■ 講演Ⅰマイクロ・ナノ工学と医療患者 QOL 向上を目指して

1.はじめに医療技術の進歩により平均寿命は延びているが、必
ずしもそれがクオリティオブライフ(QOL)を伴っているわけではない。我々の研究グループでは、マイクロ・ナノ工学を駆使し、先端医療に貢献する医療機器ならびに未病の状態からのヘルスケアに関する研究を行っている。本稿では、透析患者の日々の QOL を劇的に改善するインプラント透析システム、ならびに減塩と食事の美味しさを両立する「ソルトチップ Ⓡ」を紹介する。

今回の例会のプログラムのキーワードとして 1) 医工・産学連携を基盤とした先進医療および 2) 西洋医学と東洋医学の融合として重要な位置を占める漢方であります。
現 在、 本 邦 に 於 い て は 日 本 医 療 研 究 開 発 機 構(AMED) が推進しているプロジェクトの一つに「未来医療を実現する医療機器・システム研究開発」があり、このような時代背景の中で、最近、自分自身で講演
を聞き、皆様方と情報を共有したいと思った 2 課題、マイクロナノ・工学と医療及び ICT、IoT や AI が活躍する医療を選びました。また西洋医学と東洋医学の融合で西洋医学だけでは患者さんが満足する医療が提供出来ないと以前から考えており、両者の融合が必要と思い、漢方に対する臨床・基礎研究を続けて参りました。その結果、十分な成果が漢方を用いた臨床に於いて得られ国際学会での発表や論文化が可能となりました。
さらに 2009 年アデレードの国際外科学会では元会長の Sir Peter Moris から今や漢方医学は補完代替医療ではない。と称賛いただきました。そこで今回は日本漢方生薬製剤協会のオピニオンリーダー加藤照和氏に漢方の現況と近未来像について講演をいただきました。多くの出席者から充実した講演内容とお褒めの言葉をいただきました。

