Under this goal, the National Quality Improvement team will partner with organisations to prioritise five patient safety areas to deliver targeted improvements in line with the HSE Service Plan (2019) and reflective of Sláintecare (2018) objectives. There are five key areas for priority improvement. These were chosen after much consideration and seek to meet urgent needs identified by the HSE service plan (2019) and to support the Sláintecare (2018) objectives. For more information on specific partnerships available at this time and how to get involved please read on...

What are the priority areas for improvement?

We want to partner with you to help you as you continue to build momentum for improvement and strengthen your own existing quality improvement structures / processes. We have put in place teams who will engage and partner with services to deliver sustainable improvements. Our aim is to support staff that have been trained in QI to utilise these skills to systematically integrate quality into their daily work. The approaches we will use include QI Collaboratives, QI Learning Sets, Site visits, Master classes and connecting people across the country via webex webinars. We will also make available and provide advice on how to use a QI Toolkit and methodology to enable organisations, in particular middle managers, to support staff and develop an organisational approach to QI.

How will we work together?

We know that services in the HSE are at different stages in their improvement journey. However, we also know that if you want to effectively deliver improvements and sustain QI capacity and capability across your organisation, there are several critical success factors that need to be in place. Building on the success and learning of previous partnerships/projects, we identified the following four as crucial… 1. Commitment from organisations to provide Senior Executive leadership and support for QI. 2. An organisational QI team with a dedicated QI lead or equivalent change agent with support to drive our

organisation’s Quality Improvement agenda 3. An appropriate governance system for QI within our organisations to ensure progress and support

implementation. 4. There should be evidence of incorporation of Quality at Board, Senior Management and Quality & Safety

Committees to support the improvement team and monitor measures.

Invite to join a National Quality Improvement Collaborative aimed at

reducing the number of Falls and avoidable Pressure Ulcers

This year we are offering the opportunity for services to participate in a shared QI collaborative which will focus on Reducing Falls and Improving Bone Health Awareness and /or reducing avoidable Pressure Ulcers. Your service can choose to express interest in one or both of these topics.

Who is involved?

We will work with local clinical experts and clinically proven interventions to engage with multidisciplinary teams to accelerate improvements in their service area. The QI collaborative will involve your organisation sending

a team who can work to implement across the organisations, or

one to two multidisciplinary teams from each participating site.

What is the time commitment?

The teams will have to attend three full one-day learning sessions over a nine month period from June 2019 to December 2019 with a further one day celebration event to mark and share successes. They will also need to be released to work on the improvement work within their service.

How will the collaborative work?

We will support frontline teams to incorporate a QI methodology to identify and introduce improvements in their chosen clinical area (reducing falls or pressure ulcers prevention) during action periods between learning sessions. Teams will also be supported during this time via online webex platforms. We have developed an organisational toolkit for QI based on the

Model for Improvement. In tandem with the collaborative participation we will workshop through a QI toolkit to support middle managers to support and mentor your staff. During and following completion of the QI collaborative there will be a report sent to you or your nominated lead for this process, detailing the outcomes of each of your team’s status.

What are the critical success factors that need to be in place?

Building on the success and learning’s of previous partnerships we have identified a number of critical success factors that need to be in place to effectively deliver improvements and sustain QI capacity and capability across your organisation. For example:

Commitment from organisations to provide Senior Executive support for QI

Dedicated QI lead or equivalent change agent and

Ensuring an appropriate governance system is in place within organisations to support the improvement team and monitor measures.

Based on these learning’s we recommend where possible that the participating team includes a mix of those roles as outlined in the expressions of interest form. At least one of the team should work at across the local service e.g. QPS Lead/ QI Lead/ Clinical Director/ Nurse Practice Development.

When is the closing date?

The closing dates for expressions of interest is Friday, 3rd May 2019. It is envisaged that we will undertake a number of phases of this collaborative to meet the demand and include as many frontline teams as possible. In 2017, the National Quality Improvement Team (formerly the Quality Improvement Division) published “A Board’s Role in Improving Quality and Safety: Guidance and Resources”. The purpose of this guidance was to support boards when reflecting on their performance and approach to improving quality and safety. The work of (QI) for Healthcare Boards will build on this publication, and as we plan for 2019 we will be engaging with Hospital Groups to work together to support boards and board members in a number of different ways including:
