Background
• Over the past six months, NHC’s VHA members have begun asking for assistance and support to inform patients about complex insurance information and health reform.
• In reviewing the resources that would be available to educate patients, NHC identified a series of gaps in the information that people can obtain about insurance.
• Patients are at higher risk than other consumers when it comes to plan selection. Enrollment in one plan versus another could mean a difference of thousands of dollars in out-of-pocket costs over the course of a year.
The overall aim of this project is to better inform patients the out-of-pocket implications of plan choice and to help them consider the factors to weigh in deciding on a particular marketplace plan. For patients who have high health
needs, the importance of plan choice is vital.
Objectives
NHC is re-launching the Putting Patients First® platform to: • Establish Putting Patients First® as a credible & reliable resource for
exchanges/marketplace information. • Empower patients to share their stories as they participate in the
exchanges. • Utilize the story bank data to assist NHC & members with health
advocacy and strategy efforts. • Offer patients a representative estimate of annual OOP spending via
patient vignettes. • Provide a cost estimator for patients to estimate OOP spending, a
tool that is not available through the government. • Help NHC become a more effective voice for people with chronic
conditions by using the data collected to inform advocacy efforts.
Phase 1 Platform
Phase 1 Website Goals
• Redesign the platform to focus more on patients and their educational needs regarding the health insurance marketplace.
• Drive patients to the story bank to share their experiences engaging with the marketplace.
• Create a repository of health insurance marketplace resources and tools from NHC and its’ members.
• Generate excitement for the Phase 2 website launch.
• Provide an opportunity for NHC’s members and partners to give feedback on the platform.

Phase 2 Platform
Overview
Phase 2 of the Putting Patients First® platform will encompass two major components:

• Estimate My Costs
Phase 2 Website Goals
• Continue to drive patients to share their enrollment and exchange stories.
• Provide patients with the ability to explore plan estimates via patient vignettes.
• Highlight how various heath plans can result in different out-of-pocket costs for patients.
• Identifying some of the features of plan design that will have the biggest impact on out-of-pocket costs.
Explore My Options: Overview
• A series of six patient vignettes will illustrate how plan choice affects OOP spending.
• The six patients will reflect a range of healthcare utilization patterns from minimal utilization to higher utilization.
• The vignettes will highlight additional elements that could affect OOP spending, such as the in-network/out-of-network cost differential, unexpected medical expenses, or utilization management.
• Users will be encouraged to explore all of the different patients to see how patients with low health needs fare across plans compared to patients with medium and/or high health needs.
Estimate My Costs: Overview
• An estimator to give patients a personal look at the range of potential OOP spending under different exchange plans.
• Users enter personal details, including state of residency and age; expected medical services used; and a list of their medications.
• The estimator will pull actual plan coverage details from their state and apply the user’s utilization to each of the plans.
• Users will receive a final report that offers insight—including graphs and charts—into the types of plans that might be best for the user, based on the data entered.
Estimate My Costs: Data Elements
• Personal Data (i.e., age and state)
• Medical Data (i.e., primary care visits, specialist visits, emergency room visits, hospitalization, outpatient surgery, and outpatient mental health visits)
• Drug Data (i.e., predictive typing entry mechanism for users to enter all of their medications)
• Premiums (i.e., age-adjusted premiums)
• Plan Coverage Details from SBCs (i.e., 8-10 plans per state across bronze, silver, gold, and platinum)

Phase 1 Platform Release • Soft launch of Putting Patients First® platform to NHC members and partners. • Education of the platform will be provided for NHC members and partners via
meetings and/or webinars. • Small scale social media push to drive patients to share their story on Putting
Patients First®. • Talking points on key messages will be provided to NHC members and partners.
Phase 2 Platform Release • Email announcements, social media, traditional & digital collateral, newsletters, and
a press release will be utilized to generate buzz from the public and news media. • Search engine optimization will be done on the platform to increase the website’s
prominence. • Talking points on key messages will be provided to NHC members and partners. • Additional education of the platform will be provided for NHC members and
partners via webinars.