2.インプラント透析システム現在、国内で血液透析療法を行っている患者は 32
万人を超えている。技術の進展や合併症に対する予防法の浸透により、現在の血液透析療法はほぼ完成の域に達しているが、患者は週 3 回、1 回 4 時間の治療が必須であり、生活への大きな負担となっている上に、通常の注射針より格段に太い針による毎回の穿刺、厳しい食事制限など、QOL が高いとは決して言えない。また、医療費も年間 1 兆 7000 億円を超えている。
これらの問題に対する工学的な解の一つがインプラント透析システムである。継続的に透析を行うことができるため、患者の通院頻度ならびに穿刺の回数を低減し、かつ身体的な負担も低減できる。さらに食事制限の緩和も可能であり、患者の QOL を劇的に改善する。ウェアラブル人工透析装置も提案されているが、感染症や、装置破損による事故のリスクが無視できないため、我々のグループはインプラント型のシステムを研究開発している。
インプラント透析システムと従来の透析システムの最も大きな違いは、その使用期間である。インプラント透析システムでは長期間、例えば 3 年、短くても1 年の使用が求められる。治療効果維持の実証に向けて、ラットならびにイヌで装置性能評価、血管との装置接続、術式について検討を行っている。特にラットでの短期実験を効果的に、複数回行うために、我々の研究室の学生は機械工学科所属でありながら、ラットの手術ができる。このように、工のバックグラウンドを持ちながら、医の知識はもちろん、技術を有する人材を輩出することが、医工連携研究成功の鍵を握ると考えている。現在までに術式はほぼ完成し、2018 年度中の装置詳細設計の決定、さらにその先の長期動物実験を目標に研究を進めている。
3.「ソルトチップ」を用いた減塩技術インプラント透析システムの共同研究を進めるう
ちに、食事制限による透析患者の QOL 低下を知る事となった。厳しい塩分摂取制限は、美味しい食事を難しくし、食欲不振による体力減少や、食事制限が守れないことによる症状の悪化につながる。減塩は透析患者だけでなく、日本国内で 4,000 万人ともいわれる高血圧患者にとっての有効な治療法である。
日本人は平均 10 g 程度の食塩を 1 日に摂取している。病院等で出される制限食は 1 日 6 g にコントロールされており、病院食は美味しくない、と言われる原因となっている。
食事療法効果を最大限に生かすためには、「美味しい」減塩、すなわち少ない食塩量で、十分な塩味を感じさせることが必要である。これを工学的に解決するため、我々は舌表面に直接食塩を、必要最低限の量のみ提供することができるソルトチップを開発した。ソルトチップは歯の裏に貼り付けて使用する。しっかりと貼りつくため、貼り付けた状態で食事することも可能である。唾液を経由し、もしくは舌でソルトチップを舐めることで、舌上に直接食塩を提供し、十分な塩味を提供できる。徐々に溶解し、およそ 5 分でなくなる。ソルトチップに含まれる食塩量は、0.1 g 以下だが、5 分間の間十分な塩味を提供する。例えば、この 5 分間に 2 個おにぎりを食べると、2 g の食塩摂取となるが、ソルトチップを用い、無塩のおにぎりを2 個食べると、食塩摂取量は 0.1 g 以下に抑えられ、減塩効果は 1/20 である。美味しさと、これだけの減塩効果を両立する技術はこれまでに皆無である。このソルトチップを用いた食事療法改善効果について、大崎病院東京ハートセンターと臨床試験を行い、特に手術後の喫食量の増加、それによる体力回復効果に有意な差が得られた。ソルトチップの普及のために株式会社 LTaste を創業した。
4.おわりに
紹介した両技術とも、患者 QOL の向上に貢献することは確信している。しかし、患者までその技術が届くかどうか、というのは別問題である。患者の QOL、治療効果はもちろんであるが、医師や業者の経済的なメリットについても考慮しなくてはならない。ソルトチップについても、現在厳しい食事制限を課されている人への普及は早いだろうが、一方で緊急性のない、例えば高血圧予備軍の方々は、減塩の必要性は感じているものの、ソルトチップを使ってまでの減塩をするには障壁が高い。新しい技術が普及して QOL 向上に貢献するためのエコシステムを構築することも重要である。

