From the Editor

If you are reading this, then the world did not end on 12/21/12. I am not an expert in the Mayan civilization or astronomy as it relates to calendar systems. The methods used to calculate and create Gregorian, Julian, Hahai and Islamic calendars are complicated and confusing. I applaud the Mayan’s for being able to figure it out as far as they did. But I suppose they did not have the current distractions we face in our culture. Perhaps if there were no Xbox, Netflix or Youtube, I too would be able to spend more time on scholarly pursuits.

We are all busy. That is a fact of our modern life. Dentistry is complex and sometimes confusing. The amount of information we must process on a daily basis would overwhelm

even the smartest Mayan. Like many dentists, I have a large stack of medical and dental journals in my office that would qualify me as a hoarder. My goal is to one day sit down and read all of the articles I have marked with little Post It notes. But I must check my Facebook status first....

I know our members do not have time to read all of the articles in those journals. So, for the 2013, we are adding a new feature called Pearls and Perspectives. These will be very short articles which readers can quickly gain some insight and wisdom on a particular subject. My hope is that the membership will submit articles of wisdom for our readers. I will start off with an article on a patient I saw last year with Factor V Leiden.

We are also starting a new column called Specialist’s Corner, which will highlight cases from some of our specialist colleagues. I encourage any oral surgeon, periodontist, endodontist or pediatric dentist to submit an article for the newsletter.

This is also the beginning of the 83rd Texas legislative session. Please make an effort to introduce yourself to your legislator and support the advocates of dentistry. It is easy just to make a phone call to their offices and even a small contribution goes a long way to let them know you are an active part of their constituency.

I hope all of our members have a peaceful and joyous 2013!

PRESIDENT’S MESSAGEDon W. Morgan, DDS, MA

Albert Einstein once said, “Any fool can approach tasks and make it more difficult; a smart person can approach the same task and make it easier to do with much less effort completing the task often with an even better result quicker.” That is the genius in the process.

The genius process of the San Antonio District Dental Society (SADDS) is in its many members. All those who participate in the various functions of the group help simplify the complex process of maintaining our organization. This multifaceted professional organization cannot successfully exist without the input and energy of its many parts - the members. All of us - not just the officers and directors. Each of us must participate in any way we can. Even the mere act of attending the general membership meetings is valuable to our survival. They dynamic energy of a well-attended meeting lends to the forward progress of the organization.

We have mentioned many times in our past newsletters the importance of recruiting new members.

We also have a list of members who have not renewed their membership. Each one of us should make an effort to recruit new members and to urge those failing to renew to rejoin the group. If the officers and directors need to direct our efforts to correcting errors of omission or commission please let us know. We have several meetings each year which include dental students and recent graduates. We all should make an effort to encourage these potential members to join us. Inform them of the many benefits of membership; opportunities to network with experienced dentists, inexpensive continuing education, reduced cost for insurance and attendance at meetings. In fact, often the savings will offset the cost of membership.

The most pressing event on the horizon is commemorating the first 100 years of SADDS. We have formed a committee to plan the celebration and would welcome participation and suggestions from anyone willing to help. This is a milestone and we should make it a memorable occasion.

Be sure to read the very first SADDS Newsletter Journal, Volume 1, Number 1 from 1946, printed on pages 35-37. Some things have changed quite a bit....and others not so much!

This is a legislature meeting year and we have planned a Legislative Day for all who wish to attend a chance to meet your representatives in Austin and make our voice heard. SADDS has arranged for a chartered

bus for the trip to Austin. Make it a point to call the office or register on the website for the transportation to Austin for Legislative Day on February 20, 2013 in Austin. Join us for this continuing effort to protect our interests in dentistry.

One other thing before I close this brief visit with you. Make plans to attend The TEXAS Meeting May 2-5, 2013. There is a great opportunity with your membership to get some free CDE by assisting one of the speakers as a classroom host. Check the TDA Journal for information on becoming a host or contact Dr. Lisa Masters who is on the TDA Council on Annual Session.

The next general membership meeting is on February 12, 2013 and will once again be hosted by the SADDS Committee on New Dentists and will be featuring eight (8) speakers in our Perils & Pearls format. Until then, keep smiling!

By Dr. Henry ChuAt the beginning of each year, I review hospital cases over the last 12 months in an effort to develop strategies on how to improve our patient care. While sifting through patient files, I ran across an individual who has Factor V Leiden. To protect her privacy, I will refer to her as “V”.

