Where we need to pass informationto other firms, it will only be for that purpose. Thesefirms will be Insurers, other insurance brokers, firmshandling claims, finance providers and firms thatprocess or administer our records, includingFederations/Trust Administrators.When we contact you, it will either be for the abovereason, or because we have a legitimate interest inmarketing related products. IMPORTANT INFORMATIONAPPlicAble To All benefiTs

Applying to joinServing officers can apply to join the scheme at any time bycompleting an application form which is available from theFederation Office. The first three monthly subscriptions arefree of charge when joining the scheme for the first time.New student officer recruits to the police service may join thescheme without the need for the completion of the medicalunderwriting application form and their first 52 weeks ofservice are free of charge. The Trustees and/or Philip Williams& Co reserve the right to decline any applications.

Subscription collectionSubscriptions are collected monthly by deduction direct fromsalary/pension unless that is not possible when alternativearrangements may be agreed. Monthly subscriptionpayments must be maintained in order to remain a memberof the scheme and to qualify for any benefits.

InsurersA list of the insurers is available on request. Full policywordings for certain policy sections are available todownload via QR codes. These may also be available at theFederation Office, on the Federation Website or by visitingthe Group Scheme section of www.philipwilliams.co.uk

Cohabiting Partner ExtensionsAny cohabiting partner extension will cease when theServing member or cohabiting partner reach 65 years of age(75 years for retired members), whichever is the sooner. Anycohabiting partner cover and/or extension will cease whenthe member ceases to be a member of the scheme.

Career breaks, maternity leave, secondmentor living overseasThose going on a career break, maternity leave, secondment orare living overseas must contact the Federation Office toidentify if cover can be maintained.

Retirement from the Police ServiceServing officers upon retirement may remain in the schemeas a retired member by submitting a completedmembership continuation form to the Federation Officeprior to retirement.

Individuals are not eligible to join the scheme after theirretirement date.

Transfer, resignation or dismissalMembers who transfer, resign or are dismissed from thepolice service are not eligible to remain in the scheme and allmembership and benefits will cease including any cohabitingpartner extension.

How to cancel your coverIn the event that you need to cancel your cover pleasecontact the Federation Office.

Complaints procedureThe Insurance Scheme is arranged on behalf of the trusteesby Philip Williams (G Ins)Management Ltd, trading as PhilipWilliams & Co Insurance Management who are authorisedand regulated by the Financial Conduct Authority(Registration Number 827663). The trustees areresponsible for organising the policies and dealing with theinsurance broker. Any complaints about any aspect of thescheme should in the first instance be directed to theFederation Office. The insurance broker will then be askedto investigate the complaint and resolve any matter eithervia the Federation Office, directly with the member, orthrough the appropriate underwriting organisation.

FINANCIAL SERVICES COMPENSATION SCHEMEIn the event that an insurer is unable to pay a claim you maybe entitled to compensation from the Financial ServicesCompensation Scheme if an insurer cannot meetits obligations. This depends on the type of insurance andthe circumstances of the claim. Information can beobtained on request, or by visiting the Financial ServicesCompensation Scheme website at www.fscs.org.uk

This booklet is a summary only. The benefits are subject to certain exclusions and policyconditions not stated in this booklet which in the event of a claim will be applicable. The benefitsare provided strictly under the terms of the insurance policies taken out by the trustees of thescheme. Subscription to the scheme entitles the member to the benefits provided by thescheme but confers no ownership of any of the underlying policies which are vested in thetrustees. The Insurance Scheme is an Independent Trust managed by the Trustees. TheTrustees pay the Federation for the Federation Staff/Office to administer the InsuranceScheme on their behalf.The trustees retain the full policy wording detailing all benefits andexclusions which may be viewed on request to the Federation Office.

