Safety First Briefing|October 2018
Welcome to our Safety First Brief for October.
We have closed the fatigue survey and included some feedback this month. Please take a few moments to readthrough the results. We received 167 responses, which gives a reasonable sample size, from over 900 invitations to participate which were sent out.
The feedback received from you will help develop and shape the fatigue management strategy of the business going forward, so thanks again to those who participated. We will be looking to engage directly further over the next few months, on this and other topics, so please try to participate where possible – it’s to your direct benefit to do so.
We had only 1 minor accident, and 2 vehicle accidents in September, please help us continue to improve and reduce these by taking care of yourself and others, keeping to the speed limit and only undertaking tasks with the correct tools, equipment, and PPE.
We are sending links to an online programme of Driver Assessment and Training over the next while – please keep an eye out for this and be sure to complete within the allocated time.
Remember to raise any close calls and highlight any issues you see while onsite, and please take care on your journey to and from work as the weather starts to change and conditions become more challenging.

Accidents and IncidentsDuring September there was 1 minor accident and 3 incidents reported:
1. Accident Details
On 18th September, an ISS OLE Linesman was working on the wiring train for Alstom, when while setting upthe rig the chain from the pull lift got caught in the basket. The IP was holding the chain, which releasedvery quickly and their hand got caught between the chain and the basket.
He removed his cut 5 gloves and had sustained some bruised blood and a cut to the soft tissue on the lowerpart of his left index finger. The COSS was on hand to give First Aid and managed to clean the wound andput a bandage on the finger. IP attended A&E for a check up, didn’t need any stitches and had the woundcleaned and a fresh dressing put on his finger.
He was given a fresh dressing and advised to change the dressing in a couple of days.
Investigation
The rig moved, and as the chain was caught, it meant it pulled through the IP’s hand as he was holding theslack chain from the pull lift at the time. This prevents it trailing, and is meant to reduce the hazard andpotential for incident or accident. However, in this case, the action of the chain pulling meant the IP wasunable to release in time. The IP had his gloves on, and this prevented further or worse potential injury.
Recommendations
ISS Supervisor has suggested the staff reduce the height of the basket prior to moving the machine, andattaching the pull lift to an anchor point so that the weight of the equipment is supported at all times. Thismeans should someone be holding the loose end of the chain in the future, and there be any unplannedmovement, then the equipment will not drop or pull through the hands of the person holding the chain.

Accidents and Incidents
1. Incident Details
On 09/09/18, an ISS COSS was involved in a Possession Irregularity, where he removed worksite marker
boards at Lostock for S&C North Alliance. When he arrived, he also removed Possession limit boards and
detonators in error.
Investigation
It appears the COSS was unsure what to remove, so rather than check he removed all the protection that
was in place on the back-to-back possessions. The COSS had never removed WSMBs prior to this, and was
undure of the protocol.
Recommendations
The COSS has been stood down from Safety Critical duties, and will be mentored and re-assessed prior to
undertaking these type of works in the future. In addition, the COSS had a 1-to-1 session with ISS Training
Manager, concentrating on the various responsibilities and duties of the COSS, with emphasis on Safety
Critical Communications.
2. Incident Details
On 16/9/18, an ISS COSS, working for Buckinghams on an Amey worksite, as a Task Leader. At the end of the
shift he left site to go and sign out and told the ES that all works had been stood down. The ES then rang the
machine controllers on site and asked if they were still working. They then replied that they were.
This resulted in a Close Call and investigation against the COSS for leaving works unattended.
Investigation
The main issue was around the communication between the ISS COSS, and the others still onsite, as those
he was working riectly with had also left site, but others falling under his duty of care as Task Leader had
not.
Recommendations
As it was the same COSS as above, the recommendations apply for both incidents. The COSS has been stood
down from Safety Critical duties, and will be mentored and re-assessed prior to undertaking these type of
works in the future. In addition, the COSS had a 1-to-1 session with ISS Training Manager, concentrating on
the various responsibilities and duties of the COSS, with emphasis on Safety Critical Communications.

3. Incident Details
On 09/09/18, an ISS Authorised Person was involved in an Isolation Irregularity, where he incorrectlyinstalled DEP short earths on the wrong lines (Fasts instead of slows). This was only discovered when theIsolation was being handed back, as previously there had been an all lines Isolation on the same area. Thedelay in handing the Isolation back caused the first train to be delayed.
Investigation
The AP had been issued with 2 STED forms, covering all lines and ¼ mile earthing locations, and had beenbriefed on installing the 2 mile limit DEP earths, then the ¼ mile earths to cover the superseded Isolation.
He made an error in identifying the correct DEP location, as all paperwork and briefing had been carried outcorrectly. No other external factors found to have caused the issue.
Recommendations
AP was stood down and For Cause tested, all clear. Client investigation still to be closed. The Client works were stood down following the incident, and ISS have increased our monitoring of certain OLE sites for this client.

You’ll be aware we have a telematics system in our Company and hired vehicles, and we are monitoring closely – not only speeding but also driving behaviour.
We have taken steps recently to remove the authority to drive from some staff because of their speeding, and also have terminated the employment of someone on probation, due to their unauthorised use of a company vehicle, and due to excessive speeding.
Please treat the Company vehicles with respect.

This morning a piece of glass had been removed from its fixing and was left standing on the platform. Rather than waiting for the relevant staff, Pete used his own tools to tighten up some of the other panels and then moved the pane of glass into his van and took it to Queens Street. I hope you agree Pete went over and beyond his duties in making the area safe and bringing stuff back to the depot on behalf of Kam. Not sure who he works for, is it ISS, but it may be courteous to let them know how helpful he has been.

World Mental Health Day – 10th October 2018
Presented by the World Federation of Mental Health, World Mental Health Day will be on October 10, 2018. The goal is to help raise mental health awareness. Each of us can make a contribution to ensure that people dealing with problems concerning mental health can live better lives with dignity.
The Dilemma
Every year one adult in four, along with one child in ten, will have a mental health issue. These conditions can profoundly affect literally millions of lives, affecting the capability of these individuals to make it through the day, to sustain relationships, and to maintain work.The stigma attached to mental health causes a damaging, albeit ill-informed, attitude, making it more difficult for those affected to pursue help. According to UK estimates, only about one-fourth of those with mental health problems undergo ongoing treatment. By stark contrast, the vast majority of those affected with these problems are faced with a variety of issues, ranging from isolation to uncertainty on where to get help or information, to relying on the informal support of family, friends or colleagues.
Raising Awareness
The best way to deal with this stigma is through facts and a better understanding of mental health problems. From identifying the causes, pinpointing solutions, and ultimately recognizing that we are really dealing with medical issues. For your information, we have updated our Fundamental Facts Guide.
