1 National Honor Society DIRECTIONS FOR FILLING OUT THE INFORMATION FORM Please PRINT out the NHS application and complete all the sections. Do not be modest!!! I. Administrative Information Fill in your Name, your PICTURE, Grade and Section, and Class Schedule. II. Co-curricular Activities Part A: Clubs and Activities. List clubs and activities outside of sports. Check the years you participated, the names of advisers, and a description of your participation. (member, officer, responsibilities). Have the adviser for each activity initial the box provided. In addition, ask one activity advisor to complete the feedback form, seal in an envelope with signature on the flap and include this sealed feedback form with your application. (NOTE: The feedback form for all activities is available on the Brent web site under Academics/Upper School/National Honor Society.) Part B: Sports and Teams. List sports and/or teams you have been a part of. Check the years you participated, the role you played (position, leadership role – for example, captain, or responsibilities – for example, manager). Have the coach initial the box provided. In addition, ask one coach to complete the feedback form, seal in an envelope with signature on the flap and include this sealed feedback form with your application. Part C: Community Service. List all of the activities in which you have participated for the betterment of the community (Binan area or your home area) and note major accomplishments in each. Designate whether it was an organized “in-school” (for example, Habitat for Humanity) or “out of school” (for example, a church or other volunteer work) service or experience. Work experience (paid or volunteer) would be responsibilities working in a business environment. In addition, ask one community service advisor to complete the feedback form, seal in an envelope with signature on the flap and include this sealed feedback form with your application. III. Be sure to list and/or explain your major accomplishments in each involvement. For community service list all community activities in which you have participated and note any major accomplishment in each. However, each activity needs to be designated as “in-school organized” (yes) or “out of school organized” (no) in the appropriate column. “Outside of school activities” are activities you have participated in for the betterment of the community, but were not organized in-school. For example, church groups, volunteer groups, or community art endeavors. An example of an “in-school organized community activity” would be Gawad Kalinga.
