Neurosurgery

Beaumont neuroscience center of excellence

With renowned surgeons, leading edge technology and a collaborative team, the Neurological Surgery department at Beaumont provides world class care to patients of all ages, from newborn babies to elderly adults. We have the most specialty fellowship-trained neurosurgeons of any hospital system in Southeast Michigan. In addition, the operative neurosurgery team consists of dedicated anesthesia teams, nurses and surgical technicians specifically trained for this specialized and very technical discipline. Part of a Level 1 Trauma Center, this high quality and skilled professional group is available around the clock to accommodate all scheduled and emergency cases.

Our neurosurgical faculty is very skilled with new and cutting edge surgical techniques, ranging from skull based treatment of lesions to complex spinal surgery. Many of these surgeries involve the use of minimally invasive approaches, often working collaboratively with surgeons from other services to approach the brain and spine in unprecedented ways. In addition to adult population practitioners, we also have pediatric specialists for the same procedures.

BrainAs the only non-transplantable organ in the body, the brain requires skilled and delicate techniques to treat the different abnormalities and injuries that can affect it. At Beaumont, we treat and repair aneurysms, tumors, arterio venous malformations, hydrocephalus and traumatic brain injury, among other conditions. Our skilled surgical staff is specially trained in brain tumor resection, skull base and endoscopic techniques, microsurgery and Gamma Knife® radiosurgery.

SpineNeuro-spine surgery at Beaumont adopts and even seeks to improve upon endoscopic and minimally invasive surgical techniques whenever possible. Our cutting edge spine techniques result in less blood loss, decreased post-operative pain, lower infection rates and shorter hospital stays for patients. Most patients recovering from neuro-spine surgery now return to normal activities within a few short weeks – a vast improvement compared to the many months of recovery often

seen with less advanced, more invasive approaches. Our neuro-spine specialists treat the most simple of spinal conditions, like herniated discs, as well as the most complex conditions, including multilevel spine trauma fractures, cord tumors and debilitating spinal degeneration.

Peripheral nervesPain, numbness and weakness are frequent neurological conditions present in patients with central or peripheral nerve problems. These issues can be a diagnostic challenge for many physicians as pain from nerve damage can be masked as many other, more common conditions. In addition, peripheral nerve problems are not limited to mechanical compression and also can result from other conditions like diabetes, mercury or lead intoxication, metabolic disorders like gout or drug-related neuropathies like those from chemotherapeutic agents. Single or multiple nerves may be affected by these conditions and a careful and detailed evaluation of a patient’s symptoms, examination and diagnostic tests are required. The multidisciplinary and comprehensive approach to the evaluation of peripheral nerve problems implemented by the Beaumont Neuroscience team can lead to a complete and accurate diagnosis and subsequent treatment in a timely manner. Nerve conditions treated include facial pain, headache, carpal or ulnar nerve compression and trigeminal neuralgia, among others.

Technological advancementCoupling our physicians’ surgical expertise is high-tech equipment designed to assist in providing the best care for our patients. Our neurosurgical operating rooms feature a number of advanced equipment technologies,

Page 5: Neuroscience Center of Excellence

including a computer-assisted guidance system, Stealth Station,® which has become common protocol for craniotomy procedures. Our neurosurgeons are applying this technology to neuro-spine surgery and are experiencing success at exceptional rates. In conjunction, the O-Arm® Imaging system takes a high quality image of a patient’s anatomy, which is then loaded into the Stealth Station® system for a detailed anatomical map. The symbiotic use of these technologies not only assists our neurosurgeons in providing optimal patient care, but also provides safety for all operating room staff by limiting the amount of radiation exposure, which can be considerable with as many procedures as our neurosurgeons perform.

Other operating room suite advancements include intraoperative monitoring that provides a real-time assessment of the status of the nervous system during surgery, operating microscopes that allow microsurgical techniques while maintaining visibility and accuracy integrity as well as endoscopic equipment using fiber-optic cable and high-definition television monitoring.

