Entry into the Public System• GP referral with as much quality information regarding your

main findings and issues as possible. This helps with triage.

• All GP referrals go to the CRS (Central Referral Service).

• Scanned and sent to the hospital with the required clinic in the catchment area (determined by postcode).

• Triaged by the hospital into Cat 1 (0-30 days), Cat 2 (30-90 days), Cat 3 (90-365 days).


Psychological supportØ The Complex Pain Service at PCH.

• A multidisciplinary team runs a CBT-infused rehabilitation

program. Need a referral from a Dr and the be engaged

with PCH.

Ø Arthritis and Osteoporosis WA has clinical psychologists

experienced in working with EDS.

Ø Western Kids Health have Psychologists trained in pain

management and experience with EDS.


Community based funding and supports available

➢ Medicare

• Chronic Disease Management plan – up to 5 allied health sessions annually that attract a Medicare rebate. Must get a plan and referral from GP

• Better Access to Mental Health care – up to 10 individual and 10 group sessions annually with a clinical psychologist, psychologist, mental health OT or mental health social worker that attract a Medicare rebate. Must get a Mental Health Care Plan and referral from GP, or a referral from Psychiatrist or Paediatrian.


➢Community Aids and Equipment Program (CAEP)

· Assists people with a disability to stay in their own homes by providing equipment such as a wheelchair on a long term loan, or funding home modifications. You must meet eligibility criteria and require a GP referral.


➢National Disability Insurance Scheme (NDIS)

· Support for people with a disability, their families and carers.

· For people under the age of 65 with a permanent and significant disability, to

provide the reasonable and necessary supports they need to live an ordinary

life.

· Access to this is not means tested

➢Home and Community Care (HACC)

· For people under 65 with a disability who can not access NDIS· Heavily discounted housework, personal care, meals, social support,

nursing care, transport, equipment, allied health and home maintenance.· The costs are means tested· To determine eligibility contact the Regional Assessment Service (RAS) on

1300 785 415


➢ Centrelink· Newstart Allowance with a sickness certificate - GP can provide a Certificate for

up to 3 months at a time, stating that you are too unwell to work or participate in job seeking activities.

· Sickness Allowance - You must already have a job or be a full time student, and be temporarily unable to work or study because of a medical condition. Requires a medical certificate from your GP

· Disability Support Pension - Must have a condition that is fully treated, prevent you from participating in any work activities and unlikely to improve in the next 2 years. Must have medical evidence that shows you meet 20 points on the “Impairment Table”.

· Carers Allowance – must be giving daily additional care and attention to someone because they have a disability or severe illness, or are frail aged.


➢ Employment Support

· Job Access: get government funded financial support for workplace modifications plus more. Ph: 1800 464 800

· Disability Employment Services: for job seekers with a disability, injury or health condition who need assistance to find a job and/or support in the workplace to keep a job.


➢Welfare Advocacy· Different agencies based on your geographical location.· Free support, including representation and advocacy if you have a

Centrelink Issue, Family assistance Office Issue, Residential Tenancy issue or Social Security Prosecution issue.

➢Health Consumers Council of WA· If you are unhappy with any aspect of your Healthcare experience, the HCC

Advocacy Service provides flexible, individualised support. Provide information about your health rights, help you link with a service, attend appointments with you, or help you make a complaint.


➢ Equipment for Living Grant· For equipment such as an electric bed or chair. Available through the

Independent Living Centre. Must meet eligibility criteria.

➢ Community Living and Participation Grant· For equipment to support people with a permanent disability achieve

greater inclusion in community life. Includes equipment, resources, training and activities connected to their particular social and recreational goals. Must apply according to selection criteria. Available through the National Disability Service