EXPLANATION OF BENEFITS

Life insuranceOn death of a member or subscribing cohabitingpartner the cash benefit will be paid. The policy iswritten in Trust so that if a member dies, the proceedscan be paid, by the Trustees, to the member'sdependants quickly free of tax and without having towait for probate. Members should ensure that they havean up-to-date beneficiary nominated and have lodgedthe details with the Federation Office, to assist theTrustees in the event of a claim. If a member aged 63 orunder receives a terminal prognosis of 12 months orless, they may apply to the Trustees for an advance ofthe death benefit of 20% of the relevant sum insured.

Supplementary Life and Critical Illness InsuranceThe option to purchase additional (Top Up) life andcritical illness insurance cover is available to existingsubscribing members of the scheme only, we invite youto consider the options to increase your life cover forboth yourself and your cohabiting partner.

Child Death GrantPaid upon the death of a dependant child of a member,aged between six months and 17 years.

Permanent total disablementPayable when a member is unable to perform anygainful employment and is unable to exist indepen-dently, requiring continual supervision for activities ofdaily living, as a result of an accident.

Accidental Loss of Useand Permanent Disabling InjuriesVariable Benefits paid in the event of:- - Loss of sight in one or both eyes- Loss of use of one or more limbs- Total loss of hearing in one or both ears

Unrecovered Criminal Court CompensationCover is provided for serving officers only. If, as a resultof an on-duty assault, compensation you have beenawarded by the Court has not been paid within sixmonths from the payment date of the award thebenefit will be made.

Hospitalisation- Payable when admitted as an in-patient to hospital between midnight and 07.00- Unplanned admission arising from accident or emergency, payable from first night- Planned admission payable after three nights

Injury as a Result of Firearms or KnivesIf a serving police officer sustains accidental bodilyinjury in the course of duty directly caused by thedischarge of either firearms or shotguns or caused byassault involving a knife and as a consequence of theinjuries is unable to work for a period of at least threeconsecutive days immediately after the attack thepolicy will pay: (a) £1,500 as a result of Firearm or Shotgun injuries (b) £1,000 as a result of knife injuries

Dental Injury & EmergencyProvides cover for Dental Expenses in the event of:- Dental Injury- Emergency Dental Treatment- Dentist Call-out Fees- Hospitalisation- Mouth CancerDefinitions are shown in the full policy wording.

Please note - For Emergency Dental Treatment cover isprovided only for the initial dental treatment providedat the emergency appointment urgently required forthe relief of severe pain, arrest or haemorrhage, thecontrol of acute infection or condition which causes asevere threat to the general health. Any subsequenttreatment required after the initial emergencyappointment is specifically excluded.

Making a claimNo prior authorisation is required. Undergo thetreatment, pay the dentist direct, and subsequentlysubmit a claim. Please ensure that proof of treatmentand receipts of costs are obtained. Claim forms areavailable from the Federation Office.

Sick Pay BenefitIf a member suffers a reduction in pay under PoliceRegulations or terms of employment, the benefitsas shown in the policy will become payable. Benefitsunder this section for Police Staff are shown in thefull policy wording.

The benefit ceases on return to work or if the memberretires, resigns, is discharged from the police service, orfails to pay the monthly scheme subscription. Inaddition the benefit shall not be payable if the memberhas been offered reasonable adjusted duties with areturn to full pay and has declined such duties withoutreasonable cause.

Any overpayment due to a reversal in the decision bythe employer which results in a resumption of pay(including any back payment) or due to a failure by themember to inform the insurers of a return to work mustbe repaid in a prompt and timely manner.

Unsociable Hours BenefitIf a Serving Officer sustains Accidental Bodily Injuryor contracts sickness during the Period of Insuranceresulting in total disablement entirely preventing thatInsured Person from engaging in or giving attentionto his/her usual occupation We will pay a benefit equalto £1 per Unsociable Hour the Insured Person is unableto work that had been scheduled prior to thecommencement of disablement as recordedin Police Force records.

The maximum benefits we will pay in respect of thissection is £60 per week.