NeurologyOur team of neurologists is concerned with the study and treatment of disorders of the nervous system, brain, spinal cord and nerves and treats the full spectrum of neurological conditions including epilepsy, Parkinson’s disease, muscular dystrophy, concussion, Alzheimer’s disease, balance and movement disorders, multiple sclerosis, sleep disorders and many others.

Epilepsy and functional disordersThe Epilepsy Clinic at Beaumont Hospital, Royal Oak specializes in the pre-surgical evaluation and medical treatment of epilepsy and seizures in both pediatric and adult patients. This sub-specialty clinic offers collaborative treatment for patients suffering from intractable epilepsy, women of child bearing age with epilepsy and other complex forms of the disorder. The clinic has a comprehensive care approach to treatment offering evaluation and epilepsy management options from epileptologists, neurologists, neurosurgeons, psychiatrists, neuropsychologists and social workers.

If further evaluation is necessary, patients will be directly referred to the Epilepsy Monitoring Unit (EMU) at Beaumont, Royal Oak or Beaumont, Grosse Pointe, which serve as specialized inpatient units designed to evaluate, diagnose and treat children and adults with seizures. Experienced technologists provide patients with on-site monitoring 24 hours a day from the EMU control room and perform neurological testing as events occur. Electroencephalography (EEG) technologists and nurses are specially trained in recognizing and responding to seizures to maximize patient safety. The EMUs offer leading diagnostic capabilities and physician consultation.

Parkinson’s disease and movement disordersThe broad range group of movement disorders can include Parkinson’s disease, dystonia, spasticity, tremor and Huntington’s disease. All are progressive disorders of the brain caused by death or impairment of nerve cells. Beaumont is a leader in using deep brain stimulation (DBS) as a surgical technique to help treat these disorders.

Alzheimer’s disease and cognitive disordersAlzheimer’s disease is the most common form of dementia or mental impairment due to aging and is a complex disease that is irreversible and has a cause that is not yet clear. The disease destroys brain cells, causing problems with memory, thinking and behavior severe enough to affect work, lifelong hobbies or social life. At this time, there is no cure for Alzheimer’s, no way to slow the progression of this disease and no treatment available to reverse the deterioration caused by Alzheimer’s disease. New research findings give reason for hope, and several drugs are being studied in clinical trials to determine if they can slow the progress of the disease or improve memory for a period of time. Our Neurology team works to assess a patient’s symptoms and provide the most effective treatment for improving quality of life.

Neuromuscular disordersNeuromuscular disorders affect the muscles and the direct nervous system control associated with them. Common neuromuscular conditions include multiple sclerosis, muscular dystrophy and myasthenia gravis, among others. Comprehensive diagnosis and rehabilitative treatment offered within the Beaumont Neuroscience Center of Excellence allow safe and effective pain management to be an integral part of treatment for these conditions. Our physicians have innovative options for treating pain and improving quality of life.

Dizziness and balance disordersBalance disorders like vertigo and dizziness are disturbances that cause an individual to feel unsteady or have a sensation of movement or spinning. We offer a multidisciplinary approach in the diagnosis and management of balance disorders that includes specialists in otolaryngology and neurology and uses techniques like electro- and videonystagmogram, audiological testing and auditory brainstem responses.

INterveNtIoNal NeuroradIologyNeuroradiology is a medical specialty that deals with the diagnosis and treatment of the brain, head and neck, spine and vascular lesions using X-rays, magnetic fields and ultrasound. Our interventional neuroradiologists perform special procedures to aid in the treatment of neurovascular head and spine abnormalities. The use of these various procedural and imaging modalities aids in the accurate diagnosis and treatment of disease. Interventional neuroradiology is a minimally invasive approach to the treatment of vascular diseases of the central nervous system and spine. At Beaumont, we perform many procedures designed to treat endovascular diseases and conditions, including aneurysms, vascular malformations, stenosis and tumors of the brain, head and neck. These used to require surgical intervention, but today’s technology allows consideration for endovascular treatment approaches to reach the lesion. Interventional techniques, such as thrombolysis, are useful in the management of patients with acute ischemic stroke. Many disease processes of the spine also may be treated utilizing percutaneous techniques. These image-guided therapies allow treatment of previously untreatable or difficult to access lesions.

strokeAll three Beaumont hospital locations are designated as Primary Stroke Centers by the Joint Commission, an accolade achieved after individually passing rigorous on-site reviews and examinations of patient care. These certifications indicate that Grosse Pointe, Royal Oak and Troy have all been recognized as providing high-quality medical care that follows national, evidence-based standards, meeting the specialized needs of stroke patients and making exceptional efforts to foster better outcomes. This designation is considered a “seal of approval” and recognizes the exceptional efforts of the Beaumont stroke care team at providing advanced care according to the latest stroke treatment standards.

