Job Profile To call the patients who have accepted to be enrolled in the Patient Support Program and enrol them by taking informed consent. Create the patient profile by administering questionnaire over the telephone. Provide customized counselling to the patient on Diabetes and its management for appropriate behavior modification Provide expert advice on Life style modification and Diet Should have the ability to develop good relationship with the patients. Maintaining Data after every counselling session with the patient.
