Choose one (1) evaluator with a deep knowledge of the personal history of the organization. T

letter of the recommendation must specify the reasons, circumstances, and impressio

surrounding the organizations life story especially the changes that happened in the regio

council this year that the evaluator thinks are vital to organizations recognition as an outstand

regional council.

Indicated below are the guiding questions for the evaluators.

1. How did you know the regional council?2. What are the strengths and weaknesses of the organization?

3. Does the regional council deserve this award?

4. Feel free to add all necessary information that would help our dear evaluators to know m

about the regional council such as its qualities, how it works as a leadership temple and so

other information which you believe would be value-adding to this screening process.

The evaluators answer must be written on a one-page letter. On the professional letterhe

information below must be included:

recommendation letter will not be honored. The sealed envelope must be attached to the l

The candidate must properly secure the following documents in hard copy.

1. CD copy- Flash/Movie Presentation of no less than 2 minutes but no more than 5 minutesummarizing the activities of the Regional Council, its achievements, etc.

2. Accomplishment Report. Comprising objectives, short description, information andevaluation/report regarding each activities and photos of each event.

Size: 8.5 x 11 (Short Bond Paper, Soft-Bounded)

3. Audited Financial Statements for the Federation Year (duly certified by the Adviser & Au4. Summary of Achievements for Academic and Non-Academic5. Photocopy of all corroborating certificates supporting all claims and assertions made

bid. (It must be in chronological order). Photocopies or pictures of Medals and Plaques ar

accepted. It must be numbered and arranged chronologically (for it is needed on the reference

part of summary of achievements).

Qualifications:

1. The Regional Council should have at least 10 registered participants in the 15th

MYC andatleast 8 registered participants in the 30th Annual National Convention.

The criteria for judging will be divided into three, NF Rating, Evaluation and

Panelists interview. The former will be only 20% of the final score and the Evaluat ion and

Panelists interview will be 30% and 50% respectively.