V is a 59 year old, female patient who was referred to us after having been to a dozen other dentists. She required extensive restorative and surgical treatment. Her history was remarkable for Factor V Leiden, history of deep vein thrombosis and surgery for placement of a Greenfield filter. She was in otherwise healthy condition with the exception of being obese. Her medication included Coumadin, Lovenox and Zocor.

Factor V Leiden is a thrombophilia which predisposes the patient to developing abnormal blood clots. Deep vein thrombosis is one of the risks of this condition. The Leiden mutation is resistant to activated protein C which normally inactivates coagulation factor V.

Some of the patient’s previous dentists were suggesting she discontinue her Lovenox and Coumadin before treatment even though her INR was 2.0 and she had no history of abnormal bleeding. Most of the time when patients are being treated with antithrombotics or any “blood thinner”, it is to reduce the risk of thrombus formation in a healthy individual with normal hemostasis. In essence reducing the patient’s clotting ability beyond their normal physiological limits.

For a patient with the Leiden mutation, the medications are used to bring the patient closer to a normal state. So after consultation with the hematologist, we treated her with no changes in her medications. We applied sequential compression device during the procedure and made sure she was ambulatory as soon as possible in post op. The procedure was completed without complications and she was discharged the same day.

The pearl in this case is to remember that the circulatory system is a dynamic balance where the liver and bone marrow is constantly producing coagulation factors which if left unchecked, would cause our blood to spontaneously clot in the vessels. Fortunately, the vascular endothelium is actively producing substances that counteract those clotting factors. There was a time when we thought blood vessels where merely conduits for blood flow. What we now know is that the vascular endothelium is not merely a lining of the blood vessel, but is also an important biologically active structure.

In the next issue we will explore the vascular endothelium, the interaction of these factors and how dentists should use lab reports, medical consultation and common sense to treat patients with bleeding disorders.

Synopsis: Each procedure we perform on our patients has inherent risks that are associated with them. We always intend to help our patients, but complications can and do occur. This presentation will help to identify simple measures that can be employed to avoid adverse outcomes. Early recognition and diagnosis of a complication and basic knowledge of how various situations can be managed will be reviewed.

Course Objectives: Identifying risk factors associated with minor oral surgery procedures; Rec-ognizing and diagnosing when a problem has occurred; Reviewing treatment and management of adverse outcomes.

FEBRUARY GM SPEAKERS & TOPICS

Clinton Falk, DDS - “To Save A Tooth or Not Save A Tooth” AGD Code: 735 - Treatment Planning/Oral Medicine, Diagnosis, Patho

Synopsis:Treatment planning can be the most challenging task dentists face on a daily basis. Deciding on the best treatment option can be difficult for both the patient and dentist. Knowing which option presents the best long-term prognosis provides many advantages. The success of dental implants can make that decision even more difficult. Each individual tooth can present with its own challenges and we will look at prognosis on a tooth by tooth basis. We will review scenarios when a tooth is periodontally, endodontically, and/or restoratively compromised. This course will review the factors involved in making a decision to save a tooth or restore with a dental implant.

Course Objectives: Treatment planning decisions on individual teeth; Factors involved in making treatment decisions; Long-term prognosis associated with treatment; Be able to decide when to save a tooth or restore with implant.

David L. Cloutier, DMD - “Endodontic Diagnosis: A Systematic Approach to Diagnose Endodontic Disease” AGD Code 072 - Diagnosis and Medications (Endo)

Synopsis: An accurate diagnosis is the most critical component of any subsequent dental treatment. To achieve an accurate endodontic diagnosis, a clinician must follow a systematic diagnostic evaluation process. This presentation will review the important methodological processes in obtaining a proper pulpal and periapial diagnosis. This lecture will discuss the limited endodontic clinical exam and diagnostic testing including the following: radiographic interpretation, pulp testing, percussion, palpation, periodontal exam, bite test and transillumination. The current American Board of Endodontics clinical classification of pulpal and periapical disease will also be covered.

Course Objectives: Diagnosing pulpal and periapical disease using a systematic diagnostic evaluation process; Reviewing endodontic examination and diagnostic testing techniques; Reviewing pulpal and perapical classification nomenclature.

Barbara A. MacNeill, MS, DMD - “Dental Considerations for the Pregnant Patient” AGD Code: 750 - Special Patient Care

Synopsis: Pregnant and lactating dental patients are seen quite frequently in the dental office. As more physicians are becoming aware of the oral-systemic relationships, more women are being referred to the dentist during pregnancy for treatment. This presentation provides a review of the proper and safe management of these patients.

Course Objectives: Summarize the physiologic changes associated with pregnancy; Summarize changes in the oral cavity associated with pregnancy; Understand the safe dental management of pregnant and lactating patient.