Treatment for Beaumont stroke patients begins in the pre-hospital setting as medical direction and stroke identification is provided to local paramedics and ambulance teams. If stroke is suspected, our stroke team is activated, the patient has immediate brain imaging and a decision is made regarding further imaging or treatments, which could include tPA, interventional options and surgical options.

Our stroke team includes specially trained and certified nursing staff and more interventional stroke specialists than any other hospital in Michigan. Beaumont has dedicated intensive care units (ICU) beds for stroke patients and has interventional neuroradiologists and neurosurgeons that can administer intra-arterial tPA and remove blood clots from the brain. We also feature dedicated neuroscience progressive units.

NeurotraumaBeaumont’s Emergency Centers (ECs) are committed to the treatment of neurotrauma patients with many time sensitive conditions, such as stroke, spine trauma and traumatic brain injury. Our ECs offer leading edge imaging and advanced telemetry for fast, accurate diagnosis of injuries. Our multidisciplinary team of EC staff, trauma surgeons, neuroscience physicians and rehabilitation specialists work together to begin immediate treatment. In addition, Beaumont’s Direct Access Center arranges transport by ambulance or Beaumont One helicopter to our EC from referring health centers.

PedIatrIc NeuroscIeNceAt Beaumont Children’s Hospital, pediatric neuroscientists use advanced technology and are available to treat a variety of conditions in infants and children. As the only hospital in Southeast Michigan providing pediatric interventional treatment of brain aneurysms, arteriovenous malformations and vascular tumors, we reduce or eliminate the need for traditional surgery methods. Our experts are skilled and equipped to handle the most severe head and spine trauma issues in children.

Our state-of-the-art pediatric intensive care unit (PICU) offers dedicated pediatric specialists in every discipline of medicine and surgery available to assist in our young patients’ care around the clock.

In addition to diagnosing autism and diagnosing and treating sleep disorders, Beaumont Children’s Hospital treats attention deficit disorder (ADD), developmental disorders and delay, epilepsy, seizures and headaches/migraines. We also offer board-certified pediatric neurosurgeons who are recognized experts in surgeries for the treatment of aneurysms, dysraphism, hydrocephalus, spasticity, Chiari malformation, brain tumors and spinal

abnormalities, among other conditions. Also, we are the only hospital in Southeast Michigan performing peripheral nerve reconstruction for brachial plexus injury in infants and children.

Beaumont Children’s Hospital is a regional referral center for complex pediatric neurosurgical cases. The Level I Trauma Center at Beaumont, Royal Oak is equipped to handle even the most critical cases.

Beaumont has one of the largest pediatric ophthalmology and retinal services programs in the country. Our Pediatric

Ophthalmology team performs procedures on children with a range of congenital retinal diseases that can compromise sight. Newborns with suspected retinopathy of prematurity come from across the country and around the world to receive vision-saving surgery from Beaumont specialists.

Premature newborns that develop eye problems do not have symptoms, so the diagnosis depends on examination of the back of the eyes by a specially trained pediatric ophthalmologist. Treatment consists of laser applications to the outermost portions of the retina to stop abnormal growth of blood vessels and decrease the risk of retinal detachment and vision loss.

craNIofacIalThe Ian Jackson Craniofacial and Cleft Palate Clinic offers expedited, multidisciplinary care for children born with craniofacial conditions such as cleft lip or palate, as well as adults with facial deformities from various causes.