〇未来医療とは第4次産業革命(IoT、ビッグデータ、人工知能
(AI)、ロボット、シェアリングエコノミー等)のイノベーションをあらゆる産業や社会生活に取り入れ、様々な社会課題を解決する「Society 5.0」の実現を目指している日本。Society5.0 という言葉ですら、知らない方が多いと思います。Society1.0 が「狩猟社会」、Society2.0 が「農耕社会」、Society3.0 が「工業社会」Society4.0 が「情報社会」に続く、人類史上5番目の新しい社会、それが「Society(ソサエティ)5.0 です 1)。イノベーションを「健康寿命の延伸」や
「社会コストの抑制」に活用するためには、医療に様々な要素を組み込んでいかなくてはなりません。
未 来 医 療 と は、 様 々 な 情 報 や 技 術 を 取 り 入 れSociety5.0 の社会を実現しようとする日本において必要不可欠なイノベーション医療であると私たちは定義しています。今は実現していない技術といっても50 年、100 年先の医療ではなく、3 年、5 年後の近い未来に実現化していくものも未来医療です。また、医療はこれまで病気のみを治癒することに主眼をおいていましたが、予防という概念が固定化され健康管理も含めて診ていく世界が近い将来に現実のものになると信じています。
〇未来医療のデバイス生活習慣病のうち、肥満、高血圧、糖尿病、脂質異
常症は動脈硬化と関連しており、これらは脳梗塞や心筋梗塞を引き起こす原因の一つと考えられています。
高血圧は血圧を測ることで診断しますが、昔から血圧は安静時に計測するものとされてきました。しかし、実生活の観点でみると、人は運動をすることもあれば、仕事、食事もしますし睡眠もとります。活動状態によって血圧はそれぞれ違うはずですが、これまで、様々な行動の中での血圧測定は出来ていませんでした。
私たちは、ウエアラブルリストバンド型血圧・脈波計を開発しています。この血圧計は、カフレスで測定できる MEMS(メムス、Micro Electro Mechanical Systems)の 3 軸センサを用いた血圧計です(図1)2,3)。これにより、運動しているときや仕事をしているときなどの血圧を測定することが可能になりました。現在、その精度を向上させて最終段階に入っています。私たちは、救急現場にこのようなデバイスを提供することで、自分の状況を救急隊 4)や医師等に迅速にかつ正確に伝える IoT(Internet of Things)として活用されることを期待しています。また、疾患リスクの高い高齢者等が常につけることにより血圧や脈波の微妙な変動で、脳卒中や虚血性心疾患、不整脈等の予兆を早期に検知し、救急搬送されるような事態を予防できるのではないか考えています。スマートフォンという持ち運び可能でリアルタイムにインターネットに繋がるデバイスが急激に広まったおかげで、IoT という考え方が生まれて、リアルタイムに個人に関する様々なデータの入手が可能となりました。また、データが集まることで、AI (Artificial Intelligence、人工知能)による解析技術の発展により、既存のデータを学習することによって疾患の自動診断や危険な状態にある患者に警告などをできる製品の開発が可能になっています。このような技術の集合により、高齢者の血圧や脈波情報とその人の過去の病歴や健康情報とリンクすることにより、病気になる前のより早い段階での診断及び治療が可能になる未来医療が徐々に具体的になってきています(図2)。
〇病気から予防への未来医療未来医療は、このようなウエアラブルデバイスだけ
にとどまりません。最近では、血糖を継続的に計るデバイスや目の網膜から動脈硬化の判断、さらには、声や表情から、認知症やうつ病の診断まで出来るのでは

ないかと様々な開発が行われています 5)。医師もこのような技術から目をそらさずに理解して行かなければならなくなっていく時代です。病院は、病人だけを診る時代から、病気を予防すること、つまりは、健康管
理をする場所としても今後活躍していく時代になります。ICT や IoT や AI が活躍する医療が私たちの平均寿命を 100 歳へと導く時代も遠くないでしょう。医療は、新しいステージで、病気になるまえにアプロー

チするという選択肢を手に入れながら今後進んでいくはずです。また、そのためには医療現場が変化を恐れないこと、新技術の開発者及び支援者が世界とのイノベーション競争を見据えて教育と経済的な投資をしっかりと戦略的に行っていくことが重要です。

今回は、加藤照和先生にご講演を依頼しました。加藤先生は中央大学商学部卒業後、昭和 61 年に株
式会社津村順天堂(昭和 63 年に株式会社ツムラに社名変更)に入社。TSUMURA USA,INC. の取締役社長などを歴任されたのち、平成 24 年に代表取締役社長に就任されました。日本漢方生薬製剤協会の会長など数々の団体においてもご活躍されています。
今日は漢方の現況と近未来像についてお話しいただきます。
自然と健康を科学する―サムシング・グレート―

ツムラの創業は、1893 年(明治 26 年)である。創業者の(初代)津村重舎が、母方の実家である藤村家家伝の婦人薬に改良を加え、「良薬は必ず売れる」という理念のもと、東京日本橋で「中将湯」を発売した。
創業当時の社名は「津村順天堂」であり、中国古典

