You can apply for a maximum number of 4 season tickets per application form. Please give the details of the other season ticket holders. On the reverse of the application form (page 4). The price categories apply to the following criteria:- • Under 12s tickets are available to those supporters who are under the age of 12 on or before 05/08/2018.• Under 18s tickets are available to those supporters who are under the age of 18 on or before 05/08/2018.• Under 23s tickets are available to those supporters who are under the age of 23 on or before 005/08/2018.• Over 65s tickets are available to those supporters who are over the age of 65 on or before 05/08/2018.• Over 75s tickets are available to those supporters who are over the age of 75 on or before 05/08/2018.Due to limited capacities in some areas of the stadium, we advise you to provide us with two choices of where you would like to be seated. If the seating area you have selected is not available please note your application may be delayed until after the Season Ticket seat renewal deadline has passed.

CUP MATCHESCup match tickets can be pre-ordered but depending on your seating area, your usual seat may not be available if some areas of the stadium are not open. Please note: The pre-ordering of cup tickets is only available provided your credit/debit card details are entered in the payment section and your card is valid at the time the cup matches are played.

Seasonal Car Park PermitsShould you wish to purchase a seasonal car park permit, please add this cost to your total amount in the space provided. Your application will be processed and your pass will be forwarded prior to the start of the season. NB: Car Park Permits include all Football League, cup and friendly matches. Prior to June 10 they will cost £135 and then £145 after this date.

New Lifeline MembersShould you wish to join the Lifeline Scheme, please telephone 01204 673774. Please do not include your 5% discount within your season ti cket price as this will be refunded once 12 weeks’ Lifeline payments have been received.

You may purchase up to four tickets per application form - please complete Additional Clients section including date of birth in order for us to process the correct age range as these differ from club to club.