Assessing and treating patients with craniofacial, cleft lip and/or palate conditions is a complex process that requires the coordination and participation of many specialized

medical experts. Our highly trained team of health care professionals evaluates an

individual’s condition, conducts

necessary tests, makes treatment recommendations, coordinates all diagnostics and care, performs surgery, if necessary, and maintains long-term clinical outcome records. Treatment recommendations and coordinated care decisions are based upon input from the entire team, which includes plastic surgeons, otolaryngologists, ophthalmologists, prosthodontics, dentists, maxillofacial surgeons and neurosurgeons, as well as speech and language pathologists and social workers.

Unique about this clinic is that it treats patients of all ages, not just children.

otolaryNgologyAs a regional referral center, Beaumont has a host of ear, nose and throat specialists (otolaryngologists) who can provide treatments for many neuro-otology conditions. Using the latest technologies, these professionals treat anything from common conditions such as having tubes put into ears to more complex issues dealing with head and neck cancers and craniofacial deformities. Patients from the very young to the most mature can rely on Beaumont ear, nose and throat specialists to provide the best treatment available.

As one of the largest groups of ear, nose and throat physicians in the area, the doctors at Beaumont have a wide range of specialties including otolaryngology/skull surgery, head and neck surgery, cosmetic surgery, pediatrics, general conditions and cancer. Beaumont doctors use image-guided CT or MRI scans to complete many of these surgeries.

Neuro-oNcologyOur specialists diagnose and treat malignant brain and spine tumors in children and adults using neuroimaging techniques like positron emission tomography, magnetic resonance spectroscopy imaging, Gamma Knife® and spinal radiosurgery. The team includes neuro-oncologists, neurosurgeons, radiation therapists and neuroradiologists. Beaumont is a world leader in radiation oncology. Beaumont teams have developed some of the most advanced radiation treatments available, including adaptive radiation therapy, image-guided radiation therapy, innovative brachytherapy, Omnibeam and hyperthermia therapy.

Gamma Knife® treats brain tumors and neurological conditions with pinpoint accuracy by delivering a highly therapeutic dose of radiation to the brain accurately and without the risks of open surgery, like general anesthesia, infection and bleeding. Only the target tissue receives a significant radiation dose and no incision is required. Recovery is quick and treatment is generally done on an outpatient basis.

In addition to tumors, Gamma Knife® can be used for arteriovenous malformations, trigeminal neuralgia, Parkinson’s disease, essential tremors, epilepsy and obsessive compulsive disorder. Beaumont also has the largest practice in gamma radiosurgery in the state, allowing us to treat many focal pain conditions, especially trigeminal neuralgia. Using the most advanced endoscopic techniques, we are able to treat tumors of the brain with minimally invasive surgery.

NeuroscIeNce ImagINgBeaumont Imaging Centers have the newest, state-of-the-art imaging equipment, including digital radiology, Flash CT and open MRI. Flash CT scanners allow unprecedented speed, with studies performed in less than a second and significant reduction in radiation dosage. The speed of the flash CT scanner also allows scans to be performed for uncooperative patients like many pediatric, trauma or confused individuals without the use of sedation or general anesthesia. This newer generation equipment gives clearer images and uses fine-tuned radiation dosage, which means less exposure.

Beaumont Imaging Centers offer CinemaVision goggles for MRI procedures. The goggles help reduce anxiety and fear and, together with earphones, offer a pleasant and entertaining distraction. Patients can watch movies or television during the procedure.

Beaumont radiologists are available on-site 24 hours a day, 365 days a year to provide consultation and are board certified and fellowship trained, ensuring a higher standard of knowledge. Beaumont also has children’s imaging specialists, including dedicated pediatric radiologists, physicists and technologists.

INPatIeNt NeuroscIeNce careNeuroscience intensive care units (ICU) are dedicated to the care of neurosurgical patients admitted from the Emergency Center or from inpatient and surgical suites. The neuroscience ICU teams are composed of neuroscience certified midlevel providers and nurses who also are trained in advanced critical care and work in collaboration to enhance implementation of care pathways for neuroscience patients. A unique program has also been developed with the Pediatric Critical Care team for the care of children with acute neurological problems in the pediatric ICU.