の「孟子」に記載のある「天に順う」の一文を引用したようである。以降ツムラは、漢方・生薬をベースとした生業を脈々と継承している。
変遷におけるひとつの転機は、昭和 51 年のツムラ医療用漢方製剤の薬価収載である。現在、ツムラの売上高に占める医療用漢方製剤の割合は 90% 以上にも及ぶ。また、医療用漢方製剤の市場は、約 1500 億円(2016 年度)で、ツムラの占めるシェアは 80%以上である。
漢方は、自然治癒力を高め、体を整えることが基本的な考え方である。また、漢方医学は、「漢」の時代の古典を原点とする中国起源の伝統医学であり、それが江戸時代中期に伝来することによって、日本で独自の発達を遂げていったものである。
その後、1950 年には、大塚敬節先生、矢数道明先生、細野史郎先生らにより、日本東洋医学会が設立、2001 年には、医学・薬学教育のコア・カリキュラムに漢方医学が採録され、現在では、医学教育、薬学教育、歯学教育、看護学教育の医療系 4 領域全ての「教育モデル・コア・カリキュラム」に漢方関連の文言が記載されている。
漢方生薬関連の協会が実施した「漢方製剤の処方実態調査」(2011 年)によれば、89.0%の医師が「何らかの形で漢方製剤を処方した経験がある」旨を報告している。また、漢方製剤の処方理由としては、「西洋薬では効果の得にくい症例で漢方が有効である」が最も多い回答であった(56.6%)。同様に、外部機関が実施した「漢方薬使用実態調査」(2012 年)によれば、「漢方を処方するようになった動機」の中で、実に 62.9%の医師が「西洋薬のみの治療では限界がある」旨を回答している。
行政は、治療満足度や薬剤貢献度において、まだまだ満たされていない領域が少なくないことや、将来の環境変化などを見据え、「健康日本 21」、「新オレンジプラン」、「地域包括ケアシステム」、「がん対策加速化プラン」、「女性活躍推進法の成立」などを制定している。ツムラも、それら環境の変化に順応すべく、高齢者関連領域、がんの支持療法領域、女性関連領域などに資源を集中化、重点化している。
高齢者領域のひとつである認知症疾患領域と漢方製剤の接点において、抑肝散という漢方製剤が注目されている。抑肝散は「子供の “かんのむし”」に使用さ
れ「母子同服」の記載もある興味深い漢方である。その症状が認知症の周辺症状と類似していることから、様々な研究報告が集積され、メタ解析によっても、その有効性が報告されたところである。同時に、グルタミン酸神経系やセロトニン神経系への西洋薬とは異なった作用メカニズムも報告され、高齢化社会における認知症治療の一助を担うことが期待される。
また、がん領域と漢方製剤との接点も注目されている。「がん対策加速化プラン」の支持療法の開発・普及の実施すべき具体案として、「術後の合併症・後遺症を軽減する観点から、栄養療法、リハビリテーション療法や漢方薬を用いた支持療法に関する研究を進める」と記されている。現在ツムラでは、がん医療の拠点施設等と連携し、抗がん剤の副作用軽減に対するエビデンスを積み重ねている。
そのような研究の積み重ねによって、近年、漢方製剤の海外論文などエビデンスが集積され、各種診療ガイドラインにも掲載されるようになってきた。日本東洋医学会 EBM 委員会は、漢方製剤の記載がある診療ガイドライン数は、年々増加していると報告している。
一方で、医療用漢方製剤の世の中への浸透とともに、マルチステークホルダーからの更なる要望や期待、そして、課題なども浮き彫りとなってきた。2016 年からスタートした 『国民の健康と医療を担う漢方の将来ビジョン研究会』では、産官学連携のもと、高久史麿先生や北島政樹先生に加え、横倉義武日本医師会会長等が中心メンバーとなって、漢方製剤の課題抽出や解決の道筋等について様々な議論が重ねられ、先般、提言書も纏められたところである。
最近では、新技術と漢方製剤との新結合として、医療ビックデータを用いた大建中湯による術後癒着性イレウス患者の入院医療費削減の経済効果や、システムズ・バイオロジーの技術を利活用した“こむら返り”に使用される芍薬甘草湯のレスポンダー(効果を示しやすい病態)解明、さらには、メタボロームデータの解析技術を用いることによって、インフルエンザや感冒などに用いられる麻黄湯の多標的な作用機序等が解明されつつある。
今後、科学技術の発展と共に、未科学である漢方のベールは徐々に解明されてくるであろう。我々は、順天の精神で、常に漢方という一つのサムシング・グレートと謙虚に向き合い、学び続けていく必要がある。

was held at the Gakushi Kaikan in Tokyo on May 17.
2018 Dr. Masaki Kitajima, Director of the International
Medical Society of Japan (IMSJ), presided over the
meeting.

