MOTION TO TRANSFER SUA SPONTE

Pursuant to R.C. 2111.471, the Court sua sponte moves for an order to transfer jurisdiction over

the above-captioned guardianship to ___________________ County Probate Court, due to the ward’s

relocation to ___________________ County, Ohio. The Court ORDERS that a copy of this motion be

served upon the guardian and the ward. The guardian and ward shall have ten (10) days from the date

this motion was filed in which to file a written objection to the transfer.
