of the first child and refreshes
the page for the next child. You can see in the picture below that you are now about to
enter information for the second child. You can add more children by repeating the
previous step. Clicking on the “Cancel Child Addition” button will stop the addition of
another child.
* Once you save the details of the children, you can always come back to the Children
Below 21 form and view any of the children’s details by clicking on the children’s links;
e.g. Child 1, Child 2, Child 3, etc.
10) Click on “Save & Continue To Next Section” button at the bottom to go to the next
form, which is the Medical History form.

11) Fill up the information in the Medical History form and click on the “Save & Continue
To Next Section”. This will bring you to the next form, i.e Finance and Accounts form.
12) Fill up all the details for the first account. In the event if the applicant has more than
one account, additional accounts can be added by clicking on the “+ Add Account”
button at the top of the accounts’ details table, as indicated below. You can add up to 5
accounts:

13) Fill up all the details for all the accounts in the Finance and Accounts form and click
on the “Save & Continue To Next Section”. This will bring you to the next form, i.e
Sponsor form.
14) The Sponsor form should be filled up by the MM2H agent. Fill up all the required
information. If you are not ready to submit your application yet to MM2H, then click
on the “Save & Go To Previous Section” button. This will save your Sponsor form
information and goes back to the previous form, which is the Finance and Accounts
form. Here you can click on the “Exit” button to logout from the application and
continue back at a later time.
15) If you are ready to submit the application to MM2H, then click on the “Save & Submit”
button, as indicated below:
16) You application status is now Pending Supporting Documents. An email will be sent
to the applicant. Your application forms has been generated and available for download.

Please download the MM2H Application Forms by clicking on the “Download MM2H
Applications Forms” button, as indicated below.
17) The downloaded forms are in PDF format. Print the downloaded forms and fill up
whatever extra details needed.
18) Please submit the printed forms together with relevant supporting documents over the
MM2H counter.
19) You may exit now by clicking on the “Exit” button and download the forms later from
this website. You can also check your application status at this website whenever
needed. All these will be explained in the next sections.

Download For Reprint
If you have submitted your application and want to re-download your generated application
forms, follow the steps below:
1) Go to mm2honline.motac.gov.my. Click on the “Download For Reprint” link or icon,
as highlighted below.
2) Fill up the necessary information and click on the “Download My Application Forms”
button, like shown in the example below:

3) In the following page, click on the “Download MM2H Application Forms” button to
download your already submitted forms in PDF format:
4) Click “Exit” if you want to exit the page.

Check Status
If you want to know the latest status of your application, follow the steps below:
1) Go to mm2honline.motac.gov.my. Click on the “Check Status” link or icon, as
highlighted below.
2) Fill up the necessary information and click on the “Check My Application Status”
button, like shown in the example below:

3) In the following page, you will the latest status of the application.