For the program keywords in the regular meeting
this time, there are 1) Advanced medical care on the
basis of Medical-Engineering/Academic-Industrial
Cooperation, and 2) Kampo (Chinese medicine) that
holds a prominent position as the integration of
Western medicine and Eastern medicine.
Since one of the projects in Japan currently
promoted by Japan Agency for Medical Research
and Development (AMED) is “Development business
of medical devices/system study realizing the future
medical care”, in such historical background, I
selected 2 research subjects “Micro-Nano Science
& Technology and medical care” and “Medical
care with ICT/IoT/AI” for sharing with you all after
I recently had an opportunity to hear these lectures
myself. Furthermore, because it has been personally
considering for some time that patients may not
be satisfied only with Western medicine with the
consideration of the integration of Western and
Eastern medicine, I continuously participated in
the clinical/basic research for Kampo due to a

necessity for their integration. As a result, I achieved
satisfactory outcomes in the clinical research and
can now provide the presentation and article in the
international conference.
In addition, at the conference for the International
College of Surgeons (Adelaide) in 2009, I have
received a compliment from the former chairperson
Sir Peter Moris as “Kampo is not complementary and
alternative medicine anymore”. Thus, we invited the
opinion leader Mr. Terukazu Kato of Japan Kampo
Medicines Manufacturers Association for presenting
a lecture “The present situation and the near-future
vision for Kampo”. He received compliments from
a number of attendees for the fulfilling lecture
contents.

Medical Technologies Enabled by Micro/Nano Science and TechnologyTo Improve Quality-of-Life of Patients

INTRODUCTION
While the life expectancy is increasing thanks to
progress of medical technologies, the quality of life
(QOL) is not necessarily improved. Our group has
been developing implantable dialysis devices, or
artificial kidney, which will free the patients form their
frequent hospital visits and drastically improve their
QOL. In addition, the medical budget can be saved by
1/3 by replacing the current hemodialysis treatment.
We are also attempting to improve QOL from the
healthcare point of view. Salt intake reduction is
necessary for the dialysis patients and is known to
be an effective treatment for high blood pressure
patients. However, low salt diets do not exhibit good
taste, which leads to not only low QOL of patients
but also low appetite and thus, malnutrition. We
invented Salt Chip® to enhance the salt taste without
increasing the salt intake. In this presentation, we
introduce the state-of-art of our technologies and the
challenges we are facing now.
IMPLANTALBE DIALYSIS
Dialysis therapy is currently widely used for
patients with end-stage renal diseases. The therapy
is well-developed, however, the patients must visit
hospitals three times a week and stay there for 4 h
in each visit. At each treatment, patients experience
puncture of thick needles. Patients must adhere
to dietary restrictions. These issues lead to low
QOL of the patients. In addition, the medical fee
of the dialysis therapy is now exceeding 1.7 trillion
yen. As a promising solution, we propose hybrid
dialysis therapy with an implant auxiliary kidney,
which is a hemofiltration system. The implant
hemofiltration system removes water and other
small molecules continuously and the patients
receive conventional dialysis treatment once a week
to balance the electrolytes and remove wastes that
are not sufficiently removed by the hemofiltration.
The less frequent hospital visit and alleviated dietary
restriction, i.e., patients are allowed to drink water,
drastically improves their QOL. It is estimated that the
medical fee can be compressed to two thirds of the
conventional treatment.
Our group first developed a microfluidic device
which was experimentally verified to have sufficient
dialysis capacity, i.e., it could remove low-molecule-
weight molecules, such as urea, and maintain
proteins. Hemofiltration using the device was
successfully demonstrated with rats under anesthesia.
The challenge is long-term reliability of the system.
We target at 3 years of successful operation. In order
to prove the concept, we have conducted short-

