Thank you for choosing to use our online and phone service. This service is an easyway for you to manage your Premium Bonds and other selected accounts andinvestments with NS&I.

To get started, just complete and sign the form overleaf and return it to us at theabove address. Please make sure you complete the form in full.

Once we’ve received your completed form, we’ll send you a unique identificationnumber, known as your ‘NS&I number’. We’ll also send you a temporary password –for your security, this will arrive separately. You can start to use the service whenyou’ve received both your NS&I number and password. In the meantime you canstill deal with us by post if you need to buy more Bonds or cash any in.

Important points to notel If you already have an NS&I number and password – for example if you

have an NS&I Direct Saver or Direct ISA – you’re already registered to useour online and phone service, so you don’t need to complete this form. Youcan manage your Premium Bonds online and by phone straight away usingyour existing password and security details. (If you can’t view your PremiumBonds when you next log in, it’s possible that there’s a slight difference inyour personal details on our records. If so, you will need to complete andreturn this form and we’ll match up your records.)

· When you have received your NS&I number and temporary password, youcan start using the service. You’ll then be able to choose options such asreceiving prizes by direct credit to your bank account with notification byemail. This doesn’t happen automatically – you will need to log in or call usto select these options.
