REFLECTIVE JOURNAL (CLINICAL ATTACHMENT) Name: __________________ Admission Number: ___________ Tutorial group: _______________ It was my 4th attachment in Changi General Hospital as a Diploma in Nursing student and I was attached to a B2 medical ward. As I know, I am already a year 2 student so I will need to further get myself ready for my PRCP. I decided to improve to the best on my nursing skills and try to learn those nursing skills that I haven’t learned in school, so that when I go back to school to learn more skills, I can use the knowledge that I learned in the hospital into the school lessons so that I can understand the school lessons even better. Every day, my clinical instructor will come up to the ward to observe us doing our nursing skills and asked us if we have any doubts. Part of the attachment assessment is to perform a clinical skill in front of her and she will evaluate it for us. So everyone took turns to ask her if she could assess the skill for them, so one of my schoolmate chose NGT feeding for this Bed 34’s patient. My clinical instructor told all of us (total there were 4 student nurses) to gather at Bed 34 to see our fellow school-mates perform NGT feeding for bed 34’s patient. When we all arrived at bed 34 together with the Clinical Instructor, some thoughts came into my mind because this patient doesn’t have a (Tube Feed) label on his wall behind his bed, and also he doesn’t have any NGT feeding requisites on his bedside table. Quickly, I told and asked my clinical instructor whether this patient is really on NGT feeding because Changi General Hospital’s style for NGT feeding patients is that they were always be a (Tube Feed) label on the patient’s wall behind the bed, and also NGT feeding requisites on his bedside’s table. After I told and asked her, she was amazed by my assessment of patient prior to the clinical skills being done and also she praise me by saying “ Wow yong I am amazed by your assessment skills “. She also didn’t realize that this patient doesn’t have any (Tube Feed) tag and NGT tube feeding requisites beside his bed until I told her. We then together asked our schoolmate if this is the correct patient that he is going to perform NGT feeding on it, then our fellow schoolmate told us that he got mixed up with the bed number and it’s actually bed 44 instead of bed 34. From that moment my clinical instructor praised me, I felt that I had really improved since my 1st attachment in Changi General Hospital because when we are a new batch of student nurses, usually we will just follow the orders blindly without knowing why we need to do this and what are the prior assessment before performing this particular clinical skill. Lastly, I will still continue to work and learn hard throughout my journey to become a Staff Nurse.

Title of Learning Activity: Rock-climbing adventure (1-day trip)

Description of the Learning Experience

This 1-day activity has been an enriching experience for me. It was my first time trying out rock-climbing. When I rock climbed for the first time, I was terrified that the rope would break, terrified of the height and exposure, and probably mostly terrified that I would not make it to the top; that I would fail, that I would look foolish in front of my peers. On my ascent up the rock face, I slipped a couple of times, became frustrated, and eventually began to cry. As I grew more tired and discouraged, I let my fear of the activity overwhelm me. I eventually lowered off the climb, convinced that I would never rock climb again.

However, my friend who was very determined to pick up the rock-climbing skill, encouraged me to work through my fears. So I persevered on. When I got into difficult climbing situations, I still experienced moments of panic and I occasionally shut down. I knew that if I was going to keep climbing, I needed to get a grip on my fear.

My instructor provided me with sound advice. He described how he came up with a system of visualisation for himself. He looked at a problem, pictured it in his head, and visualised himself succeeding. He would literally look at a climb or a series of moves and picture himself moving upward, step by step, until he was past the difficult part.

I gave it another try and tried his method. I reached a hard move, then visualised a mental movie of myself pulling and stepping myself through the move. It worked! And I realised that before this, I had always reached hard moves and pictured myself falling from them. Once I began to visualise myself succeeding, I began to climb with more confidence, more grace, and much less fear.

What did I learn from the activity?

Although I may feel alone in the face of my fears, I am probably not the first person to have felt this way, and I certainly would not be the last. I learned to talk to others about my fears, ask for advice, and get some help. Getting some support can be critical in learning to cope in the face of fear. Knowing that someone else supports my ideas or believes that I can succeed can make the difference between triumph and failure.

Visualising success on a regular basis also translates into increased self-confidence. Once I begin to regularly view and think of myself as successful, I meet with more triumphs, and I begin to feel more self-assured.