and mid-term ex and in vivo experiments with rats
and dogs to determine the protocol of the long-
term experiments. The students in my laboratory are
working on ex vivo tests with rats to validate the
device design while their background is mechanical
engineering. The researchers with knowledge and
skills of engineering and medicine are mandatory for
successful medical-engineering research.
SALT CHIP
Salt Chip® is a square thin plate, 5 mm ×5 mm×1
mm in size and consists of adhesive carboxymethyl
cellulose (CMC) and polymer material containing
0.08 g of salt. It can be attached to the back of the
lower front tooth and dissolves in approximately 5
min while discharging salt. The users can eat and
drink with Salt Chip® being attached to the tooth.
While in ordinary food intake, a certain amount of
salt inside the food does not reach the tongue and
is not tasted but taken, all the salt inside Salt Chip®
is delivered to the tongue and efficiently evokes salt
taste. Even though it contains only 0.08 g of salt, it
provides strong salt taste until it completely dissolves.
According to the clinical tests at Osaki Hospital
Tokyo Heart Center, enhanced salt taste with a help
of Salt Chip® for the low salt meals increased the
eating amount during the early postoperative period,
maintained Alb, and alleviated edema. This prevented
secondary sarcopenia, which led to early recovery of
activities of daily life and short period of hospital stay.
SUMMARY
The both technologies discussed herein are ready to
improve QOL. We founded LTaste inc. to develop and
sell Salt Chip®. Not only the technologies but also the
eco system to deliver them to the patients need to be
developed in parallel.

Abstract
By incorporating innovations from the 4th industrial
revolution (IoT, big data, artificial intelligence, robots,
sharing economy, etc.) into all industries and aspects
of social life, Japan is aiming to realize Society 5.0,
which resolves a variety of social challenges. Society
5.0, which follows the hunting society of Society
1.0, the agricultural society of Society 2.0, the
industrial society of Society 3.0, and the information
society of Society 4.0, is the 5th and newest type
of society in the history of humankind. In order to
utilize innovations to extend healthy life expectancy
and control social costs, various aspects must be
incorporated into healthcare in the world today.
Future healthcare is a type of innovative healthcare
that is essential for Japan, which is aiming to realize
Society 5.0 as described above. We develop wearable
blood pressure and pulse wave monitor wristbands.
The blood pressure monitor utilizes 3-axis sensors
equipped with micro-electrical-mechanical systems
technology, allowing for blood pressure to be
measured even while driving, as measurements are
taken in a cuff-less manner. Additionally, devices to
continuously measure blood sugar levels, devices to
determine arteriosclerosis from the retina of the eyes,
and even devices to diagnose dementia or depression
from voices and facial expressions are being developed
around the world. Hospitals are transitioning from an
age where only the ill were treated into an age where
illness is prevented and health is managed. A time
when healthcare supported by ICT and IoT leads the

average lifetime to over 100 years may not be far off.
I believe that healthcare will advance into a new stage
in which there will be a new choice to avoid illnesses.

We invited Mr. Terukazu Kato for a lecture.
After graduation from Faculty of Commerce, Chuo
University, Mr. Kato joined Tsumura Juntendo
Company in 1986 (In 1988, the company changed
its name to TSUMURA & Co.) . After holding
prominent positions such as the president of
TSUMURA USA,INC., he became the President
(CEO) of TSUMURA & Co. in 2012. Currently, he has
also been actively serving for various organizations
including the chairperson of Japan Kampo Medicines
Manufacturers Association. Today, Mr. Kato will be
lecturing for “The present situation and the near-
future vision for Kampo”.