Stewart “Whit” Pharr, DMD - “The Marius Implant Bridge: An Implant Supported, Fixed Removable Option” AGD Code: 675 Precision/Semi-Precision Attachments/Prosth/Remov)

Synopsis: Edentulous patients requesting fi xed restorations have many prosthetic treatment options. The esthetic need for lip support or patient hygiene limitations may preclude the use of fi xed dental prostheses such as acrylic hybrids or porcelain fused to metal restorations. The Marius Implant Bridge allows for the rigid support of a fi xed restoration as well as the lip support and hygiene benefi ts of an overdenture. This presentation will review key elements in treatment planning, laboratory fabrication and clinical delivery of this unique prosthesis.

Course Objectives: Identifying patients that need a prosthetic fl ange for lip support; Clinical fi ndings that cation the use of fi xed dental prothesis; Treatment planning and sequencing for a Marius implant bridge; Laboratory communication and fabrication.

James R.VandeBerg, DDS, MS - “Transitional Dentition: When to Refer” AGD Code: 371 - Diagnosis and Treatment Planning (Orthodontics)

Synopsis: As children lose their primary teeth and enter the transitional dentition, many problems begin to emerge. Parents have concerns about crowding, spacing and excessive overjet. We will discuss common problems and when it is appropriate to refer to the orthodontist. Often times no treatment is the best course of action. Some of the topics covered will be: Crowding, how much is too much? Class II/III Anterior and posterior crossbites. Premature loss of a primary tooth, maintain space or not? Retained primary canines, why? Thumb sucking.

Course Objectives: What to look for when examining a 7 - 10 year old; Knowing when and when not to refer; What some common treatments are for the orthodontic problems in this age group.

Courtney Alexander, DDS & Susie Hayden, DDS “Hidden Decay - What Are YOU Missing?” AGD Code: 430 - Pediatric Dentistry

Synopsis: With the increased abundance of children drinking sports drinks and eating sour candy we are seeing more kids with “Hidden Decay”. This decay is barely detectable upon examination but once excavation is started, turns into a very large lesion. This is very common in children who have acid refl ux also. This presentation will include multiple case presentations with visuals before and after decay is exca-vated. Dietary sources of acids including sour candies, sticky chewy fruit candies, sodas, sports drinks, lemonades and gummy vitamins also will be discussed. The highlight of the December 11th meeting was the crowning of Miss Smile 2013. For 25 years, San Antonio high school girls have been writing essays about the importance of dental health, competing for a scholarship prize and the chance to ride like a princess atop the SADDS Fiesta float.

The 1st Place award, Miss Smile 2013, was awarded to Stephanie Saathoff who wrote a compelling essay de-tailing why good dental health is important to her personally and how it benefits overall health in the long run. Stephanie is a senior at Hondo High School and has been involved in Varsity Cheerleading, track, and the marching band there. SADDA awarded her a $1000 college scholarship to Texas A&M University, where she will attend in the fall.

The 1st Runner-Up $600 award went to Michelle Hilburn, a senior at Judson High School. Michelle is an active member in the Judson Dental Internship program, and has already earned certification as a Registered Dental Assistant. In addition to being involved with HOSA, an organization for future health professionals, Michelle works with Outreach for Smiles and Tooth Time Community Outreach here in San Antonio.

The 2nd Runner-Up $600 award went to Amanda Esparza, a senior at Harlandale High School. Amanda is her senior class president, as well as an active girl scout. Amanda quoted Anne Roiphe in her essay, stating that “A woman whose smile is open and whose expression is glad has a kind of beauty no matter what she wears.”

CASE PRESENTATION RHEUMATOID ARTHRITIS BY DR. ROBERT LEMKE, DDS., MD.

For patients with rheumatoid arthritis each day’s activities can present many challenges. Even the simplest of tasks can pose significant obstacles.

I am pleased to present to you the case of Mrs. DK that was referred to my practice.

Mrs. DK has severe rheumatoid arthritis. Her fingers are gnarled and have nodules. Her feet have clawtoes, bunions, and and are most often in pain with little movement possible. Because her feet aredeformed, she has a tendency to trip and fall.

During one of these falls, she fractured her few remaining mandibular teeth. Prior to her fall, she had a partial denture, yet was experiencing significant problems using the partial. Removing and replacing the partial was close to impossible.

We used cone beam computed tomography to evaluate the severity of TMJ arthritic involvement, look for pathosis within the bone, and later to assess the integration of implants.

In my office, we extracted her fractured teeth and place two

implants. She decided to pursue the “All on Four” treatment concept whereby teeth are fixed to four implants. We then placed the additional two mandibular implants and the same day converted her denture to a fixed bridge.

