Background The live exercise is currently the most known type of simulation and it was evidenced to be very effective in improving technical and non technical skills. Other types of simulation are establishing to be equally effective compared to the first one. Both table-top and computerized simulation are useful tools of training and teaching in Disaster Medicine. Basing on these evidence we explored if a 1-day exercise session about Mass Casualty Incident (MCI) Triage, based on innovative simulation tools, could be effective to improve basic knowledge and if it could be appreciated by nurses. Methods 27 nurses enrolled in an Italian first-level Master on principles of Disaster Preparedness and Management were exposed to three different types of simulation (table-top exercise, virtual reality simulation and live scenario). After a 20-min lecture about MCI Triage, every nurse was exposed subsequently to the three simulations aiming at triaging 10 victims in a limited period of time (30s/victim). The simulation environment and the victims’ clinical conditions were identical in the three scenarios. All participants were assessed on their education background and knowledge in MCI Triage with a standard multiple-choice test before and after the whole session with addiction of an exercise where the correct using of S.T.A.R.T. triage algorithm was tested. Also participants’ satisfaction trough a 6-items questionnaire rated on a 10-point Likert scale were inquired. Results The average of right answers was 61% in the pretest and 80% in the post-test. The triage accuracy was 84% and 94% in the pre-test and post-test respectively. 21 nurses reached in the post-test 100% of triage accuracy. The overall mean score of the nurse satisfaction with the entire learning session was 9/10. Conclusion The whole session based on new teaching methodologies were able to increase the basic knowledge of nurses in MCI Triage topics. Also the study demonstrates that the overall training session generates improvement in triage accuracy. The nurses really appreciated the use of simulation tools and exercises. The data represents only an initial report of a future nationwide study that will include all the 180 nurses enrolled in to the Master. SIMMED - Florence, May 2012TRANSCRIPT