Tsumura & Co. was founded in 1893. The founder,
Jyusha Tsumura, first marketed Chujoto, an herbal
remedy passed down from his mother’s family that
focuses on improving women’s health. Tsumura
marketed Chujoto with the strong belief that “good
medicine will definitely sell.”
Our company started out as Tsumura ‘Juntendo,’
which was presumably derived from the saying
“Follow God (Junten)” in the Chinese classic the
Mencius. Since starting out, our business has focused
on manufacturing and marketing Japanese herbal
medicine, called Kampo, and products derived from
crude drugs.
One of our landmark events took place in 1976
when our Kampo formulas were first listed in the
National Health Insurance Drug Price List. Currently,
prescription Kampo medicine accounts for over 90%
of our sales. In Japan, Tsumura accounted for about
84% of the approximately 150 billion yen in total
sales for prescription Kampo medicine in 2016.
Kampo is a traditional medicine that originated
in China. It is based on the Chinese classics
published during the Han Dynasty. It developed
uniquely in Japan around the mid-17th Century.
The fundamental concepts of Kampo medicine
are enhancing an individual’s healing power and
coordinating one’s body.
In 1950, the Japan Society of Oriental Medicine
(JSOM) was established by Kampo medicine giants,
including Keisetsu Otsuka and Domei Yakazu . In
2001, Kampo medicine education was incorporated
into the core curriculum for undergraduate medical
and pharmaceutical education. Now, medical,
pharmaceutical, dental and nursing education
includes Kampo medicine in their core curricula.
According to a survey on the use of prescription
Kampo medicine conducted by the Kampo medicine
association in 2011, 89% of physicians said they
have prescribed Kampo medicine in their practices.
Also, 56.6% said they prescribed Kampo medicine
because of its efficacy where Western medicine was
ineffective. Similarly, a survey on the use of Kampo
medicine conducted by an external institution in 2012
showed that 69.9% of Japanese physicians started
using Kampo medicine because they felt limited by
Western medicine alone.

several policies including “the New Orange Plan,”
“Community-based Integrated Care System,”
“the Acceleration Plan for Cancer Control” and
“an Act on Promotion of Women's Participation
and Advancement in the Workplace” because of
low satisfaction from treatment and lack of drug
contributions in some therapeutic fields, as well as
environmental changes in the future. Tsumura is also
addressing these issues by focusing its resources on
care for the elderly, women, and cancer support.
Regarding elderly care, yokukansan is gaining
attention for treating symptoms of dementia. One
of the indications treated by yokukansan is children’s
tantrums , which have similarities with the peripheral
symptoms of dementia. Many clinical studies are
being conducted and efficacies have been reported
thus far. Yokukansan has a different mechanism
of action compared to Western medicine, and is
expected to contribute to the treatment of dementia
in aging societies.
Cancer care with Kampo medicine is also gaining
attention. We are cooperating with cancer research
institutions to accumulate evidence on chemotherapy-
induced drug reactions. Such dedicated research has
contributed to the collection of scientific evidence
on Kampo medicine in international journals and
publications in various clinical practice guidelines.
The Committee for Evidence-Based Medicine (EBM)
for JSOM announced that the number of practice
guidelines for the use of Kampo medicine has grown
over the years.
While awareness of Kampo medicine is rising, some
issues have emerged. To resolve these issues, the
Vision for Kampo Medicine and the Public Welfare
Forum, which members includes Dr. Fumimaro
Takaku, Dr. Masaki Kitajima, and Dr. Yoshitake
Yokokura started in 2016 with the objective of
summarizing issues that need to be tackled through
collaboration between industry, government,
academia and the public .
There are other examples of research using the
latest technology with Kampo medicine. Big data
has shown that hospital expenses are reduced in
postoperative ileus patients treated with daikenchuto,
systems biology is used to identify responders in
shakuyakukanzoto treatments for muscle cramps,
and a metabolome analysis technique is used to
understand multiple mechanism of actions of maoto .
The mystic veil of Kampo medicine is gradually
being lifted with the development of science and
technology. We will continue to sincerely pursue and
understand Kampo, a form of something great, with
the spirit of ‘Junten.’