The policy will not pay for scheduled unsociable hoursfor the first 14 days of each period of disablement andis payable for up to a maximum of 24 weeks.

The benefit will be payable if a member, member’s subscribing cohabiting partner or their child, agedfrom 30 days to 17 years, suffers from an insured illness and survives for more than 14 days from thedate of diagnosis or surgery.

Insured PersonsCover applies to you and your spouseor permanent partner if you are bothunder 75 at the date the tripcommences. It also includes alldependant children who are under 18years or 23 if in full time education (atthe date the trip commences).You and all the people listed mustpermanently live together andpermanently reside in the UnitedKingdom. Your partner and/or childrencan travel separately and still becovered by this policy.

The policy covers travel worldwideand in the United Kingdom for anynumber of trips in any year up to60 days per trip up to a maximumof 180 days per year.

Pre-Existing Medical ConditionsThis policy will not cover you for any claims arising from Pre-existingMedical Conditions as set out below:-(a) Any claims made under the Cancellation, Curtailment or Changeof Itinerary and/or Medical & Emergency Travel Expenses sectionof this policy as a result of any Pre-existing Medical Condition wherea Qualified Medical Practitioner has not permitted the Insured Personto travel.(b) Any claims under the Cancellation, Curtailment or Changeof Itinerary section of this policy as a result of any Pre-existingMedical Condition where the Insured Person is on a waiting list forin- patient treatment.(c) Any claims made under the Cancellation, Curtailment or Change ofItinerary and/or Medical & Emergency Travel Expenses section of thispolicy where you are travelling for the purpose of obtaining medicaltreatment abroad.(d) Any claims made under the Cancellation, Curtailment or Changeof Itinerary and/or Medical & Emergency Travel Expenses sectionof this policy where you have been given a terminal prognosis.(e) Purchase of any prescription medicines relating to aPre-existing Condition.(f) If an Insured Person or Close Relative has suffered a Pre-existingMedical Condition that You could have reasonably foreseen would havegiven rise to a Cancellation or Curtailment or Change of Itinerary claimunder the Cancellation, Curtailment or Change of Itinerary sectionof this policy.

Covered Vehicle The car, motorcycle, campervan, motorhome, domesticvans, or car-derived vans all up to 3.5 tonnes, which acovered individual is travelling in/on at the time of theBreakdown including any towed caravan or trailer of aproprietary make which is fitted with a standard towinghitch and doesn’t exceed 7 metres/23 feet (notincluding the length of the A frame and hitch).

Claims will be validated with costs for any claimsfrom non-eligible persons being the responsibilityof the claimant.

Cover will apply during the period of insurance andwithin the territorial limits. There is no age limit to thevehicle for breakdowns occurring in the UK. A vehicleage limit of ten years applies outside the UK.

Your mobile phone is covered against the repair orreplacement cost of your mobile phone in the event of:- theft- accidental loss - accidental damage- breakdown whilst in your possession

The liability of the insurer in respect of any one claimand in aggregate for any 12 month period will be therepair or replacement cost of the mobile phone and inany event shall not exceed a total claim cost up to amaximum of £1000 including VAT.

A £75 excess is payable per claim.

The mobile phone should have a fully functioning SIMcard and be no more than eight years old at the time ofthe incident as evidenced by the relevant proof ofownership. Upon acceptance of a claim, the insurer mayat its discretion repair or replace your mobile phone.Replacement may be with a refurbished unit or afunctionally equivalent product. This is NOT a new forold policy.

If the claims administrator replaces your mobile phone,your original item becomes the insurer’s property andthe replacement mobile phone is your property, withcoverage for that item continuing for the remainingperiod of insurance.

The maximum liability limit includes cover of up to £750including VAT for unauthorised data usage for a periodof up to 24 hour directly following a valid theft oraccidental loss claim.

Please note any claim involving theft or accidental lossmust be reported to the appropriate police authoritieswithin 48 hours.

In addition to proof of ownership, verification ofmembership will be required before a claim can beprogressed.
