National Honor Society DIRECTIONS FOR FILLING OUT THE INFORMATION FORM

Please PRINT out the NHS application and complete all the sections. Do not be modest!!!

I. Administrative Information Fill in your Name, your PICTURE, Grade and Section, and Class Schedule.

II. Co-curricular ActivitiesPart A: Clubs and Activities. List clubs and activities outside of sports. Check the years you participated, the names of advisers, and a description of your participation. (member, officer, responsibilities). Have the adviser for each activity initial the box provided. In addition, ask one activity advisor to complete the feedback form, seal in an envelope with signature on the flap and include this sealed feedback form with your application. (NOTE: The feedback form for all activities is available on the Brent web site under Academics/Upper School/National Honor Society.)

Part B: Sports and Teams. List sports and/or teams you have been a part of. Check the years youparticipated, the role you played (position, leadership role – for example, captain, or responsibilities –for example, manager). Have the coach initial the box provided. In addition, ask one coach tocomplete the feedback form, seal in an envelope with signature on the flap and include this sealedfeedback form with your application.

Part C: Community Service. List all of the activities in which you have participated for thebetterment of the community (Binan area or your home area) and note major accomplishments in each.Designate whether it was an organized “in-school” (for example, Habitat for Humanity) or “out ofschool” (for example, a church or other volunteer work) service or experience. Work experience (paidor volunteer) would be responsibilities working in a business environment. In addition, ask onecommunity service advisor to complete the feedback form, seal in an envelope with signature onthe flap and include this sealed feedback form with your application.

III. Leadership, Recognition and AwardsList all elected or appointed leadership positions held in the school, community or work environment.These would include positions in which you were responsible for directing or motivating others (forexample, team captain or newspaper editor). Also list any honors, awards or recognitions whichsupport for bid membership in N.H.S.. Do not list honor roll role awards or headmaster list awards,etc. -- awards that deal with scholarship. List awards such as the Brent pride award or all APACawards, etc..

IV. Personal ReflectionPlease include a written exposition in the space provided, explaining why you would like to be amember of the National Honor Society. If necessary, you may attach one extra sheet of paper to thisform for your personal reflection.

III. Co-curricular ActivitiesList all sport teams, clubs/activities, and community service in which you have participated during Upper School. There is a separate section for each area. Be sure to list and/or explain your major accomplishments in each involvement. For community service list all community activities in which you have participated and note any major accomplishment in each. However, each activity needs to be designated as “in-school organized” (yes) or “out of school organized” (no) in the appropriate column. “Outside of school activities” are activities you have participated in for the betterment of the community, but were not organized in-school. For example, church groups, volunteer groups, or community art endeavors. An example of an “in-school organized community activity” would be Gawad Kalinga.
