Since sh e was otherw ise asymptomatic and many in her community had the same “problem, ” she did not seek medical att ention. For the past few yea rs, however , she has had a sensation of suffoc ating a few minutes after fa lling asleep. Physical examination showed a goiter that was large, lobular, soft, and painless, with cyanosis and protrusion of t he inferior lip (Panel A), which was probab ly related t o chronic respiratory insuff iciency . Serum thyrot ropin and free thyroxine levels were normal. Computed tomog raphy (Panel B) reveale d a heterogeneous goiter, 16 cm by 7 cm, that was in contact with t he trachea (arrow) and was displacin g vascular st ruc- tures (arrowheads). Cytologic examinat ion of a s pecimen obtained by aspirat ion show ed a colloid goiter. Surgical treatment was successfully accomplished early in 2007. Copyright © 2007 Massachusetts Medical Society.
