2020 Annual Report
Instructions & Scoring Rubric
A. General Instructions
Chapters should complete the AR outline that corresponds to their Council affiliation (IFC,
MGC, Panhellenic, PFC).
Document only events that occurred between July 1, 2019 and May 18, 2020.
Programming categories have two sections:
i. Ranking Questions – used to determine the chapter’s Star Ranking
ii. Awards Questions – added to the overall Programming category score, which
determines the recipients of Programming Achievement Awards. Awards
questions are optional and will not have an impact upon a chapter’s Star Ranking
Verified by OFSA – the OFSA staff is responsible for verifying your chapter’s completion
of these questions.

Supporting documentation should clearly show proof the chapter completed the question.
Supporting documentation can come in many forms: meeting minutes, photos, printed
programs, e-mail messages, flyers and so on. Be creative, but not inventive.
Attendance – chapters are required to utilize getInvolved Event Pass to create and submit
attendance lists. Event Pass instructions will be provided.
If your chapter has made a strategic decision NOT to plan/participate/attend a specific
program or question contact your OFSA Liaison to discuss. The chapter may still be able
to receive credit if you provide a rational or an acceptable alternative program.
B. Evaluation & Scoring
Ranking Questions are worth 5 points
Award Questions are worth 10 points
Programming Achievement Awards are earned when a chapter scores 90% of the total
available points in each specific programming category.
Category Rankings are based upon the questions/points from the Ranking section of each
Programming Category
i. Four Star Chapter 90% - 100% of the total ranking points
ii. Three Star Chapter 80% - 89% of the total ranking points
iii. Two Star Chapter 70% - 79% of the total ranking points
iv. No Stars 69% or less of the total ranking points
C. Final Submission Dates (unless otherwise dated):
Fall semester submission date Friday, December 27, 2019
Spring semester submission date, Monday, May 18, 2020.

Academic Achievement
Purpose: The chapter has a well-developed scholarship program that contributes outstanding individual
and overall chapter academic achievement.
Ranking Questions
1. The Total Chapter GPA is at or above a 2.50.

3. The chapter meets or exceeds the University’s four year graduation rate.

4. The Scholarship Chair provides an academic program to the New Member class no later than
Oct. 7/Feb. 24. The program must include the following components:
● Individual academic performance expectations
● Overview of University academic resources
● Overview of support measures provided by the chapter
● Accountability measures for chapter members that do not meet academic expectations

5. The chapter provides an academic accountability framework for members whose cumulative GPA
falls below a 2.50. The framework includes (but is not limited to) the list below. Provide the page

Advisor Engagement
Purpose: The chapter recognizes that an engaged chapter advisor/advisory board is critical to the overall
success of the chapter.
Ranking Questions
1. The chapter advisor/advisory board member participates in the annual OFSA Advisor Academy.

2. The chapter advisor/advisory board member attends Convention/Regional Conference OR National
Advisor Training. Provide photo of advisor’s convention nametag.

official position description, Executive officers chapter governance responsibilities, Policy
review and Goal setting

Brotherhood/Sisterhood Development
Purpose: The chapter recognizes that activities designed to build the bonds of brother/sisterhood are
critical to the overall success of the chapter.
Ranking Questions
1. The chapter performs one aspect of its ritual at least once per semester (i.e., private/members only
ceremonies like the Pledge Ceremony, Initiation Ritual, Officer Installation Ceremony, etc.)

2. The chapter (50%) holds at least one Brother/Sisterhood event each semester. Provide event

4. The chapter invites a panel of alumni to attend a chapter meeting to discuss lifetime membership

5. The chapter sponsors a program that celebrates the national organization’s cultural heritage OR
hosts an annual Founders Day celebration on or around the national organization’s founding date.

Chapter Management
Purpose: The chapter is organized and run in an efficient manner. All inter/national policies and
procedures are followed.
Ranking Questions
1. Chapter minutes are recorded properly, consistently and distributed in a timely manner. Provide
minutes from one meeting in (F19) due 12/27/19 and one meeting in (S20) due 5/18/20

2. The chapter is in Good Standing with OFSA for at least 12 weeks each semester.

3. A Membership contract is signed annually (at the start of each academic year) by all chapter
members. The contract must include the following components:

4. The chapter creates and distributes a comprehensive chapter calendar each semester. The
calendar must include the following:

6. The chapter utilizes free Task Management software to keep track of Annual Report Tasks/Dates

7. The chapter utilizes the University Encumbrance Policy. The encumbrance language is included in
the Financial/Membership contract (see question 4). Copies of the chapter’s executed

Community Service
Purpose: The chapter is involved and supportive of the university and surrounding community.
Ranking Questions
1. The chapter completes an average of five (5) hours of service per member (initiates and new

Purpose: The chapter administers a well-developed education program for the newest members of the
chapter. The program promotes fraternity education as well as personal development. All percentages are
based upon the number of students that accepted invitations to join.

conduct). Provide an email from the national office that verifies the chapter has started the
semester in Good Standing. Due Sept 1/Jan 20
