Preface

The MSP Physician Resource Report 2010/2011- 2019/2020 is produced by the Health Sector Information, Analysis and Reporting Division. The Physician Resource Report contains 10 years of statistics on physicians in British Columbia, based on the fee-for-service payments they received for providing services to MSP registrants.

Related publications on MSP data are the MSP Information Resource Manual 2019/2020 and the MSP Fee-For-Service Payment Analysis 2015/2016 - 2019/2020.

Introduction

Under the authority of the Medicare Protection Act, the Medical Services Commission administers the Medical Services Plan (MSP). MSP insures registrants for medically required services provided by general practitioners, specialists and other health practitioners. MSP pays practitioners on a fee-for-service and on an alternative payment basis. The Physician Resource Report contains statistics based on fee-for-service payments made to British Columbia practitioners by MSP for services provided to MSP registrants. The Methodology section outlines general data inclusions and exclusions that apply throughout the manual, as well as methodology specific to practitioner counts, practitioner specialties and days worked.

Methodology

The data used to generate statistics in this manual include MSP fee-for-service payments made to physicians—both general practitioners and specialists—for insured services provided to MSP registrants. Only services performed in a given fiscal year (April 1st to March 31st) and paid on or before September 30th of the following fiscal year are included. Physician Specialty

Public Health (21) and Occupational Medicine (23) are grouped with General Practice Pain Medicine (54) is grouped with Anaesthesia (18) and Radiation Oncology (55) is grouped with Radiology (16) Physicians with the following specialties previously billed under Internal Medicine (15):

• Respirology (49) was effective January 2006 • Cardiology (26), Endocrinology (51), Gastroenterology (56), Nephrology (59), Infectious

Disease (67) and Hematology Oncology (74) were effective August 2007 • Critical Care Medicine (53) was effective April 2010 • Pain Medicine (54) was effective January 2017 • Radiation Oncology (55) was effective June 2016 • Internal General Medicine (57) was effective July 2018 • Palliative Medicine (75) and GP – Emergency Medicine (76) were effective July 2019

Physician Counts

Practitioners are counted only once per fiscal year using the following conditions:

• Only practitioners registered with MSP are included • Practitioners must have expenditure total greater than 0 (zero) dollars in the fiscal year • Practitioners are assigned to one specialty for each fiscal year, using their Most Recent

Specialty • Total physician count includes physicians with unknown gender. • To maintain confidentiality, physician count equal to or less than 5 is replaced by an

asterisk. Total Services and Expenditure

Inclusions

• Expenditures paid for by MSP (includes the adjudicated fee-schedule amount, retroactive payments, rural retention program payments and tray fees)

• Expenditures paid to physicians for services referred by registered midwives or nurse practitioners

• With the implementation of the Laboratory Services Act on October 1, 2015, fee item

values are based on the new Fee-For-Service Outpatient Laboratory Services Payment Schedule and are subject to change by the Minister of Health

• Expenditures for temporary H1N1-specific fee items in 2010/2011

Exclusions

• Interest on late payment of claims • Payments for General Practice Services Committee and Specialist Services Committee

initiatives • Services associated with the following service codes as these services are charges in

addition to the principal fee: o 9 – General Practice Visit Premiums* o 19 – No Charge Referral o 49 – Procedural Premiums* o 71 – Tray Fees*

• Alternative payments made for contract, salaried, session and other services • Payments for services performed out of province

* Billings are included in the Total Expenditure statistic but are not counted as services to avoid double counting of services.

Days Worked A day is counted as a ‘Day Worked’ if a practitioner’s total payment received for that day is greater than 0 (zero) dollars. Patients per Physician This is the sum of the number of patients who received services from a physician specialty in the fiscal year, divided by the total count of distinct physicians within that specialty. Practitioners are counted the same way as previously described in the ‘Physician Counts’ section. Patients are counted only once per fiscal year using the following conditions: Inclusions:

• Patients seen by a physician regardless of their place of residence (e.g. includes out-of-province patients)

• Patients seen by a physician and who have a non-person personal health number (PHN) reported on the fee-for-service billing claim. Non-person PHNs are associated with organ donors, jail inmates, and so on.

Exclusions:

• Patients treated by physicians where no fee-for-service payment was made for the care provided to the patient in a fiscal year. This happens for patients whose care is paid out-of-pocket and/or if every physician specialist seen by the patient is compensated by the province through alternative payments (e.g. contract, salaried, out-of-province, etc.)
