Personal Applications (Private and Sole Traders) All applicants must provide one form of Original Primary Identification and a minimum of one form of Original Secondary Identification. Requirements are split into primary and secondary. NB: ID sighted must be current, achieve a minimum of 100 points and be an original copy. Company and Government Accounts All applicants must provide a letter of Authorisation/Purchase Order (dated within 60 days of purchase) plus one form of Original Primary Identification and a minimum of one form of Original Secondary Identification. NB: Original Authorisation Letter on original company letterhead or Purchase Order signed by the company representative. Unacceptable ID The following forms of ID are amongst those considered as unacceptable for both personal and company accounts: - Any identification which has expired - Photocopies of ID - Club cards (e.g. RSL, AFL, League clubs, etc) - Store cards (e.g. Myer/Grace Bros, David Jones, Katies, Cash Converters, Sportsgirl, Esprit, etc) - Non Tertiary Student ID - Housing Commission/Rent Assistance cards or passbooks - Business Cards - Group certificates - Frequent Flyer cards - NSW birthcard Business Rules - New post-paid connects on Next G™ 3G and GSM networks. - Does not include pre-paid services. - Does not include handset upgrade to an existing Active service. - Telstra reserves the right to change or withdraw any part of the identification criteria without any prior notice. Corporate Customers Existing Corporate or Government customers and their authorised representatives who have an established agreement with Telstra, i.e> 6 months, will NOT be required to undergo the full 100 point ID check. An original letter of Authorisation or Company Purchase Order must still be provided. The representative must produce a security pass or company ID, the details of which should be recorded on the application. NEW Corporate or Government accounts MUST COMPLETE AND SUBMIT the 100 points ID requirement. Current Telstra Mobile Customer upgrading to a contract All applicants must provide one form of Original Primary Identification or two forms of Original Secondary Identifications. NB: Company upgrades require a letter of Authorisation/Purchase Order. Important: Read the terms set out on the back of this application form, our Business Mobile Services “What you need to know when signing up” Booklet (“Booklet”) and any relevant promotional or pricing brochure. These terms apply to your mobile plan and include requirements to pay charges and other terms which limit your rights and our liability in certain circumstances. If you don’t understand or agree to any term, please speak to the dealer or us before signing. Only sign this section if you wish to be bound by these terms.

By signing this form I understand and agree that, from 12 midnight on the day Telstra accepts the application:

• If I do not already have an existing Group Saver or New Group Saver and wish to establish a New Group Saver, Telstra will establish a New Group Saver in my name on the terms set out in this application form and in the promotional brochure, which I have received and read, and I will comply with these terms.

• I will receive a single account for all the fees and charges incurred by the eligible Telstra mobile GSM/Digital CDMA/3G/Next G Network services listed in Section 1 above.

• I will be legally liable to pay all fees and charges, including minimum monthly spend, access fees for each of the eligible services listed in Section 1 and any additional contract payments, incurred by or applicable to, the eligible services listed in Section 1 above as and when they fall due and all fees and charges incurred by the eligible Telstra mobile GSM/Digital CDMA/3G/Next G Network service listed in section 1.

• If a service listed in section 1 remains subject to a contract term as at the date on which the application is accepted by Telstra and that service is subsequently cancelled by Telstra or me before the end of the contract term, I may be liable to pay an additional amount or earlier termination charge to Telstra.

• If the Existing Mobile Service Owner(s) of the service(s) listed in section 1 has elected a 12, 18 or 24 month Mobile Repayment Option in relation to the service(s), I will be legally liable for all fees and charges associated with the repayment of the telephone (excluding fees and charges for any repayments owed by the Existing Mobile Service Owner under an equipment Financing Arrangements).

Mobile Plans and Telstra Business Mobile Plans Transfer of Ownership/New Group Saver – Terms and Conditions

1. Transfer of Ownership/Services

General You agree that the contracts for Telstra Mobile GSM/Digital CDMA/3G/Next G Network services listed in Section 1 will be transferred to You under the Account nominated in Section 2, on these terms and conditions, the Booklet, Telstra’s Our Customer Terms (as amended from time to time), the relevant promotional brochure (if any) and the original service application form (“Terms”). Telstra may reject this application for any reason, including if the services listed in Section 1 are not eligible to be listed under Your Account. You agree to pay all fees and charges (including any Monthly Spend, if applicable, call charges and fees and charges for any value added services) for each service transferred to You under the Account nominated in Section 2.

