Severity Classification Category
Definition
1 Critical
A failure mode which causes an immediate loss of component function (100% probability of component failure). Component function is defined as the credited contributions to power generation, preservation of pressure boundary, mitigation of accidents or transients, environmental compliance, regulatory compliance, continuation of mission or preservation of personnel health or safety as defined in plant documents such as the Maintenance Rule Scoping Document. Degradation related to this category causes an equipment functional failure.
2 Severe
A failure mode which will result in a loss of function in a very short time if not immediately addressed (i.e. repaired, fixed, or replaced) or a delay in the return to power or could result in the severe degradation of the performance of the component such that system function is in imminent jeopardy.
3 Minor
A failure mode which is not expected to cause injury or property damage or system damage but will require unscheduled maintenance or repair to resolve. This doesn’t cause the component to fail (< 20% probability of component failure) but is an unacceptable nuisance (e.g. a housekeeping issue such as a water or oil leak).
4Inconsequential A failure mode with is not expected to cause system damage or require
maintenance to resolve.