Mrs. DK was very excited to regain function and was more than pleased with the esthetics of her new smile.

The patient's masticatory function can be recovered and

maintained in the long term after implant treatment, even for elderly

edentulous patients with osteoporosis, if dental implant treatment is performed following precise examination of the jawbone. Oral hygiene is accomplished very well with the use of a Water Pik.

It is not uncommon to hear patients with arthritis have most if not all of their symptoms resolve after removing their infected teeth. It makes sense that an inflammatory based disorder is only worsened when there are other sources of inflammation in the body. As dentists, we can assist people not only with oral

function and esthetics, but in improving their quality of life.

system which attacks mainly

the healthy tissues and joints

within the system. The bone

and the cartilage becomes

weaker and weaker until it

reaches very low density

levels which are not enough

to sustain a healthy skeletal

structure.

According to study results

presented at the 2012

European Congress of

Rheumatology, tooth loss

appears to be an indicator

for the subsequent

development of rheumatoid

arthritis. A greater number of

missing teeth was also

associated with greater

arthritis disease activity and

worse response to treatment.

RA is a progressive disease

that affects the joints of the

body, though the TMJ is one

of the last joints affected

comprising about 17% of RA

patients.

L.T. Pratt, Sr.1922 - December 20, 2012

Retired for over 15 years, Dr. L.T. Pratt Sr. rose up from the horrors of The Great Depression to serve in WW II, gaining national recognition in the late l950’s for being an aggressive proponent of the use of Flouride in toothpaste and city water systems, one of the most important advancements in the history of dental health. He was the youngest president of the San Antonio Dental Society. His work led to his induction to The American College of Dentists, an invitation-only honorary fraternity of achievers in his profession. Dr. Pratt was a 1944 graduate of Baylor University.

A loving husband, father, grandfather and great grandfather he is survived by his wife, Erin, daughter Kath-leen and son L. Truett Jr. along with their children and grandchildren, and a large loving family. Service will be Wednesday January 2, 2013 11:30am at Ft. Sam Houston Cemetery.

Dr. Pratt was the 55th President of the San Antonio District Dental Society from 1968 - 1969. Many of our members have told us how much he meant to them and how he always went out of his way to help and men-tor many young dentists. According to Dr. Leighton Wier, “Truett gave me my first job in organized dentistry. In the early ‘70s, he was Editor of the SADDS and I became his Assistant Editor. In Memoriam

George Richard Bauer September 3, 1948 - December 30, 2012

George R. Bauer, II D.D.S, 64, passed away December 30, 2012 at his home with his family at his side in Del Rio, Texas. He was born September 3, 1948 in Beaumont, Texas. He grew up on the LeRoy Bauer Ranch in Winnie, Texas. His achievements were many and included educations at Schreiner Institute in Kerrville, Texas, Commander of Seaton’s Rangers, and Battalion Commander of Military School at Schreiner Institute with the rank of Lieutenant Colonel. He met his future wife Kathi on a blind date in December of 1969. She was the Battalion sponsor for George’s final year at Schreiner. George attended Baylor University at Waco, Texas and graduated with a Bachelor of Science in Biology and Math, and also a degree as Doctor of Dental Surgery at Emory University School of Dentistry in Atlanta, Georgia. He was a member of the First Presbyterian Church of Del Rio, the Lion’s Club of Del Rio, member of the Masonic Lodge #646 AM & FM. George was licensed from the Broad of Examiners for State of Georgia and Texas, member of the American Dental Association, American Orthodontic Society and Texas Dental Association. He married Katherine Loraine Schultz on June 10, 1972 in Dallas, Texas. They then moved to Del Rio in 1976 where he continued his dental business. He is survived by his wife: Kathi Bauer; daughters: Courtney and Katie Bauer; son: Matthew Bauer; granddaugh-ters: Georgia and Vivien Rzanca; mother: Patricia Bauer; sister-in-law: Candace Rollins; brother-in-law Derk Schultz; brothers: Hobart Bauer and wife Faye and LeRoy Bauer Jr., and wife Karen. He was preceded in death by his father LeRoy H. Bauer Sr., grandparents: Hobart and Juanita Brown, and grandparents: George and Ruby Bauer. George Richard Bauer will be missed. George was respected for many things. He was a role model of a husband, father, friend, and member of the community. He showed us daily what it means to be dedicated to the traditions of service to others and hard work. Honesty and a sense of integrity were traits that guided everything he did. George quietly showed his love and caring for all through his actions. George showed us the best qualities of a good man and we will miss him.