You agree that if any of the contracts for the services listed in Section 1 require payment of an amount in certain circumstances, including any fees, cancellation or disconnection charges, You will pay those amounts to Telstra if such circumstances occur. For the avoidance of doubt, the existing contractual obligations in relation to the services listed in Section 1 and transferred to the Account nominated in Section 2, will continue to apply to the services (including any minimum contract term commitment, where relevant).

You agree to ensure that all Service Users comply with the Terms.

CDMA services are not available to new or re-contracting customers.

Additional terms for New Group Saver(a) You must provide us with proof of your Australian Business Number

(ABN), Australian Company Number (ACN) or Australian Registered Business Number (ARBN).

(b) If we accept your application, you understand that each of the Telstra Mobile GSM/Digital CDMA/3G/Next G Network services listed under your New Group Saver account nominated in Section 2 (Group Account) will share in the included voice call value (if any) that is available on services listed under your Group Account. Any unused included voice calls will be forfeited each month. You also understand that each service on your Group Account will continue to receive the Bonus Option/Business Bonus Option or Monthly Credit (if any) applicable to that service on the terms and conditions applicable to that service.

(c) If you already have an existing Group Saver on the account nominated in Section 2, you are not eligible for a New Group Saver on the same account unless you first cancel your existing Group Saver.

(d) You are required to pay the sum of the access fees/monthly spend for all of the services listed under your New Group Saver each month.

(e) You may remove services from your New Group Saver or cancel your New Group Saver at any time by notifying us.

(f) If you elect to remove a service from your New Group Saver, the New Group Saver benefits will no longer apply to that service and all calls will be charged in accordance with the standard rates for that plan.

(g) You must have a minimum of 2, and can have no more than 20 services in your New Group Saver. At least one of the services in your New Group Saver must be a Telstra Business Member Plan service, Telstra Business Casual Plan service or Telstra Business Phone Plan service. You agree that we may cancel your New Group Saver if you do not meet these requirements. We will tell you before this happens.

(h) Certain Telstra plans are incompatible with the New Group Saver. These plans are listed in Our Customer Terms. Services on these plans will not be listed under your New Group Saver.

(i) All services listed under your New Group Saver will continue to receive the benefit of the New Group Saver offer while it is still available. If the New Group Saver offer is no longer available, then we may roll you over to another current group plan which is reasonably comparable. We will tell you before this happens.

( j) In addition to any other rights we may have, you agree that we may cancel your New Group Saver if you do not pay us by the due date or if you fail to remedy any breach (that can be remedied) within 7 days of Telstra notifying that you must remedy the breach.

2. Our Customer TermsTelstra reserves the right to change or withdraw this offer, in whole or in part, as set out in Our Customer Terms (and summarised in the Business Mobile Services Booklet).

Telstra’s Our Customer Terms (as amended by Telstra from time to time) apply to the provision of all Telstra mobile services relevant to this application.

3. Privacy considerationsPlease read Telstra’s “Protecting Your Privacy” statement carefully. It summarises how Telstra, its related companies and its dealers will collect, use and disclose your personal information (including for marketing to you) and your rights in relation to accessing and correcting that information. The Booklet contains the Privacy Statement that is current as at the date of printing the Booklet. The Privacy Statement is also available at http://www.telstra.com.au/privacy/privacy_statement.html

In addition, you agree that we may, subject to the provisions of the Privacy Act 1988 (Cth) in force from time to time:

a) disclose information about you in this application, (including information contained in any application for additional services and information relating to the conduct of your account) to a credit reporting agency for the purpose of obtaining and maintaining a credit information file about you, and to another credit provider or a collection agent for the purpose of collecting overdue payments relating to credit owed by you and notifying defaults by you; and

(b) obtain and use information about your creditworthiness (including a consumer or commercial credit report) from a credit reporting agency, credit provider or other business that reports on creditworthiness for the purpose of assessing an application (including this application and any application for additional services) or collecting overdue payments.

For the purpose of this section 3, “you” and “your” refers to the Account Holder and the Account Holder’s Authorised Signatory (if applicable).

4. Goods and Services TaxWhere GST is imposed on any supply made to You under the terms of this application and the consideration payable for the supply under any other part of this application is not expressed to be inclusive of GST, You must pay, in addition to any GST exclusive consideration payable for the supply, an additional amount calculated by multiplying the value of that GST exclusive consideration (without deduction or setoff) by the prevailing GST rate. Any amount of GST payable by You on a supply made under the terms of this application (including in circumstances where the consideration is expressed to be inclusive of GST) is payable upon demand whether such demand is made by means of a tax invoice or otherwise.

You will be provided with a tax invoice for any taxable supply made to You under the terms of this application.

5. Definitions“You” or “Your” refer to the Account Holder, unless otherwise specified.
