You have applied for a ballot to vote absentee. The ballot is enclosed in sealed envelope marked BALLOT WITHIN; DO NOT OPEN EXCEPT IN THE PRESENCE OF A WITNESS.

1. Do not open the envelope marked BALLOT WITHIN until in the presence of the witness. If voting in the office of a Virginia election official, the witness must be a Registrar or Secretary of the Electoral Board. After removing the ballot, roark it in the presence of the witness but without assistance and without making known to the witness how you vote. If you are physically or educationally unable to vote without assistance, see Item 7 on the reverse side of this form.

2. In the square immediately to the left of the name of each candidate for whom you wish to vote, place a cross (X or +), or a check (/), or a line(-), leaving the square to the left of those for whom you do not wish to vote unmarked.

3. Fold the ballot with the names of the candidates on the inside, place it in the MARKED BALLOT envelope and seal the envelope. Do not place anything in this envelope other than the voted ballot. On the back of this envelope complete, sign aild date the oath, filling in all required information. The witness must also sign. If the voter is unable to sign his name, an "X" or "mark" made by the voter will fulfill any signature required of the voter.

4. Place the sealed MARKED BALLOT envelope in the return envelope addressed to the Secretary of the Electoral Board. This envelope must be mailed or delivered by you in person to the Registrar or Secretary and must be received by them before 7:00p.m. on the day of the election •

UNUSED 5. If for any reason you decide not to vote this ballot, do not open the BALLOT WITHIN envelope. You are required by law to return it unopened on or before the day of the election to the Secretary of the Electoral Board from whom you received it.

SPOILED 6. If you spoil this ballot by making an error when marking it or by acciden­tally damaging it, if there is time prior to the election, you may receive a new ballot by returning the spoiled or damaged ballot to the Secretary of the Electoral Board with a letter signed by you stating that the returned ballot is spa iled and requesting a new ballot.

VOTER 7. If you indicated on your application that you will need assistance in voting your ballot, a VOTER ASSISTANCE OATHS form is enclosed. No assis­tance can be given to an absentee voter unless this form is completed and returned.

The properly completed form must be returned in the envelope addressed to the Secretary of the Electoral Board.

In addition, in the Oath of Voter on the back of the MARKED BALLOT envelope, the words "without assistance or knowledge on the part of anyone as to the manner in which the same was prepared" must be marked out before the voter signs that oath.

8. The person assisting a blind voter must print, on the line provided in the OATH OF VOTER, the blind voter's full name followed by the words ''blind voter"; and, in the OATH OF ASSISTANT their own full name and address. Then proceed as indicated in the last two paragraphs in Item 7. above.
