EXTRA! EXTRA! CENTER NEWS!

Notice: Physician Release Form Up- date

We are thrilled that we have so many participants interested in remaining healthy! It is extremely important that you and your

physician work together on an appropriate fitness plan since you are exercising at your own risk.

In an effort to make sure that all participants are using the correct fitness equipment and/or taking the appropriate exercise class we now are requiring

any participant taking exercise and/or dance classes as well as using the fitness room or

thermal plunge pool to have a Physician’s Release Form.

We also need new release forms from all participants meeting any one of the following criteria:

• Your current release form is older than 1 year. • You have had any type of surgery or major change in medications since your release form was initially signed by your Physician . • You are taking exercise and/or dance classes and we do not have a physician release form on file. This may seem like an inconvenience but we want you to remain health and

be aware of any limitations you may have or precautions you need to take.

Please make sure that you are wearing tennis shoes in the fitness room or when taking exercise and/or dance classes.

Effective December 21, 2009 those who do not have a Physician’s Release Form on file will not be allowed to take these classes.

Additionally, anyone attending exercise and/or fitness classes must go through the fitness room orientation.

Call 661-3953 to make an appointment. Due to the high number of orientations it may take up to 2 weeks before you are able to

get an appointment.
