How to Receive Short‐Term Disability BenefitsDay 1: The employee must notify his/her supervisor of an absence as soon as possible but no later than 24 hours after the first absence. Day 3: The employee should notify HR Connections of the absence, and enter his or her absence in SAP as “STD Employee Sick – 0220” Day 7: An Employee Health Report (EHR) must be submitted to ConocoPhillips Health Services via either email [email protected] or fax to at 918 662‐6873 no later than the 7th consecutive calendar day of absence (consecutive calendar days includes scheduled time‐off, weekends and holidays). Day 30: The next EHR is due. However, if the previous EHR specifies the date of the next physician’s appointment, then you many contact ConocoPhillips Health Services to request an extension that may be granted at their discretion.

U.S. Health Improvement Incentive Program
• ConocoPhillips is committed to supporting healthy and productive employees.
• 2017 Paid Incentives • You will automatically receive the $50 Healthy Weight incentive; a biometric screening is not required.
• Action Needed: You and your covered spouse/domestic partner must certify your Tobacco Free status during your on‐line benefits enrollment to receive the $12.50 per person Tobacco Free incentive.
• 2018 Paid Incentives• Employees hired before June 1, 2017 are required to take action before Sept. 30 to earn the health incentive paid in 2018.
• To be eligible for these incentives employees must complete a biometric screening onsite or their own physician.

Earn the Healthy Weight and/or the Tobacco Free incentive. Healthy Weight incentive: $50/month If your 2017 biometric screening results indicate your BMI is less than 30, you will
earn the Healthy Weight incentive automatically. If your 2017 biometric screening results indicate your BMI is 30 or more, you must

Retirement – Cash Balance Account• Company contributes 6, 7 or 9% of eligible pay based on age and years of service
• Pay credits recognize both age and company service (one “point” for each year of service plus age)– Under 44 points ‐ 6% pay credit– 44 through 65 points ‐ 7% pay credit– 66 or more points ‐ 9% pay credit
• Interest credits are based on 30‐year Treasury rate• Cash Balance Account is 100% vested generally after 3 years
• Participation in the retirement plan is automatic based on eligibility; no action is required on your part
